PURPOSE: Hyperglycemia may rarely be seen with visual seizures. Observation of both visual evoked potentials (VEPs) and magnetic resonance imaging (MRI) in visual status epilepticus (SE) has not been reported. We describe acute and follow-up VEP and MRI findings of a patient with hyperglycemia-related visual SE of occipital origin.
METHODS: In a 59-year-old diabetic woman, complex visual hallucinations and illusions developed with < or =10 seizures per hour as an initial manifestation of nonketotic hyperglycemia.
RESULTS: Neurologic examination revealed ictal nystagmus to the right and conti

A case of post-traumatic opto-chiasmatic syndrome is described, and the principal characteristics of the affection outlines. This rare syndrome complicates severe cranial injuries, and is simple to diagnose when confronted with a patient with bitemporal hemianopsia frequently associated with diabetes insipidus or paralysis of certain cranial nerves. Suggested mechanisms for the lesion include sagittal stretching of the chiasma or an ischemic focus.

Progressive multifocal leukoencephalopathy (PML) is a demyelinating disorder of the CNS that usually causes hemiparesis or hemianopsia. Dementia occurs in combination with other neurologic abnormalities. We report a human immunodeficiency virus type 1 (HIV)-infected man whose only manifestation of proven PML was dementia that was clinically indistinguishable from HIV-associated dementia.

Background Morning glory disc anomaly (MGDA) is a rare developmental abnormality of the optic disc that is associated with many other neurological and vascular conditions. Most cases are diagnosed in childhood. Case Report We report a 57-year-old woman who presented to the ophthalmology department for assessment of long-standing poor vision in the left eye and exotropia. Examination showed a left MGDA and bitemporal hemianopsia. These findings prompted magnetic resonance imaging, revealing a transsphenoidal basal meningoencephalocele with herniation of the optic chiasm and inferior hypothalamu

Cervical manipulation, specifically chiropractic manipulation, is an important cause of vertebrobasilar and occasionally carotid distribution strokes. Neuro-ophthalmologic findings are a common and at times relatively isolated feature of cervical manipulation-induced stroke. A case of chiropractic-induced occipital lobe infarction with homonymous hemianopsia is reported, and the literature regarding neuro-ophthalmologic findings is reviewed.

A 32-year-old man with a history of delayed puberty and obesity was seen initially with bitemporal hemianopsia and headache in 1974. Neuroradiologic studies showed a mass in the chiasmatic cistern and hypothalamus. Surgical exploration of the chiasm was unrevealing. Slowly progressive memory loss and intellectual impairment developed. Necropsy in 1978 disclosed a large cavernous hemangioma in the diencephalon.

PURPOSE: Traumatic injury to the optic chiasm is rare and most frequently caused by high-velocity head trauma. It classically results in bitemporal hemianopsia and often presents in conjunction with multiple other traumatic injuries, such as skull fractures and cerebrospinal fluid leaks. We present the case of a 40-year-old woman with pre-existing thyroid orbitopathy who struck her forehead after a fall from standing height.
OBSERVATIONS: This patient suffered immediate profound unilateral vision loss from traumatic optic neuropathy and possible optic nerve avulsion. The fellow eye manifested

Five cases of cerebral paragonimiasis presenting with hemianopsia, convulsion, and gait disturbance are discussed. The cases were all in the chronic stage. The intradermal paragonimiasis reaction, complement fixation, and Ouchterlony tests were not useful for diagnosis. Computed tomography demonstrated calcifications in all cases in sites consistent with the foci of symptoms. Surgical treatment in two cases failed to improve symptoms.

A 73-year-old woman presented with a temporary stupor and right hemiplegia. Brain magnetic resonance imaging revealed acute infarction of the anterior choroidal artery. The clinical features associated with acute anterior choroidal artery infarction typically include hemiparesis, hemihypesthesia, and hemianopsia. However, our patient presented with a temporary stupor, which is a rare manifestation of anterior choroidal artery infarction.

Stroke-like migraine attacks after radiation therapy (SMART) syndrome is a rare complication of radiotherapy with complex neurological impairment. Patients present with neurological symptoms and signs such as migraine, hemianopsia, hemiplegia, aphasia and/or seizures-without recurrence of neoplastic disease. In this report, we describe SMART syndrome in two adult patients 4 and 14 years following brain irradiation, respectively.

We report two cases in which heteronymous, altitudinal visual field defects resulted in loss of fusion and transient overlap of preserved hemifields. This phenomenon resulted in complaints of diplopia similar to that described as "hemifield" slide previously described by temporal hemianopsias. These cases illustrate that generalized loss of fusion, and not the particular type of hemianopic field defect, is responsible for the symptoms known as hemifield slide.

The authors report a case of pituitary adenoma with positive immunospecific staining for beta-FSH and serum alpha subunit. The tumour was revealed by an amenorrhoea-galactorrhoea syndrome with hyperprolactinaemia and without increase in serum gonadotrophin values. Pregnancy occurred during bromocriptine treatment which made it possible to control an hemianopsia developed in the fourth month of gestation. The adenoma was successfully removed a few months later.

A 53 year-old woman presented a recurrent bifrontal headache of 2 years duration and bilateral progressive visual disturbance. The clinical and neurological examination showed a bilateral feet adactyly and bitemporal hemianopsia. The brain MRI demonstrated a Rathke's cleft cyst. The patient was operated by a transnasal endoscopic approach. It seems that this unusual association has never been described before.

A 54-year-old female presented with a cerebral metastasis of functioning parathyroid carcinoma in the right occipital lobe. After surgical removal of the metastatic tumor, her symptoms of left hemianopsia and hypercalcemia improved. Surgical treatment should be considered in a case of brain metastasis of parathyroid carcinoma associated with secondary hyperparathyroidism to prevent metabolic complications and reduce the mass of the tumor.

A rare case of epidermoid of the third ventricle was experienced. The patient was a fifteen years old boy, who had been hospitalized four years earlier due to acute hydrocephalus. The ventriculography at that time revealed a large cystic tumor containing heterogeneous mass filling the entire third ventricle, which seemed to be a dermoid or epidermoid. Ventricular drainage and ventriculo-peritoneal shunt were performed and he was discharged one month later without any complaint. On February 10th 1983, he was re-admitted with severe headache, nausea and showing markedly increased intracranial pr

Homonymous field defects cause reading and orientation disorders. The reading disorder depends on the size of the macular sparing, the side of the field defect (unfavorable in reading direction) and spontaneous adaptive mechanisms. Methods which support compensation strategies should be recommended as evidence-based training procedures: optokinetic training with scrolled text for reading disorders and saccade training for orientation disorders. By optimized utilization of the total field of gaze, general exploration of the environment, reaction times during search tasks and quality of life can

BACKGROUND: Arachnoid cysts (ACs) are collections of CSF within an arachnoid membrane. Suprasellar ACs (SACs) constitute 1%-9% of all ACs, with giant SACs representing a rarer subset at increased risk of rupture, intracranial hemorrhage, and neurological deficits. While ventriculostomy is well characterized, anatomical considerations and technical nuances of cystocisternostomy remain limited; adult presentation and outcomes remain poorly defined. The authors describe surgical technique and postoperative outcomes in 2 adult patients undergoing endoscopic ventriculocystocisternostomy for giant S

We report a patient with posterior cortical atrophy (PCA) who manifested as agraphia for kanji and statokinetic dissociation (Riddoch phenomenon). A 52-year-old, right-handed woman complained that beginning at age 50, she could write only kana (a phonographic script) but not kanji (a morphographic script). She could not write even her own name in kanji. Neuropsychologic examinations disclosed kanji-dominant agraphia, acalculia, right-left disorientation, finger agnosia, constructional apraxia, and simultanagnosia. Many of these, including agraphia, are components of Gerstmann syndrome. She man

Hyperosmolar hyperglycemic state (HHS) is a severe complication of uncontrolled diabetes, often presenting with nonlocalizing neurologic symptoms such as encephalopathy. Rarely, focal neurologic deficits like hemianopsia can occur, even with serum osmolality below the diagnostic threshold for HHS. We report two cases of hyperglycemic patients with borderline serum osmolality presenting with neuro-ophthalmologic symptoms. Case 1 involved a 43-year-old male with type 1 diabetes who developed right-sided visual disturbances and was found to have cortical T2/FLAIR hyperintensity and subcortical hy

We report for the first time two cases of physiologic pituitary hyperplasia causing visual disturbance during adolescence. Case 1. A 15-year-old boy visited our department with a pituitary mass lesion on magnetic resonance imaging (MRI) associated with headache and transient bitemporal hemianopsia repeating. A visual field defect became apparent on the bitemporal upper side, and the area of the visual field defect enlarged to hemianopsia. The visual defect usually occurred in relation to exercise and continued for approximately 10 min. MRI showed the mass effect of pituitary hyperplasia to the
