The direct anterior approach in hemiarthroplasty for displaced femoral neck fractures.

PURPOSE: Hip replacement is the most common treatment for displaced femoral neck fractures in the elderly, and minimally invasive surgery is popular in the field of orthopaedic surgery. This study evaluated the outcome of monopolar hemiarthroplasty by the direct anterior approach over a postoperative period up to 2.5 years. METHODS: A total of 86 patients with displaced femoral neck fractures were included (mean age of 86.5 years). Surviving patients were reviewed three months (retrospectively) and one to 2.5 years (prospectively) after surgery. One-year mortality was 36 %. RESULTS: For all

Hemiarthroplasty for pigmented villonodular synovitis of the shoulder: a report of two cases.

Pigmented villonodular synovitis is a reactive condition characterised by exuberant proliferation of synovial villi and nodules. It may be localised or diffuse and can cause severe articular damage. This report is of 2 patients with pigmented villonodular synovitis of the shoulder causing extensive arthritic changes. Both patients underwent shoulder hemiarthroplasty and total synovectomy and achieved satisfactory painless range of motion, with no signs of local recurrence or loosening of the prosthesis after 4 to 5 years of follow-up.

[Fixation of the tuberculum in multifragment fractures of the proximal humerus treated with hemiarthroplasty].

Hemiarthroplasty is an effective treatment for multifragmentary fractures of the proximal humerus. Purpose of this study was to compare 2 different methods of tuberosity dislocations, which is one of the primary reasons for successful posttraumatic arthroplasty. Between 2006 and 2008, 29 patients with multifragmentary fractures of the proximal humerus underwent hemiarthroplasty of the shoulder. Mean gae was 69y., while mean postoperative follow up was 27 months. We evaluated two different techiques of tuberosity fixation after anatomic reattachment on humeri: (A) intertuberosity suture stabiliz

Early management of proximal humeral fractures with hemiarthroplasty: a systematic review.

We performed a comprehensive systematic review of the literature to examine the role of hemiarthroplasty in the early management of fractures of the proximal humerus. In all, 16 studies dealing with 810 hemiarthroplasties in 808 patients with a mean age of 67.7 years (22 to 91) and a mean follow-up of 3.7 years (0.66 to 14) met the inclusion criteria. Most of the fractures were four-part fractures or fracture-dislocations. Several types of prosthesis were used. Early passive movement on the day after surgery and active movement after union of the tuberosities at about six weeks was described i

[Hemiarthroplasty and total hip arthroplasty for idiopathic osteonecrosis of the femoral head].

Patients with idiopathic osteonecrosis of the femoral head (ION) are relatively younger (usually in their 30s to 50s) than those with osteoarthrosis (OA) , and usually have higher activity levels. When the necrotic area is large, or when advanced OA has developed, hemiarthroplasty or total hip arthroplasty (THA) are considered. When indicating these implant surgeries, age and life style of patients should be considered in selecting type of surgery and implant. In our previous study, THA was superior in durability to hemiarthroplasty in the entire series of ION patients and in the patients with

Unstable hemiarthroplasty of the hip-treated with a simple technique of acetabular augmentation.

Hemiarthroplasty of the hip is one of the most frequently performed orthopedic procedures for trauma in the elderly. Dislocation is one of its well-recognized complications and ranges from 1% to 16% (Acta Orthop Scand. 2003;74:45-48) in different series. Often, dislocation can be recurrent when the morbidity is even higher. Suggested methods of treatment for this difficult problem range from closed/open reduction followed by conservative management to Girdlestone excision arthroplasty. We report a unique, simple, and effective method of managing this problem, particularly in those elderly pati

Comparison between bipolar hemiarthroplasty and THA for osteonecrosis of the femoral head.

It is controversial whether bipolar hemiarthroplasty or total hip arthroplasty should be done for Ficat Stage III osteonecrosis of the femoral head. A prospective comparative study was done using the same cementless femoral components for both procedures. Forty cementless bipolar hemiarthroplasties and 31 cementless total hip arthroplasties were done in 54 patients with Ficat Stage III osteonecrosis of the femoral head. Age, gender, and followup were matched between patients having bipolar hemiarthroplasty and total hip arthroplasty. Treatment with total hip arthroplasty increased the total hi

Short- to midterm outcomes following pyrocarbon glenohumeral hemiarthroplasty: a retrospective review.

BACKGROUND: Glenohumeral osteoarthritis (GHOA) in young patients remains challenging due to high revision rates following arthroplasty in active individuals. The purpose of this study was to evaluate clinical outcomes and radiographic parameters following pyrocarbon (PyC) hemiarthroplasty (HA) in patients with GHOA. METHODS: A retrospective review was performed at a single academic institution from 2015 to 2025 including patients who underwent PyC HA with a minimum of one year of clinical and radiographic follow-up. Mean clinical and radiographic follow-up was 2.7 years (range, 1-7.5 years).

Arthroscopic Management of Arthrofibrosis After Hemiarthroplasty for Proximal Humerus Fracture Dislocation.

Poor functional outcomes after hemiarthroplasty for proximal humerus fractures are common, yet revision surgery is relatively rare. Arthroscopic treatment for postoperative stiffness can be considered in the setting of functional limits to glenohumeral range of motion impacting activities of daily living after adequate conservative treatment with physical therapy and in the setting of healed, well-positioned tuberosities and humeral components. This Technical Note illustrates a stepwise approach to an arthroscopic lysis of adhesions and capsular release for the treatment of arthrofibrosis of t

Effect of hip hemiarthroplasty dislocation on mortality after hip fracture surgery.

INTRODUCTION: Hip arthroplasty is the treatment of choice for displaced femoral neck fractures among the older population. The hip prosthesis dislocation is one of the most pointed potential complications after hip arthroplasty, but there is a lack of updated information on the effect of dislocation on the survival of older hip fracture patients so treated by hip hemiarthroplasty. We aim to evaluate the standalone effect of hip prosthesis dislocation after hip fracture hemiarthroplasty on patients' survival outcomes. MATERIALS AND METHODS: We conducted a retrospective multicenter study, inclu

Hemiarthroplasty proximal femoral endoprostheses following tumour reconstruction: is acetabular replacement necessary?

AIMS: Dislocation rates are reportedly lower in patients requiring proximal femoral hemiarthroplasty than for patients undergoing hip arthroplasty for neoplasia. Without acetabular replacement, pain due to acetabular wear necessitating revision surgery has been described. We aimed to determine whether wear of the native acetabulum following hemiarthroplasty necessitates revision surgery with secondary replacement of the acetabulum after proximal femoral replacement (PFR) for tumour reconstruction. PATIENTS AND METHODS: We reviewed 100 consecutive PFRs performed between January 2003 and Januar

Shoulder hemiarthroplasty: outcomes and long-term survival analysis according to etiology.

BACKGROUND: The indications for hemiarthroplasty in glenohumeral joint diseases remain controversial and depend mainly on the original underlying diagnosis. Our objective was to investigate the influence of the primitive aetiology on long-term prosthesis survival and on the Constant-Murley score. MATERIALS AND METHODS: We studied 272 shoulders with the following diagnoses: fracture sequelae (n=73), primary osteoarthritis (n=67), cuff tear arthropathy (n=43), avascular necrosis (n=40), rheumatoid arthritis (n=31), and other (n=18). Of the 272 shoulders, 139 were evaluated after at least 8 year

Clinical results of conversion total hip arthroplasty after failed bipolar hemiarthroplasty.

Total hip arthroplasty (THA) has been the most common surgery performed for complications of bipolar arthroplasty. We wanted to evaluate functional results and complications after conversion of bipolar hemiarthroplasty to THA in 25 patients followed for an average of 7.2 years. Indications for conversion included acetabular erosion with well-fixed femoral stem in 13 patients, acetabular erosion with femoral loosening in 8 patients, and periprosthetic fracture in 4 patients. The average Harris Hip Score improved from 41 (34 to 67) before conversion to 85 (65 to 95) at final follow-up. The avera

Preoperative factors associated with improvements in shoulder function after humeral hemiarthroplasty.

BACKGROUND: The relationship between the characteristics of the shoulder that can be determined before humeral hemiarthroplasty and the functional improvement after surgery is not known. The goal of this study was to test the hypothesis that the functional outcome of this procedure correlated significantly with factors that are identifiable preoperatively. METHODS: The study group included seventy-one shoulders in sixty-eight patients undergoing hemiarthroplasty, performed by the same surgeon, for diagnoses other than acute fracture. The mean age of the patients was sixty-one years (range, th

Bipolar hemiarthroplasty in juvenile rheumatoid arthritis: long-term survivorship and outcomes.

Because controversy surrounds the management of end-stage hip disease in juvenile rheumatoid arthritis (JRA), this study evaluated the long-term outcome of bipolar hemiarthroplasty as an alternative to conventional joint arthroplasty. A total of 24 JRA patients underwent 39 hemiarthroplasties; follow-up averaged 12 years (range, 3 to 15 years). There were 14 hips (36%) revised, and 25 hips (64%) maintained the original components. Mean Harris hip scores in surviving hips improved from 29 to 69 points (P<.001). Radiographs showed progressive bipolar superomedial migration (P<.01) despite attemp

Diagnostic Test Performances for Identifying Periprosthetic Joint Infection in Hip Hemiarthroplasty.

INTRODUCTION: Periprosthetic joint infection (PJI) is a devastating complication of hip hemiarthroplasty (HHA) that is not well-represented in the literature. Therefore, this study aimed to evaluate diagnostic markers for identifying PJI in patients after HHA and compare them with the most recent 2018 International Consensus Meeting on Musculoskeletal Infection criteria. METHODS: A total of 98 patients (64 PJIs, 65.3%) were analyzed. Patients were identified by relevant Current Procedural Terminology and International Classification of Diseases-9/10 codes from 2000 to 2021 across a single hea
