Removal of cranial springs after spring-mediated cranioplasty.

Cranial spring hardware is generally removed 3 months after placement for spring-mediated cranioplasty. Spring removal is performed as an outpatient procedure under general anesthesia in approximately 15 minutes through the incision locations of the index procedure. Herein, the authors provide a multimedia demonstration of cranial spring hardware removal after spring-mediated cranioplasty for sagittal craniosynostosis. The video can be found here: https://vimeo.com/511179695.

Extraction of cannulated percutaneous screw from scaphoid: a simplified technique.

Removal of a cannulated headless screw following scaphoid fixation, when necessary, can prove to be difficult due to loss of bony integrity and the shape of the screw. This can result in greater exposure and more extensive soft tissue dissection for removal of the hardware. We describe a simple technique for percutaneous extraction of this hardware that can be done reliably with no additional equipment.

Does instrumentation removal cause curve progression in patients with adolescent idiopathic scoliosis?

Adolescent idiopathic scoliosis (AIS) is usually instrumented using a posterior approach. Hardware removal may be performed for specific clinical reasons. Little data is available on whether removal influences curve magnitude. The aim of the paper is to evaluate the impact of instrumentation removal on curve progression, and the safety and efficacy of the procedure. We analyzed 59 patients who underwent instrumentation removal. Curve types, reasons for removal, period between procedures, and Cobb angles: at baseline, immediately after correction, after removal and in follow-up were evaluated.

A meta-analysis comparing complications of anterior versus dorsal osteotomy and plating for distal radial malunions.

This systematic review and meta-analysis compare the incidences of complications, reoperations and hardware removal between anterior and dorsal plating after corrective osteotomy for dorsally angulated distal radial malunions. A total of 403 patients were included; 253 patients underwent anterior corrective osteotomy and 150 underwent dorsal corrective osteotomy. Mean follow-up was 30 months. Anterior plating was associated with fewer reoperations (9% vs. 28%), less hardware removal (3% vs. 18%) and comparable major complications (5% vs. 6%) compared to dorsal plating. The adjusted model showe

Editorial Commentary: Locking Plate Removal Is Safe and May Improve Function 1 Year After Medial Opening Wedge High Tibial Osteotomy.

The development of angular stable locking late fixation in realignment osteotomy has resulted in the ability to be more aggressive with weight bearing and rehabilitation after high tibial osteotomy. One of the downsides of some of these plate fixators is the degree of soft-tissue irritation and discomfort that can be experienced by many patients, resulting in the need for hardware removal. Studies have shown greater than 50% of patients requiring hardware removal with no resulting loss of correction, although there may be cultural reasons for the higher number, not solely attributable to the s

Reoperation Rate Differences Between Open Reduction Internal Fixation and Primary Arthrodesis of Lisfranc Injuries.

BACKGROUND: Controversy persists as to whether Lisfranc injuries are best treated with open reduction internal fixation (ORIF) versus primary arthrodesis (PA). Reoperation rates certainly influence this debate, but prior studies are often confounded by inclusion of hardware removal as a complication rather than as a planned, staged procedure inherent to ORIF. The primary aim of this study was to evaluate whether reoperation rates, excluding planned hardware removal, differ between ORIF and PA. A secondary aim was to evaluate patient risk factors associated with reoperation after operative trea

Screw Diameter and Use of a Washer Affect Symptomatic Implant Removal Rates in Pediatric Medial Epicondyle Fractures.

OBJECTIVE: Medial epicondyle fractures are a common pediatric injury. When operative, cannulated partially threaded screws, with or without a washer, are commonly utilized. These implants may need to be removed after full healing if symptomatic. There is mixed evidence regarding the influence of a washer on rates of implant removal, and the influence of screw size has not been studied. We aim to determine the rate of symptomatic deep implant removal for each fixation type and identify factors associated with the need for removal. METHODS: This was an IRB-reviewed, retrospective, case-cohort s

Tibial interference screw removal following anterior cruciate ligament reconstruction.

A small number of patients developed pain and tenderness at the tibial tunnel following anterior cruciate ligament reconstruction. Twenty-three knees in 22 patients underwent removal of the tibial interference screw. Ten knees had a preoperative flexion contracture and underwent a concomitant procedure to address the loss of motion at the time of hardware removal. In the 13 knees with full extension, the interval between ligament reconstruction and screw removal averaged 16 months. Eleven of these knees also underwent arthroscopy, but no intra-articular causes of pain were identified. Roentgen

Mini-fragment plate fixation after olecranon osteotomy for distal humerus fractures.

PURPOSE: Although olecranon osteotomies are helpful for distal humerus visualization, traditional methods of fixation are commonly irritating for patients and require hardware removal. Recent studies have shown lower hardware removal rates for medullary screw constructs and 3.5-mm plates, but no studies have investigated the use of 2.7-mm plates for olecranon osteotomy fixation. The purpose of this study is to report on the outcomes of single 2.7-mm mini-fragment plate fixation of olecranon osteotomies for distal humerus intra-articular fractures. METHODS: Patients who sustained an intra-arti

Unexpected iatrogenic fracture of the femoral neck during subtrochanteric fracture fixation in a patient on bisphosphonate treatment for osteoporosis: Case report.

Osteoporotic patients being treated with bisphosphonates present an interesting dilemma when removing hardware such as dynamic hip screws "DHS". In this paper, we describe the case of a 66-year-old osteoporotic patient who was placed on long term bisphosphonate therapy after sustaining an intertrochanteric hip fracture which was stabilized with a DHS. She presented with a subtrochantric fracture on the ipsilateral side. She was planned for DHS removal and intramedullary nailing. Removal of the dynamic hip screw proved to be difficult, likely due to possible cold welding of the DHS to the barre

Hardware considerations in infection and nonunion management: When and how to revise the fixation.

The occurrence of both nonunion and fracture-related infection provides challenges for both the patient and the treating orthopaedic surgeon, with the potential need for complex reconstructive procedures to achieve union and/or eradicate infection. In addition to addressing the multiple different factors that often contribute to nonunion, surgeons are often forced to deal with difficult hardware issues at the time of revision surgery including infected hardware, loose or failing hardware, malaligned hardware, or inappropriate hardware constructs. This article reviews common causes of nonunions

Vertical Interfragmentary Doubled Suture for Displaced Patella Fractures: Sequential Compressive Tightening with Nice Knot.

In the treatment of displaced patella fractures, open reduction and internal fixation is essential for patellofemoral congruency and restoration of the knee extension mechanism. Various surgical techniques and materials can be used, and their clinical outcomes are favorable. However, soft-tissue and skin irritation, pain, and limited range of motion due to metallic hardware can occur, and removal of hardware such as screws and K-wire may be required after bony union. We present a vertical interfragmentary suture technique for patella fractures using sequential compressive tightening with the N

Three-Year Outcomes Following Hip Reconstruction in Cerebral Palsy: A Population-Based Analysis.

BACKGROUND: Pelvic reconstruction is commonly performed to manage hip displacement in patients with cerebral palsy (CP), yet population-based outcome data remain limited. Most existing studies are from single centers and have limited generalizability. Here, we present a national descriptive study outlining outcomes of hip reconstruction in patients with CP. METHODS: A retrospective cohort study was conducted using the TriNetX Research Network, a federated database of 163 million patients across 117 US healthcare organizations. Patients aged 1 to 25 years with CP who underwent hip reconstructi

Fracture of fusion mass following anterior cervical plate removal: Case report.

We present the case of a delayed pseudoarthrosis resulting from a fracture at the site of a radiographically confirmed anterior cervical fusion following plate removal. In this case, an anterior cervical plate was removed to allow for further surgery at a supra-adjacent level. A modicum of literature exists describing delayed fractures following hardware removal in thoracolumbar fusion constructs. The development of a fracture/pseudoarthrosis following hardware removal at the site of a radiographically confirmed anterior cervical fusion has not been previously reported. We describe the clinica

Risk Factors for Failure of Separated Instrument Removal: A Systematic Review and Meta-Analysis.

This study aimed to systematically evaluate the risk factors associated with the failure of separated instrument removal. A comprehensive search of PubMed, Embase and the Cochrane Library was performed to identify studies reporting on the success rates of separated instrument removal, covering the period from 1988 to 2025. The meta-analysis was performed using Stata 17.0. Seven studies encompassing 922 patients with separated instruments were included. The pooled success rate of separated instrument removal was 66%. Meta-analysis identified several significant risk factors for removal failure:

Patient-reported Outcomes of Adolescents Treated With Guided Growth to Correct Idiopathic Genu Valgum.

BACKGROUND: Guided growth is commonly used to treat frontal plane alignment deformities in skeletally immature individuals. Treatment aims are to correct the biomechanical loading of the knee and to avoid more invasive surgery that would be required after skeletal maturity. There is little published evidence of pain perception or functional limitations in this population. In addition, the intervention has the potential to worsen pain and function with hardware implantation, and symptoms may not fully resolve after removal. Understanding of pain and function limitations in this population is im

An Unexpected Journey: Migration of a Scoliosis Rod to the Groin.

Rod migration is a rare but potentially serious late complication of spinal instrumentation. A case of long-distance rod migration to the groin following anterior lumbar fusion for adolescent idiopathic scoliosis is presented. A 43-year-old female, who had undergone selective anterior lumbar fusion 20 years earlier, presented with progressive lumbar pain. Initial radiographs demonstrated screw loosening and rod dislodgment. Before the planned revision surgery, the patient re-presented with pain localized to the hip and groin. Imaging revealed distal migration of the rod toward the anterior asp

Staged screw removal in external plate fixation for distal tibial fractures: a finite element study on adaptive biomechanics.

High-energy distal tibial fractures present treatment challenges due to poor soft tissue coverage and risk of delayed union. External locking plates offer stability while preserving biology, yet the optimal strategy to dynamically modulate fixation rigidity in accordance with healing progression remains unclear. We constructed finite element models of a comminuted distal tibial fracture incorporating callus at four healing stages (0, 1, 3, and 6 months). Seven screw configurations and the effect of an intact fibula were evaluated under 800 N axial load. Outcome measures included fracture site

Complication rates in open versus arthroscopic ankle arthrodesis in patients with avascular necrosis of the talus.

BACKGROUND: Treatment strategies for talar avascular necrosis range from nonsurgical management to surgical interventions, including joint-sparing and joint-sacrificing procedures. However, no consensus exists on the optimal treatment approach, particularly in cases requiring ankle arthrodesis. PURPOSE: This study evaluated whether open ankle arthrodesis and arthroscopic ankle arthrodesis differ in complication rates when used for the treatment of talar avascular necrosis.
