Frequency weighting for vibration-induced white finger compatible with exposure-response models.

An analysis has been performed to derive a frequency weighting for the development of vibration-induced white finger (VWF). It employs a model to compare health risks for pairs of population groups that are selected to have similar health outcomes from operating power tools or machines with markedly different acceleration spectra (rock drills, chain saws, pavement breakers and motorcycles). The model defines the Relative Risk, RR(f(trial)), which is constructed from the ratio of daily exposures and includes a trial frequency weighting that is applied to the acceleration spectra. The trial freq

Extracellular signal-regulated kinase activation and endothelin-1 production in human endothelial cells exposed to vibration.

Hand-arm vibration syndrome is a vascular disease of occupational origin and a form of secondary Raynaud's phenomenon. Chronic exposure to hand-held vibrating tools may cause endothelial injury. This study investigates the biomechanical forces involved in the transduction of fluid vibration in the endothelium. Human endothelial cells were exposed to direct vibration and rapid low-volume fluid oscillation. Rapid low-volume fluid oscillation was used to simulate the effects of vibration by generating defined temporal gradients in fluid shear stress across an endothelial monolayer. Extracellular

The Impact of Sensory, Motor and Pain Impairments on Patient- Reported and Performance Based Function in Carpal Tunnel Syndrome.

BACKGROUND: Research has suggested that persistent sensory and motor impairments predominate the symptoms experienced by patients with carpal tunnel syndrome (CTS); with intermittent pain symptoms, being less predominant. OBJECTIVE: The study aims to determine the relative contribution of sensory, motor and pain impairments as contributors to patient-report or performance-based hand function. METHODS: Fifty participants with a diagnosis of CTS confirmed by a hand surgeon and electrodiagnosis were evaluated on a single occasion. Impairments were measured for sensibility, pain and motor perfor

A longitudinal study of finger systolic blood pressure and exposure to hand-transmitted vibration.

OBJECTIVES: To investigate prospectively the relation between vibration-induced white finger (VWF), exposure to hand-transmitted vibration (HTV) and the cold response of digital arteries in users of vibrating tools. METHODS: Two-hundred and sixteen HTV workers and 133 control men of the same companies underwent initially a medical examination and a standardised cold test with measurement of the change in finger systolic blood pressure (FSBP) after finger cooling from 30 to 10 degrees C. They were re-examined 1 year later. Tool vibration magnitudes were expressed as frequency-weighted and unwe

Negligent exposures to hand-transmitted vibration.

OBJECTIVES: If the negligence of an employer results in the disability in an employee, the employer is responsible, in whole or in part, for the disability. The employer is wholly responsible when the worker would not have developed the disability if the employer had taken all reasonable preventative measures. The employer is only partly responsible if the worker would probably have developed some disability even if the employer had taken all reasonable precautions. The employer's responsibility may be estimated from the difference between the actual disability of the worker and the disability

Hand-arm vibration exposure of dentists.

OBJECTIVES: The use of dental handpieces exposes the dental personnel to high-frequency vibration. Dentists have been shown to have a high frequency of finger-related and other upper limb symptoms and a high prevalence of osteoarthrosis in the distal interphalangeal joints. METHODS: The vibration of 22 dental handpieces was measured during normal work with accelerometer. The weighted vibration according to the standard ISO 5349-1 and the total acceleration of high frequency vibration ("ultravibration") in the frequency range of 1.6-10 kHz were analyzed. In order to compare non-contact vibrati

Acute vibration increases alpha2C-adrenergic smooth muscle constriction and alters thermosensitivity of cutaneous arteries.

The vascular symptoms of hand-arm vibration syndrome, including cold-induced vasospasm, are in part mediated by increased sensitivity of cutaneous arteries to sympathetic stimulation. The goal of the present study was to use a rat tail model to analyze the effects of vibration on vascular function and alpha-adrenoceptor (AR) responsiveness. Rats were exposed to a single period of vibration (4 h, 125 Hz, constant acceleration 49 m/s2 root mean square). The physical or biodynamic response of the tail demonstrated increased transmissibility or resonance at this frequency, similar to that observed

Dupuytren's contracture and occupational exposure to hand-transmitted vibration.

AIMS: The relation between Dupuytren's contracture and occupational exposure to hand-transmitted vibration (HTV) has frequently been debated. We explored associations in a representative national sample of workers with well-characterised exposure to HTV. METHODS: We mailed a questionnaire to 21 201 subjects aged 16-64 years, selected at random from the age-sex registers of 34 general practices in Great Britain and to 993 subjects chosen randomly from military pay records, asking about occupational exposure to 39 sources of HTV and about fixed flexion contracture of the little or ring finger.

Outcome of the abduction external rotation test among manual and office workers.

Reactions during the Abduction External Rotation (AER) test were studied among 71 platers and 70 assemblers exposed to vibratory tools and manual work and 45 unexposed white collar workers. The frequency of positive neurological reactions was 31% among the platers, 6% among the assemblers, and 16% among the white collar workers. Platers reported the highest fatigue rates in the shoulder regions during the test. A positive association was noted between a pathological AER test and current problems in the neck/scapula or shoulder/upper arm regions, day- or night-time numbness in the hands, tensio

The correlation between work environment and the occurrence of cervicobrachial symptoms.

The correlation between symptoms from the neck and upper extremities and some individual and work-related factors was analyzed in 2814 industrial workers. Physical stress by type of job was the factor most strongly correlated with ongoing cervicobrachial symptoms. Symptoms from the neck and upper extremities were twice as common in workers who used vibrating hand tools. Mental stress at the onset of the symptoms was associated with an increased prevalence of trapezius myalgia and with lateral humeral epicondylitis and "radial tunnel syndrome" in the dominant arm. Women had about double the rat

Hand-arm vibration exposure and outcomes after cubital tunnel surgery.

BACKGROUND: Cubital tunnel syndrome is the second most common upper limb compressive neuropathy after carpal tunnel syndrome. Occupational hand-arm vibration exposure (HAV) has been linked to peripheral nerve compression, though a causal relationship remains unproven. Prior studies have shown that vibration exposure negatively affects outcomes following carpal tunnel surgery, but its impact on cubital tunnel surgery outcomes remains unclear. AIMS: This study aimed to evaluate the effect of previous HAV exposure on patient-reported outcome measures (PROMs) following cubital tunnel release. Sec

Blood biomarkers for occupational hand-arm vibration exposure.

Hand-arm vibration is a common occupational exposure that causes neurological impairment, myalgia, and vibration-induced Raynaud's phenomena or vibration white fingers (VWF). The pathological mechanism is largely unknown, though several mechanisms have been proposed, involving both immunological vascular damage and defective neural responses. The aim of this study was to test whether the substances interleukin-33 (IL-33), macrophage-derived chemokine (MDC), interleukin-10 (IL-10), endothelin-1 (ET-1), C-C motif chemokine ligand 20 (CCL20), calcitonin, and thromboxane (TXA2) changed before and

Syndromes from segmental vibration and nerve entrapment: observations on case definitions for carpal tunnel syndrome.

OBJECTIVES: The purpose of this paper is to assess the overlap and stability of two different case definitions of carpal tunnel syndrome CTS. The analysis considers the association between different case definitions and objective tests (sensory nerve conduction velocities, SNCVs and vibrotactile perception thresholds, TTS), and the natural history of CTS, in the context of two vibration-exposed cohorts. METHODS: Clinical CTS cases were defined in two ways: (1) by the study physician using fixed criteria, and; (2) by questionnaire and hand diagram. SNCV in median and ulnar nerves was measured

Carpal tunnel syndrome and occupational hand exposures: a Danish nationwide cohort study.

OBJECTIVES: The aim was to examine exposure-response relations between occupational hand exposures and carpal tunnel syndrome (CTS) and to compare the relation between surgery-treated and non-surgery-treated CTS. The secondary aim was to study sex-specific differences in exposure-response relations. METHODS: We conducted a nationwide register-based cohort study of all persons born in Denmark (1945-1994). During follow-up (2010-2013), we identified first-time events of CTS. Occupational hand exposure estimates the year before each follow-up year were obtained by linking individual occupational

Ulnar tunnel syndrome.

As our understanding of the anatomy of the ulnar tunnel has increased, so too has our ability to clinically predict the specific sites of compression in ulnar tunnel syndrome. Anatomic studies have described in detail the course of the ulnar nerve as it passes through the ulnar tunnel and have helped correlate symptoms with anatomic location. Although the most common cause of compression is from a ganglion, other space-occupying lesions, such as tumors, anomalous muscles, or a thrombosed ulnar artery, are important to consider in the initial evaluation of the patient. While conservative manage
