[The GeRMIQ-Curriculum: a Blueprint for a National Training Concept in Minimally Invasive and Robotic Surgery].

PURPOSE: Minimally invasive (MIS) and robot-assisted surgery (RAS) have revolutionised surgical practice and place high demands on knowledge and technical skills as well as structured training concepts. Currently, there is no comprehensive curriculum in Germany, which leads to insufficient quality of training, with corresponding consequences. MATERIALS AND METHODS: The GeRMIQ curriculum (German Robotic and Minimally Invasive Surgery Qualification) was developed to close this gap and create a national, standardised, and forward-looking program for basic surgical training. RESULTS: The12-month

Laparoscopic distal pancreatic resection.

Laparoscopic resection is not an established treatment for pancreatic tumors. Previous reports, mainly in Europe and Japan, have demonstrated the potential utility of laparoscopic distal pancreatectomy (LDP). However, few reports have been published from the United States. We instituted a pilot program to assess LDP. A total of 11 patients were included from December 2003 to December 2004. All patients were staged with preoperative endoscopic ultrasound and received vaccinations for possible splenectomy. The indications for surgery were as follows: neuroendocrine tumor (n = 7), unspecified tum

Hand-assisted technique facilitates preserving graft viability in laparoscopic donor nephrectomy.

BACKGROUND: To achieve patient safety and minimal operative invasion in living kidney donor nephrectomy, we have performed hand-assisted laparoscopic donor nephrectomy (HALDoN) since 2006. AIM: The aim of this study was to evaluate the utility and the technique of HALDoN. METHOD: We analyzed 72 donors who underwent HALDoN from February 2008-August 2011. RESULTS: Including 8/72 donors who underwent right nephrectomy, all subjects completed HALDoN without conversion to an open procedure. None of the recipients suffered delayed graft function or an ureteric problem. Knife-to-removal time (KRT)

Intercostal and Glissonian Pedicle Approach in Laparoscopic Liver Resection for Bilobar Tumors.

BACKGROUND: Bilobar liver metastasis is challenging for laparoscopic liver resection. Especially, subphrenic liver tumors in S7 or S8 are technically difficult to be resected out because the space is limited and the angle of instruments to lesions cannot be perpendicular.1,2 Major liver lobectomy is also challenging for laparoscopic liver resection. Glissonian pedicle approach has benefit that any variation of vascular and bile duct elements does not need to be considered in the hepatoduodenal ligament under the hilar plate. Glissonian pedicle approach is simpler and faster than individual dis

Mini-incision versus hand-assisted laparoscopic donor nephrectomy in living-donor kidney transplantation: A retrospective cohort study.

BACKGROUND: Increasing use of Living Donor Kidney Transplantation (LDKT) would decrease the discrepancy between patients awaiting transplantation and organ availability. Minimally invasive surgical approaches attempt to improve outcomes and foster living donation. This report compares outcomes of open minimal incision nephrectomy (Mini N) and a hand assisted laparoscopic nephrectomy (HALN). METHODS: This is a retrospective analysis of a prospectively maintained clinical database of LDKT using HALN or Mini N at a single institution between July 2007 and December 2015. Donor and recipient demog

Right Versus Left Laparoscopic Living-Donor Nephrectomy: A Meta-Analysis.

OBJECTIVES: We assessed the safety and efficacy of right versus left laparoscopic living-donor nephrectomy. Few clinical controlled studies have compared the right and left side, and most trials have a small sample number and varied results. A meta-analysis of published trials was performed to determine the effects of the 2 different approaches. MATERIALS AND METHODS: Major databases including Medline (PubMed), Embase, Ovid, and Cochrane were searched to identify studies comparing right and left laparoscopic living-donor nephrectomy (January 2000 to January 2014). Outcomes evaluated included

Short- and long-term outcomes of 486 consecutive laparoscopic splenectomy in a single institution.

Since its introduction in 1991, laparoscopic splenectomy (LS) has become the gold standard in elective spleen surgery in many centres. However, there still lack the report of long-term outcomes of LS with the large-scale cases. The aim of the present study was to analyze the short- and long-term outcomes of LS in a single institution over 16 years, and to compare the perioperative outcomes of totally laparoscopic splenectomy (TLS) and hand-assisted laparoscopic splenectomy (HALS) for splenomegaly.Between November 2002 and December 2018, 486 consecutive patients undergoing elective LS were enro

Anatomic Diversity Encountered During Laparoscopic Hand-Assisted Transperitoneal Donor Nephrectomy: A Case Report of Complete Caval Duplication.

INTRODUCTION: Laparoscopic donor nephrectomy is widely used to retrieve a kidney for transplantation. Preoperative evaluation of the donor is of crucial importance to the recipient. In particular, vascular anatomy should be assessed with the help of modern imaging modalities. We present a hand-assisted laparoscopic nephrectomy of a kidney donor with a complete duplex vena cava. CASE REPORT: A 40-year-old male patient was admitted to our clinic as a kidney donor for his 20-year-old son. After the preliminary tests, further imaging with the use of computerized tomographic angiography showed a c

Hand-assisted laparoscopic technique in the setting of complicated splenectomy: a 9-year experience.

BACKGROUND: Laparoscopic splenectomy (LS) is significantly more challenging in patients with supramassive splenomegaly and those with portal hypertension. We hypothesized that hand-assisted laparoscopic splenectomy (HALS) could facilitate the procedure in these patients. METHODS: Before October 2009, patients with supramassive splenomegaly and those with portal hypertension underwent LS. After October 2009, we routinely applied HALS in patients with these disorders. RESULTS: We compared the HALS group (n = 41) with the LS group (n = 45). There were no conversions to open surgery in the HALS

Equal short-term outcomes of intracorporeal mechanical gastrogastrostomy in laparoscopic pylorus-preserving gastrectomy for cT1N0 gastric cancer in the middle stomach compared with the extracorporeal hand-sewing method.

BACKGROUND: Intracorporeal mechanical gastrogastrostomy (IMG) techniques have recently been developed and their short-term safety was presented in their initial evaluation. However, whether they are comparable to extracorporeal hand-sewing gastrogastrostomy (EHG) remains unclear. The aim of the study is to establish the safety of IMG in totally laparoscopic pylorus-preserving gastrectomy (TLPPG) compared to EHG in laparoscopy-assisted pylorus-preserving gastrectomy (LAPPG). METHODS: We retrospectively analyzed the short-term outcomes of patients with middle-third early gastric cancer who unde

Schloffer's tumor: Case report and review of the literature.

INTRODUCTION: The so-called Schloffer tumor (ST) is a rare inflammatory pseudotumor. It usually appears several years after abdominal surgery or trauma. PRESENTATION OF CASE: A 32-year-old man was referred to our hospital complaining of a painful mass in the left hypochondrium, postprandial distension and a weight loss of about 14kg. He had had a left inguinal hernioplasty without mesh the previous year. Ultrasonography of the abdomen showed a 2cm×2cm hypoechoic lesion in contact with the abdominal wall. Computerized tomography of the abdomen showed a heterogeneous mass in the great omentum.

Safety and Efficacy of the Hand-Assisted Laparoscopic Approach for Repeat Hepatectomy: A Propensity Score Matching Analysis.

BACKGROUND: Laparoscopic liver surgery following previous resections poses significant challenges due to adhesions and altered anatomy. Hand-assisted laparoscopic liver resection (HALR) combines the tactile advantages of open surgery with minimally invasive techniques, potentially benefitting patients undergoing repeat hepatectomy. This study aims to assess the safety and efficacy of HALR for repeat hepatectomy and compare these outcomes with those of an open liver resection (OR). METHODS: A retrospective study included patients who underwent repeat hepatectomy via HALR or OR from January 201
