Juvenile Incarceration and Health.

Addressing the health status and needs of incarcerated youth represents an issue at the nexus of juvenile justice reform and health care reform. Incarcerated youth face disproportionately higher morbidity and higher mortality compared to the general adolescent population. Dental health, reproductive health, and mental health needs are particularly high, likely as a result of lower access to care, engagement in high-risk behaviors, and underlying health disparities. Violence exposure and injury also contribute to the health disparities seen in this population. Further, juvenile incarceration it

Overcoming Barriers to Healthcare Reform: A Call to Action.

The success of healthcare reform hinges on policymaker, regulator, and administrator actions that shape policies at various levels. These policies can either facilitate or hinder the practice of healthcare professionals and collaborative work environments. It is imperative for all healthcare professionals to fully utilize their education and certification, as fostering an equitable workplace culture is vital for retaining staff and improving access to care. Using nurse practitioners (NPs) as an exemplar, this article aims to specify systemic barriers to healthcare reform and call for policymak

Pharmaceutical policy in China.

Contradictory goals plague China's pharmaceutical policy. The government wants to develop the domestic pharmaceutical industry and has used drug pricing to cross-subsidize public hospitals. Yet the government also aims to control drug spending through price caps and profit-margin regulations to guarantee access even for poor patients. The resulting system has distorted market incentives, increased consumers' costs, and financially rewarded inappropriate prescribing, thus undermining public health. Pharmaceuticals account for about half of total health spending in China, representing 43 percent

Health care and illness in Lihir, New Ireland Province, in the context of the development of the Lihir gold mine.

During 2000 a review of health care and illness in Lihir, New Ireland Province, was carried out while I was employed as a Community Health Research Officer with the Lihir Management Company, the manager of a gold mine on Lihir. The review aim was to document the health care providers for the local population and the illness burden suffered by Lihirians, and then to shape services to have a more preventive emphasis. It was discovered that the health care services available were well utilized by Lihirians. Health care was provided by the church, government and the mining company, and needed bett

Community-based participatory action research: transforming multidisciplinary practice in primary health care.

OBJECTIVES: Health care systems throughout the world are in the process of restructuring and reforming their health service delivery systems, reorienting themselves to a primary health care (PHC) model that uses multidisciplinary practice (MDP) teams to provide a range of coordinated, integrated services. This study explores the challenges of putting the MDP approach into practice in one community in a city in Canada. METHODS: The data we analyzed were derived from a community-based participatory action research (CBPAR) project, conducted in 2004, that was used to enhance collaborative MDP in

Historical roots of hospital-centred operational governance in China: a path dependence analysis and its implications for global health system reform.

BACKGROUND: Hospital-centred operational governance has been a persistent feature of China's health system. While this model has supported administrative control, rapid service expansion and organisational standardisation, it has also contributed to the continued concentration of resources in hospitals, constrained local flexibility and the relative marginalisation of primary healthcare. This study examines, from the perspective of path dependence, how this governance model gradually emerged and persisted from 1835 to the present. METHODS: Drawing on historical studies, policy documents, inst

Tiered diagnosis and value-based healthcare: can the outpatient reimbursement policy for medical costs in retail pharmacies promote both?

BACKGROUND: Amid growing health demands and tight welfare budgets, guiding rational healthcare-seeking behavior to improve health while controlling costs has become a global challenge. This study aims to examine whether the reform of medical insurance on the cost-sharing of pharmacy can promote tiered diagnosis and value-based healthcare. METHODS: Using 2011-2018 China Health and Retirement Longitudinal Study data and the Staggered DID method, this paper estimates the effects of the Retail Pharmacies Outpatient Reimbursement (RPOR) reform in the Urban Employee Basic Medical Insurance (UEBMI).

Slovakia: Health System Review.

This analysis of the Slovak health system reviews developments in governance, organization, financing and delivery of care, health reforms and health system performance. Slovakia, a central European country with a population of 5.4 million, continues to face significant health and health care system challenges. Slovakia's health system is founded on universal coverage with compulsory health insurance, a broad benefits package and a competitive insurance model. Although life expectancy improved between 2000 and 2019, the COVID-19 pandemic reversed gains, and in 2023 Slovak life expectancy remai

40th birthday of the Italian Mental Health Law 180 - perception and reputation abroad, and a personal suggestion.

How things are perceived from a distance may help better understand their nature. Perceptions at home are likely to shape perceptions abroad. The mutual cross-references between local and distant perspectives on the Italian Mental Health Law 180 may help understand the process which preceded and resulted in the reform. This editorial argues that Law 180 came about at a unique - enabling - time in history. It argues that the run-up to and passing of Law 180 constituted a great accomplishment by professionals, the wider public and politicians/administrators. This editorial goes on to argue that

Civil Society-Driven Drug Policy Reform for Health and Human Welfare-India.

The lack of adequate access to opioids in India as analgesics and for agonist therapies, forces millions to live with severe unalleviated pain, or languish with suffering associated with drug dependence. Although India is a major opium exporter, the excessively prohibitive 1985 narcotics law formulated to control harmful use of drugs, impeded the availability and access to opioids for medical and scientific purposes. Amendment of this law in 2014 established a new national regulatory framework for improved access to essential opioid analgesics. This article reflects on key elements and process

Smallpox Prevention and Public Healthcare in China in the 1920s and 1930s: Focusing on the Cases of Shanghai and Beijing.

Beijing and Shanghai, representative modern cities in China, witnessed the development of various urban infrastructures and quarantine systems in the 1920s and 1930s. Both cities established Health Demonstration Stations in the 1930s, as part of their implementation of modern health administration. This foundation played a pivotal role for making health administration more practical. Huang Zi-fang (1899-1940) and Hu Hung-ji (1894-1932), the inaugural directors of the health bureau in the respective cities, were both graduates of the Johns Hopkins University School of Public Health in the Unite

Nurse-Led, Equity-Driven Quality Improvement: An Adaptation of the Donabedian Model.

Equity and justice have long been positioned as moral imperatives in nursing, yet operational models of quality improvement seldom integrate these values at a theoretical level. This concept development adapts the Donabedian model to align structural competence, social mission, and equitable outcomes, offering a mid-range theory to guide nurse-led quality improvement, education, and policy: the Equity-Driven Quality Framework. This equity-driven reconfiguration relies on the familiar structure-process-outcome triad, then extends it to intentionally incorporate health equity as a defining crite

Trend analysis of medical expenses under DRG payment reform in China.

The impact and evolving trends of Diagnosis-Related Group (DRG) payment reform on inpatient medical expenses have drawn substantial academic and societal attention. However, there remains a notable gap in research focusing on the cost-control efficacy and its dynamic trends under this reform. Employing Mann-Whitney U testing, structural change analysis, and interrupted time-series methods, this study quantifies pre-post DRG reform shifts in inpatient expenditures, insurance compensation patterns, and expenditure trajectories. The results showed that per capita hospitalization expenses decrease

[The democratic movement, cholera epidemic and public health reform in the Zurich canton (1867)].

In the summer of 1867 Zurich, Switzerland, was struck by a severe Cholera outbreak. Recent research suggests that the Cholera epidemic had such an influence on the municipal policy makers that they pushed through a major reform of the public health system, namely of water supply and sewerage. This paper adopts a theoretical conception of crisis and social change to evaluate the plausibility of this hypothesis. The basic idea is that structural change can be usually understood as the consequence of a major social crisis. Exactly this was the case in Zurich during the 1860s. After a decade of ec

Once bitten, twice shy? Lessons learned from an experiment to liberalize price regulations for dental care.

In 2012, The Netherlands established the so-called "free market experiment", which allowed providers of dental care to set the prices for their dental services themselves. The introduction of market mechanisms is intended to improve the quality of care and to contribute to cost containment, but increasing health expenditures for citizens have been observed in this context. Using large-volume health insurance claims data and exploiting the 2012 experiment in Dutch dental care, we identified the effects of a liberalization of service prices. Using pooled regression with individual fixed effects,
