Bridging Systems to Implement HIV/STI Programming Within the Juvenile Justice System: Strengths and Challenges of a Local Change Team Approach.

Justice-involved youth are at risk for HIV/STIs but do not access services. The complex challenges of improving the delivery of health-related services within juvenile justice (JJ) settings warrant exploration of strategies to close this service gap. This study describes the successes and challenges of utilizing a local change team (LCT) strategy comprising JJ and health agency staff to implement HIV/STI programming in JJ settings, across six counties in six states in the U.S. Five focus groups comprising n = 28 JJ and health agency staff who served as LCT members were conducted. Results demon

Characteristics, needs, and help seeking of partner violence victims mandated to community services by courts and child protective services.

The rapid growth of a subpopulation of women victimized by intimate partner violence (IPV) garnered the attention of 2 human service agencies in 1 Southeastern United States city. These agencies noted a shift in their clientele from female IPV victims who voluntarily sought agency services to victims who were mandated to agency services by child protective services (CPS), the court system, or both. Court-referred victims had been arrested for perpetrating IPV against their male partners. CPS-referred victims were experiencing concerning levels of IPV in their families, whether or not the victi

Ethics of digital therapeutics (DTx).

Digital therapeutics (DTx) are a subset of digital health which are often coupled with artificial intelligence (A.I.) techniques and machine learning systems. DTx differ from common wellness apps or medication reminder tools in that they require "rigorous" clinical evidence. They are emerging as a new treatment option and are being applied in a variety of areas, including type II diabetes, hypertension, chronic respiratory problems, obesity, insomnia, Alzheimer's disease, various types of dementia or addiction (smoking, alcohol, drugs), anxiety, depression, autism, learning disabilities, and a

The status and politics of bicycling in the cities of low- and middle-income countries.

Bicycling is promoted in low- and middle-income countries to reduce emissions and improve public health. However, transportation policies often replicate built infrastructure logics from high-income countries, with limited attention to local bicyclists' experiences and contextual realities. Here, drawing on intercept surveys, semi-structured interviews and ethnographic observations in cities of four low- and middle-income countries, we examine the current state of bicycling and user-perceived barriers. We find that bicycling is largely undertaken by low-income adult men, while women and childr

Applied behavioral and decision sciences in support of US FDA's drug regulatory mission.

The US Food and Drug Administration (FDA) makes and communicates decisions that directly affect the health choices and well-being of the US public. Behavioral and decision scientists have long supported FDA's pharmaceutical (medical drug) regulatory and public health mission, working alongside clinical and pharmaceutical scientists. This Perspective describes four interrelated cases illustrating these collaborations: a) creating the Benefit-Risk Framework to guide regulatory decisions regarding new drug approvals; b) launching an internal decision support service to facilitate specific regulat

A Comprehensive Review on Trends and Patterns of Non-communicable Disease Risk Factors in India.

This review explores the trends and patterns of non-communicable disease (NCD) risk factors in India, with a focus on tobacco use, unhealthy diet, physical inactivity, and air pollution. Drawing upon existing literature and data, the review highlights the substantial burden imposed by NCDs and their associated risk factors on public health and healthcare systems in India. Key findings reveal the widespread prevalence of these risk factors, particularly among certain demographic groups and in urban areas. Socioeconomic disparities also play a significant role in shaping the distribution of NCD

Cholera in Nigeria: a five-decade review of outbreak dynamics and health system responses.

BACKGROUND: Cholera remains a significant public health concern in Nigeria, with recurrent outbreaks over the past five decades. Despite advancements in healthcare, the country continues to record high case fatality rates, largely due to inadequate water, sanitation, and hygiene (WASH) infrastructure, poor health systems, climate variability, and conflict-induced displacement. OBJECTIVE: This review aimed to examine the historical trends of cholera in Nigeria, identify key drivers of outbreaks, evaluate government and international responses, and recommend strategic measures for prevention an

Medical evaluation of persons with mental retardation referred for psychiatric assessment.

Many people with developmental disabilities and "challenging behaviors" present to primary care physicians, internists, or general psychiatrists for assessment and treatment. These clinicians seek to provide the comprehensive biopsychosocial assessment necessary for successful treatment, but may encounter interference from funding agencies. Epidemiologic data on medical comorbidity in persons with developmental disabilities with a primarily "behavioral" presentation may assist in facilitating these assessments. A total of 1135 people with mental retardation referred for mental health assessmen

Towards a global system of vigilance and surveillance in unrelated donors of haematopoietic progenitor cells for transplantation.

Safety of living donors is critical to the success of blood, tissue and organ transplantation. Structured and robust vigilance and surveillance systems exist as part of some national entities, but historically no global systems are in place to ensure conformity, harmonisation and the recognition of rare adverse events (AEs). The World Health Assembly has recently resolved to require AE/reaction (AE/R) reporting both nationally and globally. The World Marrow Donor Association (WMDA) is an international organisation promoting the safety of unrelated donors and progenitor cell products for use in

Sealing the cracks: preliminary findings from an inter-ministry initiative to address chronic homelessness in British Columbia.

It is well documented that homeless individuals with severe mental illness (SMI) are in frequent contact with multiple public systems including housing, health, criminal justice, income assistance and various community-based programs. However, this subpopulation of the homeless continues to experience significant unmet needs. This report examines preliminary findings from British Columbia's Homeless Intervention Project, an interagency collaboration designed to improve outcomes for adults with SMI who are chronically homeless. Administrative data from three provincial ministries were used to e

Unmasking the complexities of healthcare access in low-resource settings: a health systems approach to obstetric and under-5 healthcare in rural settings of Eastern Uganda.

BACKGROUND: Access to appropriate obstetric and under-5 healthcare services in low-resource settings is a challenge in countries with high mortality rates. However, the interplay of multiple factors within an ecological system affects the effectiveness of the health system in reaching those in need. OBJECTIVE: This study examined how multiple factors concurrently affect access to obstetric and child healthcare services in resource-poor settings. METHODS: The research used social autopsies [in-depth interview] with mothers who experienced newborn death [n = 29], focus group discussions [n = 8

Partnering with disabilityled organizations to prepare and respond to public health emergencies.

Over a quarter of adults in the United States live with a disability, increasing their risk for severe outcomes from coronavirus disease 2019 (COVID-19). Additionally, people with disabilities face continuous barriers in accessing COVID-19 vaccinations. Disabilityled organizations are catalytic partners for public health departments to reach and provide services to and build partnerships with people with disabilities. For this reason, the Centers for Disease Control and Prevention Foundation funded three disabilityled organizations, two nationally focused and one locally focused, to partner wi

Partner-Engaged Pragmatic Research in Aging Care: Case Studies from the Long-Term Care Quality & Innovation Lab.

Since 2015, the Long-Term Care Quality & Innovation (Q&I) Lab at the Brown University School of Public Health has advanced pragmatic, partner-engaged research to improve care for older adults and chronically ill individuals receiving post-acute and long-term care. Rather than evaluating interventions under controlled conditions, Q&I researchers conduct embedded studies in real-world care settings, working with clinicians and frontline staff to implement interventions and assess their effectiveness and feasibility in routine practice. In parallel, researchers also collaborate with health system

West Nile virus in the United States: guidelines for detection, prevention, and control.

The epidemic/epizootic of West Nile (WN) encephalitis in the northeastern United States in the summer and fall of 1999 was an unprecedented event, underscoring the ease with which emerging infectious pathogens can be introduced into new geographic areas in today's era of rapid transportation and increased movement of people, animals, and commodities. This epidemic/epizootic and the increased frequency of other exotic pathogens being imported into the United States raises the issue of whether local, state, and national public health agencies are prepared to deal with epidemics/epizootics of vec

From the field: a maternal-child health nursing competence validation model.

This group of Vermont community health nurses from different agencies collaborated to develop a competence validation framework for maternal and child health nursing in the practice areas of perinatal client teaching, breastfeeding, and prenatal, postpartum, and newborn nursing care. The framework is based on the work of Benner, using the "competent" level of nursing practice, and delineates three parameters of competence: technical skills, interpersonal skills, and critical thinking skills. Learning resource materials, including newborn and maternal assessment guidelines, were developed for e

Dangerous Placebo During the COVID-19 Pandemic: A Series of Homoeopathic Arsenicum Album-Induced Liver Injury.

Complementary and alternative medicines were promoted as health supplements, "immune-boosters" and COVID-19 preventive drugs through visual, print, and social media, during the pandemic. In this context, specifically in India, the homeopathic remedy, Arsenicum Album 30C prepared from arsenic trioxide was widely prescribed and publicly supplied through government agencies among adults and school-going children. Addressing the commercial determinants of unhealthy beverage exposure: a systems thinking approach.

BACKGROUND: The beverage industry plays an influential role in the US. Certain beverages, such as sugar-sweetened, have long been recognized as key contributors to poor population health with inequitable impacts. In response, multiple US initiatives have attempted to address these problems through public awareness campaigns and policies, such as taxes. Yet, the persistent and increasing reliance on unhealthy commercial products produced by the beverage industry suggests additional progress is needed. Leveraging a systems thinking approach, this study aimed to map out the complex forces limitin

'Only systems thinking can improve family planning program in Pakistan': A descriptive qualitative study.

BACKGROUND: Family Planning (FP) program in Pakistan has been struggling to achieve the desired indicators. Despite a well-timed initiation of the program in late 50s, fertility decline has been sparingly slow. As a result, rapid population growth is impeding economic development in the country. A high population growth rate, the current fertility rate, a stagnant contraceptive prevalence rate and high unmet need remain challenging targets for population policies and FP programs. To accelerate the pace of FP programs and targets concerned, it is imperative to develop and adopt a holistic appro

Emergency implementation of knowledge management system to support a bioterrorism response.

In a public health emergency, it becomes necessary for public health agencies to provide timely, accurate and useful information to the community. During the anthrax attacks, the Public Health Practice Program Office in the Centers for Disease Control and Prevention implemented a knowledge management (KM) system to respond to an increased number of inquiries from public health officials, first responders, and health care professionals as well as the general public. While it is possible to successfully implement a knowledge management system quickly in a crisis situation, additional challenges
