Correction of first web space deficiency in congenital deformities of the hand with the pseudokite flap.

The authors describe a new flap to enlarge or create a first web in patients with congenital deficiencies of the hand. This lozenge-shaped flap is harvested from the dorsoradial aspect of the index finger based on a narrow proximal skin bridge protecting its axial vascularization. A dorsal skin graft of the donor site is avoided by closure using a rhomboid flap. Procedures in 16 patients were performed and reviewed; the only complication was insertion of a small split-thickness graft in five patients to avoid tension at the metacarpophalangeal joint level. The flap lengthened the web fold an a

A new protocol for the treatment of hand deformities in recessive dystrophic epidermolysis bullosa (13 cases).

The recessive form of dystrophic epidermolysis bullosa creates severe hand deformities with disabling functional limitations in the main daily activities. Typically, the thumb is contracted in adduction, the first web space is obliterated, the palm and digits are contracted in flexion and interdigital spaces are lost (pseudo-syndactyly). In this paper, we present our experience with a protocol based on the association of various internationally developed techniques: brachial plexus anesthesia with ketamine sedation, dynamic splinting and coverage of the wounds with allogenic keratinocytes shee

Right-sided Poland syndrome with no classical hand deformity: A case report.

Poland syndrome (PS) is a rare congenital condition characterized by the developmental anomaly of the chest wall, and classically presents with ipsilateral agenesis/hypoplasia of sternocostal head of pectoralis major which remains as the essential feature of the condition. It may or may not be associated with ipsilateral limb abnormalities. Diagnosis is mainly clinical. Imaging modalities like chest X-Ray, CT chest, US, and 3D-VRT CT can be used to know about the extent of involvement of this anomaly. We hereby present a case of a 25-year-old male who visited our hospital with chief complaint

A technique of hand digit reconstruction using scarred and deformed donor segments.

BACKGROUND: Hand digit reconstruction with transposition of hand segments in cases of hand injury is not used frequently by surgeons, at least in part because of potential ischemic complications after cicatricial changes in tissues. The aim of this study was to investigate the opportunities for expanding the set of indications for hand digit reconstruction using scarred and deformed donor segments. METHODS: We developed a technique of ischemic preconditioning and preforming the segment before transfer to improve macro-circulation and micro-circulation in scarred donor segments with a view to

Single-stage management of a neglected radial club hand deformity in an adult.

Radial club hand is an intercalary congenital deformity involving the forearm, wrist and hand. A congenital absence of radius (partial or complete) and ulnar bowing are classical radiographic abnormalities seen in this condition. This deformity is usually treated surgically in infants and young children but the management of this problem in an adult is complex and challenging. We present a neglected case of an adult with severe and rigid deformity that was successfully treated by one-stage correction involving ulnar osteotomy and wrist arthrodesis, simultaneously.

Flexor origin release and functional prehension in adult spastic hand deformity.

Eighteen adult patients with spastic hand deformities underwent release of the flexor-pronator origin and step-cut lengthening of the flexor pollicis longus to improve prehensile hand function. The goal of surgery was to provide active grasp release in order to improve hand function. Using a previously published grading scale, patients were graded before and after operation. At follow-up after an average of 35 months (range 24-64 months), all had improved by at least two functional levels.

Dorsal skin and fingernails on the volar aspect of the hand: an unusual anatomic deformity.

A very unusual hand malformation is reported in which dorsal skin and nails exist on the volar aspect of the hands. This patient also exhibits mental retardation and microcephaly as well as a partial deletion of chromosome 6. Fifty percent of a small series of patients previously reviewed who presented with chromosome 6 abnormalities demonstrated abnormalities of the hands. Unusual deformities of the hands associated with mental retardation and microcephaly may be related to an abnormality of chromosome 6.

Split-hand and split-foot deformity inherited as an autosomal recessive trait.

A intermarried consanguineous family with split-hand and -foot deformity occurring in two sibship is presented. Both the sibship resulted from marriage between first cousins. This report, together with those of Ray (1960) and Freire-Maia (1971), further demonstrates that split-hand and -foot deformity can be inherited as an autosomal recessive trait.

Pathogenesis and management of deformities of the elbow, wrist, and hand in late neonatal brachial plexus palsy.

The incidence of neonatal brachial plexus palsy (NBPP) has remained relatively stable despite awareness of the problem and improved obstetric techniques. Deformities of the forearm and hand can result either from untreated NBPP or following early microsurgical nerve reconstruction. These deformities include limb length discrepancy, flexion contracture of the elbow, supination or pronation contractures of the forearm, ulnar deviation of the wrist, and varying types of finger paralysis. The treatment options for these deformities consist of soft tissue releases, corrective osteotomies, tendon tr

[Value of a new first web space reconstruction in congenital hand deformities. A study of 54 patients].

INTRODUCTION: Only a few studies are available in the literature on the specific problem of the congenital contracture or absence of the first web in hand deformities, and first web reconstruction. MATERIAL AND METHODS: Sixty-seven first web plasties in 54 patients were retrospectively analyzed: they included 44 cases of classical plasties, i.e., Z-plasty (21 cases); dorsal flap plasty of the index (19 cases), thumb (1 case), or the hand (10 cases), and 16 cases of' pseudo-kite flap. RESULTS: The results were difficult to assess, as the syndromes were dissimilar and the deformities were not

Prevalence of mutilans-like hand deformities in patients with seropositive rheumatoid arthritis. A prospective 20-year study.

This study examined radiographically the prevalence of arthritis mutilans hand deformities in an inception cohort of 68 rheumatoid arthritis (RA) patients. Hand deformities of 103 RF-positive RA patients were evaluated after 8 years, 83 patients after 15 years and 68 patients 20 years after entry. The grade of destruction in the hand joints was assessed by the Larsen method and Larsen scores of 0-50 were determined for both PIP (+IP) and MCP joints. At the end point, 3 patients had Larsen scores > or =40 for both PIP and MCP joints. These three patients had severe resorption in most of the fin

Outcome analysis, including patient and parental satisfaction, regarding nonvascularized free toe phalanx transfer in congenital hand deformities.

The therapy for congenital hand malformations, especially in symbrachydactyly and constriction ring syndromes, is challenging. Between 1975 and 1995, 20 children with congenital hand deformities underwent reconstruction by 56 nonvascularized free toe phalanx transfers. The average age at initial surgery was 4.8 (range, 0.5 to 22) years. Retrospectively, the children were examined after an average of 3.5 (range, 1.5 to 17.6) years for function of the hand, transplanted phalanx growth, assessment for the epiphyseal plate, and assessment for psychologic performance with their parents. Donor-site

The abdominal flap using scarred skin in the treatment of postburn hand deformities of severe burn patients.

PURPOSE: To present a random-pattern flap technique using scarred skin from the abdominal wall to cover the defect after burn scar was released from the hand and the scarred skin raised from the hand was transferred to the abdominal wall to cover the flap-donor area in patients who suffered from severe burns involving the entire body and had no available healthy skin to be used for coverage. METHODS: Ten hands of 7 patients (5 males, 2 females) were treated in our department between April 1994 and February 2001. The total body surface area involved with second-or third-degree burns was 85% to

Assessment of mutilans-like hand deformities in chronic inflammatory joint diseases. A radiographic study of 52 patients.

OBJECTIVES: To evaluate patients with mutilans-like hand deformities in chronic inflammatory joint diseases and to determine radiographic scoring systems for arthritis mutilans (AM). METHODS: A total of 52 patients with severe hand deformities were collected during 1997. A Larsen hand score of 0-110 was formed to describe destruction of the hand joints. Secondly, each ray of the hand was assessed individually by summing the Larsen grade of the wrist and the grades of the MCP and PIP joints. When the sum of these grades was > or = 13, the finger was considered to be mutilated. A mutilans hand

Soft object deformation monitoring and learning for model-based robotic hand manipulation.

This paper discusses the design and implementation of a framework that automatically extracts and monitors the shape deformations of soft objects from a video sequence and maps them with force measurements with the goal of providing the necessary information to the controller of a robotic hand to ensure safe model-based deformable object manipulation. Measurements corresponding to the interaction force at the level of the fingertips and to the position of the fingertips of a three-finger robotic hand are associated with the contours of a deformed object tracked in a series of images using neur

Casting, splinting, and physical and occupational therapy of hand deformity and dysfunction in cerebral palsy.

The treatment of hand deformity and associated dysfunction is a major focus of physical and occupational therapy for people with CP, as poor grasp and manipulation has potential to impact on many aspects of daily life. To assist therapists in analyzing patterns of movement of the wrist, finger, and thumb musculature at rest and during functional activities, five patterns of deformity commonly seen in the hypertonic hand are described. Interventions that impact on hypertonicity and associated contracture and that facilitate functional use of the hand in the presence of these deformities are dis
