Risk factors in distal interphalangeal joint arthrodesis in the hand: a retrospective study of 173 cases.

In this retrospective study we aimed to analyse the risk factors for complications after different methods of distal interphalangeal arthrodesis in the hand. Forty-four per cent were treated with K-wire/cerclage fixation, 46% with X-fuse® implants (Stryker GmbH, Selzach, Switzerland) and 10% with headless compression screws (HBS®-screw, KLS Martin Group, Tuttlingen, Germany). The median follow-up was 16 weeks (range 6-224). The overall complication incidence was 44% (minor complications 29% and major 15%). The logistic regression showed that osteoarthritis, revisional arthrodesis and smoking h

Increasing synovitis and bone marrow lesions are associated with incident joint tenderness in hand osteoarthritis.

OBJECTIVES: To explore whether changes of MRI-defined synovitis and bone marrow lesions (BMLs) are related to changes in joint tenderness in a 5-year longitudinal study of the Oslo hand osteoarthritis (OA) cohort. METHODS: We included 70 patients (63 women, mean (SD) age 67.9 (5.5) years). BMLs and contrast-enhanced synovitis in the distal and proximal interphalangeal joints were evaluated on 0-3 scales in n=69 and n=48 patients, respectively. Among joints without tenderness at baseline, we explored whether increasing/incident synovitis and BMLs were associated with incident joint tenderness

Percutaneous Distraction Pinning for Metacarpophalangeal Joint Stabilization After Blast or Crush Injuries of the Hand.

BACKGROUND: Unstable, severely comminuted fractures of the metacarpophalangeal (MCP) joint are difficult to treat. Closed treatment and casting of these fractures often fail to maintain proper alignment and impede wound care where concomitant open injuries such as gunshot wounds are present. Conventional pinning or plating techniques are not feasible if extensive bone loss and comminution are present. A distraction pinning technique represents a potential alternative, but results with this approach, to our knowledge, have not been reported. QUESTIONS/PURPOSES: The purposes of this study were

Assessment of malalignment at the metacarpophalangeal joint of the rheumatoid hand using three-dimensional computed tomogram.

OBJECTIVE: For the assessment of rheumatoid hand, three-dimensional (3D) malalignment including flexed or rotational deformities and dislocation at the finger joint is one of the important findings. The objective of this study is to prove usefulness of 3D computed tomography (3DCT) for the accurate measurement of palmoulnar flexion deformity at the metacarpophalangeal (MP) joint of the rheumatoid hand. METHODS: Swanson implant arthroplasty at the 2nd through the 5th MP joints was performed at 179 joints in 46 hands of 40 patients with rheumatoid arthritis. Pre-and postoperatively, evaluations

Short stature, ulnar deviation of hands with absent carpals and joint contractures: a new syndrome.

We report on a case of short stature with facial dysmorphism, ulnar deviation of hands and joint contractures. The X-rays of hands showed dislocation of the distal end of the radius and growth of the radius over the head of the ulna with total absence of carpal bones and hypoplastic tarsal bones. This is first report of such an association.

The significance of extensor tendon tethering and dorsal joint capsule tightening after injury to the hand.

This paper examines the clinical problem of extensor tendon tethering and/or dorsal joint capsule tightening secondary to hand injury. One hundred and forty-one patients were examined 13 to 51 months after hand injuries of varying severity. Fifty-six patients (40%) had suffered simple and eighty-five patients (60%) complex injuries. Seventy-four (52%) of the 141 patients had no extensor tendon tethering and/or dorsal joint capsule tightening. In 30 (21%), the extensor tendon tethering and/or dorsal joint capsule tightening was considered to be "obvious" in that it was easily seen on examinatio

Hand replantations following complete amputations at the wrist joint: first experiences in Hanoi, Vietnam.

Five completely amputated hands at the level of the wrist joint were replanted. The five patients were all men, ranging from 19 to 31 years and the amputations were all the result of knife injuries. All the revascularizations were successful. Secondary tenolysis was required in one case. Postoperative functional results compared to the uninjured hand were evaluated according to five main criteria: (1) the patient's overall satisfaction with the hand; (2) recovery of flexor and extensor function of the thumb and fingers; (3) recovery of thumb opposition; (4) recovery of sensitivity in the media

Free vascularized toe joint transfer to the hand. A technique for simultaneous reconstruction of the soft tissue.

Twelve patients underwent reconstruction of injured finger joints using our technique of a vascularized transfer of the second toe proximal interphalangeal joint. The age of the patients at operation ranged from 7 to 47 years and the postoperative follow-up was 9 to 48 months. All the joint transfers survived and united with resolution of the preoperative joint pain, deformity and instability. The average range of motion of the reconstructed joints was 59 degrees in the proximal interphalangeal and 54 degrees in the metacarpophalangeal joints. No patient complained of pain or functional defici

Coordination of multi-joint arm movements in cerebellar ataxia: analysis of hand and angular kinematics.

Kinematic abnormalities of fast multijoint movements in cerebellar ataxia include abnormally increased curvature of hand trajectories and an increased hand path and are thought to originate from an impairment in generating appropriate levels of muscle torques to support normal coordination between shoulder and elbow joints. Such a mechanism predicts that kinematic abnormalities are pronounced when fast movements are performed and large muscular torques are required. Experimental evidence that systematically explores the effects of increasing movement velocities on movement kinematics in cerebe

3D Hand Pose Estimation via Articulated Anchor-to-Joint 3D Local Regressors.

In this paper, we propose to address monocular 3D hand pose estimation from a single RGB or depth image via articulated anchor-to-joint 3D local regressors, in form of A2J-Transformer+. The key idea is to make the local regressors (i.e., anchor points) in 3D space be aware of hand's local fine details and global articulated context jointly, to facilitate predicting their 3D offsets toward hand joints with linear weighted aggregation for joint localization. Our intuition is that, local fine details help to estimate accurate offset but may suffer from the issues including serious occlusion, conf

Long-Term Survival of Proximal Interphalangeal Joint Denervation of the Hand: Time-to-Event Analysis.

PURPOSE: The primary aim of this study was to assess the long-term survival rate of proximal interphalangeal (PIP) joint denervation, defined as the duration until revision with a salvage procedure. The secondary aims were to evaluate pain relief, patient satisfaction, progression of osteoarthritis, and factors associated with treatment failure. METHODS: A multicenter retrospective cohort study was conducted, including patients aged ≥18 years with PIP joint arthropathy undergoing denervation surgery. Revision surgery was the endpoint for survival analysis. Pain levels, patient satisfaction, a

Wrist Joint Restriction: Impact on Foot Pressure, Center of Gravity, and the Role of the Dominant Hand.

Background: Wrist immobilization is a common clinical intervention for wrist injuries; however, its repercussions on gait parameters and plantar support have not been extensively investigated. Objectives: The purpose of the study was to determine whether wrist immobilization causes alterations in foot pressure and center of gravity, considering hand dominance and visual conditions (eyes open or closed). Methods: The research experiment was conducted using the PodoPrint S4 platform. Basic descriptive statistics were calculated to summarize the variables. Additionally, in the mixed linear model

The Microbiology, Management and Outcomes of Native Joint Septic Arthritis of the Hand in Adults of Southeast Queensland.

Background: Native joint septic arthritis (NJSA) of the hand is poorly studied. Management guidelines are extrapolated from large joint infections despite differences in epidemiology, anatomy and aetiology. Limited evidence suggests shorter courses of antibiotics can achieve outcomes comparable to longer courses. The aim of our study is to characterise the presentation, management and outcomes of NJSA of the hand in an adult cohort from Southeast Queensland (SEQ) and determine if the route or duration of antibiotics affected the outcome. Our cohort was compared to those in the literature to de

Intra-limb joint coordination measures of upper limb and hand movements: A systematic review.

BACKGROUND: People with central nervous system disorders typically have difficulties in coordination of the upper limb and hand movements, which significantly impairs their activities of daily living. Laboratory-based measures can provide quantitative and objective information about intra-limb coordination to aid the rehabilitation process of this population. However, there is currently no comprehensive review of laboratory-based measures. RESEARCH QUESTIONS: The aim of this review was to identify and summarize laboratory-based intra-limb coordination measures for different upper limb and han

EMG-based Simultaneous Estimations of Joint Angle and Torque during Hand Interactions with Environments.

It is necessary to control contact force through modulation of joint stiffness in addition to the position of our limb when manipulating an object. This is achieved by contracting the agonist muscles in an appropriate magnitude, as well as, balancing it with contraction of the antagonist muscles. Here we develop a decoding technique that estimates both the position and torque of a joint of the limb in interaction with an environment based on activities of the agonist-antagonistic muscle pairs using electromyography in real time. The long short-term memory (LSTM) network that is capable of lear

Functional outcomes after arthroplasty of the distal radioulnar joint and hand therapy: a case series.

The purpose was to present a therapy protocol for use after implantation of an ulnar head endoprosthesis and to describe the functional outcomes after hand therapy. This is a retrospective review of a series of eight patients treated with a specified therapy protocol after ulnar head resection and implant arthroplasty. Marked improvements in pain and function were reported, though some pain with exertion remained. Two patients were on worker's compensation and both have returned to their premorbid work status. Functional use of the extremity was achieved by two to six weeks, with a mean of fou

Reflex responsiveness of a human hand muscle when controlling isometric force and joint position.

OBJECTIVE: This study compared reflex responsiveness of the first dorsal interosseus muscle during two tasks that employ different strategies to stabilize the finger while exerting the same net muscle torque. METHODS: Healthy human subjects performed two motor tasks that involved either pushing up against a rigid restraint to exert a constant isometric force equal to 20% of maximum or maintaining a constant angle at the metacarpophalangeal joint while supporting an equivalent inertial load. Each task consisted of six 40-s contractions during which electrical and mechanical stimuli were delive
