Treatment of the Thumb in the Rheumatoid Hand.

Deformities of the thumb in the context of rheumatoid arthritis are not uncommon and lead to early functional limitations due to the great importance of the first ray in almost all gripping functions. The various deformities of the thumb were already classified by Nalebuff in 1968, and this classification is still valid today. The treatment of a thumb deformity depends on the functional limitations, the number of affected joints, and the patient's requirements. In addition to tendon reconstructions, arthrodesis and arthroplasty are used. The decision is made individually depending on the statu

Fowler Central Slip Tenotomy or Spiral Oblique Retinacular Ligament Reconstruction? A Cadaveric Biomechanical Study in Swan-Neck Deformity.

Background: The goal of this study is to biomechanically compare Fowler central slip tenotomy with spiral oblique retinacular ligament (SORL) reconstruction in correcting a chronic mallet deformity as part of a swan-neck deformity. Methods: We used 24 human cadaver fingers from 6 hands. Mallet finger and swan-neck deformities were created; then, Fowler tenotomy was done on one group including 3 hands with 12 fingers, and SORL reconstruction was done on the others. Results: During simulated finger extension, there was no significant difference between the 2 techniques in correcting the distal i

Splinting and hand exercise for three common hand deformities in rheumatoid arthritis: a clinical perspective.

PURPOSE OF REVIEW: Hand deformities (ulnar drift, swan neck and boutonniere) are prevalent in rheumatoid arthritis and develop early in the disease process. These deformities cause significant functional impairment and impact quality of life. This review will provide a clinical perspective using the most recent evidence regarding the role of splinting and hand exercise in their management. The pathomechanics and functional consequences of these deformities are also described. RECENT FINDINGS: Patients tend to under-report their hand problems and are often reluctant to seek treatment for their

[Pachydermodactyly: a report of two cases].

Pachydermodactyly is a superficial benign, symmetrical and painless fibromatosis, characterized by a swelling of the proximal interphalangeal joints of the fingers. Histological examination shows epidermal hyperplasia increase of dermal collagen bundles and an increased number of fibroblasts. We report two cases of pachydermodactyly in male teenagers. The aetiology remains unknown. Pachydermodactyly has to be distinguished from knuckle pads.

Acquired ulnar clubhand resulting from neonatal osteomyelitis.

Ulnar clubhand is a rare deformity. In a patient with postnatal osteomyelitis of the ulna, the wrist had an ulnar deviation with marked ulnar slope of the radius. Good hand function precluded surgical correction.

A case of Ollier's disease of the hand.

Enchondroma is a benign growth of cartilage arising in the bone metaphysis as a solitary or multiple primary lesions. The form of multiple enchondromatosis with unilateral predominance is termed Ollier's disease. We have recently treated a case of Ollier's disease with the chief complaint of deformity of the left hand. The patient was an 11-year-old boy. Radiographic examination showed honeycombed clear spaces in the metaphyses of the middle and proximal phalanges of the left ring and little fingers as well as of the fourth and fifth metacarpals, and thinning of the cortex of these bones, but

The burned small finger.

Postburn deformities are common in the small finger. A boutonniere-like deformity may develop when no injury has been noted in the central slip of the extensor tendon. Attention to the anatomic differences of the fifth digit and principles of graft contraction make this deformity preventable. In the long-standing burn hyperextension deformity of the metacarpophalangeal joint, recurrence of the deformity may occur after reconstruction. Evaluation of extrinsic extensor tightness may indicate an alteration in the soft tissue-to-skeleton relationship. Composite tissue expansion of the tendon and o

Reconstruction of the rheumatoid thumb.

Most rheumatoid patients will present with one or more thumb deformities at some stage of their clinical history. The goal of treatment is restoration and maintenance of stable and painless motion. Treatment is based on the type and stage of the deformity. The boutonniere thumb is the most common deformity. Metacarpophalangeal arthrodesis is preferred for isolated metacarpophalangeal involvement. For advanced cases in a low-demand patient, metacarpophalangeal arthroplasty with interphalangeal arthrodesis is performed. In the higher demand hand with an uninvolved carpometacarpal joint, arthrode

Reconstruction of severe burn deformities of the upper extremity.

A case of severe post burn deformity involving multiple joints in both upper extremities is presented. Principles of rehabilitation and surgical reconstruction are detailed which demonstrate the importance of a close working relationship between surgeon, patient and therapist. This case serves as an excellent example of how a systematic approach to severe burn deformities can transform disaster into a favorable functional outcome.

Symmetric hyperphalangism of the index finger in the palatodigital syndrome: a case report.

A case of palatodigital syndrome is added to the eight cases in the literature. The accessory phalanx between the index metacarpal and proximal phalanx produces a significant deformity of extreme radial deviation with mild supination. This deformity was treated by rotation and stabilization of the accessory bone. Follow-up at 1 year shows correction of the radial deviation has been maintained and that the finger has grown appropriately.

Acquired radial club hand. A case report.

An unusual case of acquired radial club hand is presented. It was almost indistinguishable from the congenital variety, but arose from a mismanaged cutaneous infection of the wrist. Surgical treatment resulted in a functionally and cosmetically acceptable hand.

Swan-Neck Correction by FDS Tenodesis and Dynamic Lateral Band Volar Relocation in the Setting of Cerebral Palsy.

While there are many proposed surgical treatment options for the correction of swan-neck deformities, none are perfect. We describe a partial flexor digitorum superficialis tenodesis that combines both a static volar plate with a dynamic oblique retinacular ligament vector reconstruction. This is performed through a single, short mid-lateral incision and requires no tendon grafts. The protected early active exercises are encouraged postoperatively, and our long-term results have been promising. The technique was designed for children with cerebral palsy, but the indications have since expanded

An unusual cause of swan neck deformity of the fingers.

OBJECTIVE: To describe a patient who presented with nonfixed swan neck deformity of the fingers and generalized body aches. DESIGN: Case report. SETTING: Tertiary care teaching hospital. PATIENT: A 38-year-old woman who presented with swan neck deformity of the fingers. MAIN OUTCOME MEASURES: Electromyographic finding and electromyographic and clinical response to intravenous immunoglobulin. RESULTS: Needle electromyography revealed continuous motor unit activity in the 50- to 70-Hz frequency during the resting state for all the muscles sampled, which suggests the possibility of neuromyot
