The World Health Organization estimates that about 170 million people are infected with hepatitis C virus (HCV). Blood transfusions from unscreened donors and unsafe therapeutic procedures are the major modes of HCV transmission in the developing world, and injection drug use accounts for most newly diagnosed HCV infections in the developed countries. Acute infection with HCV leads to symptomatic hepatitis in only a minority of patients, and recent studies suggest that spontaneous clearance of virus is higher in symptomatic acute hepatitis C infection. Pooled data from various studies suggest …

Development and testing of antifibrotic agents for the treatment of chronic hepatitis C have generally been targeted toward hepatic stellate cells, transforming growth factor-beta, the inflammatory response, or extracellular matrix accumulation. Although several agents such as interferon-gamma, long-term pegylated interferon, and caspase inhibitors have been studied, none have proved to be effective to date. There is a clear need for drugs that inhibit or reverse hepatic fibrosis as these would be immediately applicable to patients for whom antiviral therapy has failed or who have contraindica…

This review will focus on the prevalence of hepatitis c virus (HCV) infection in alcoholics with and without liver disease. Evidence will be presented to demonstrate that ethanol and chronic HCV infection synergistically accelerate liver injury. Some of the major postulated mechanisms responsible for disease progression include high rates of apoptosis, lipid peroxidation, and generation of free radicals and reactive oxygen species with reduced antioxidant capacity of the liver. Acquisition and persistence of HCV infection may be due to the adverse effects of ethanol on humoral and cellular imm…

Hepatitis C virus (HCV) is an important causative agent of liver diseases. However, HCV infection is also associated with numerous hematologic, renal, dermatologic, rheumatic, and autoimmune disorders. These include arthralgia, arthritis, vasculitis, sicca syndrome, myalgia, and fibromyalgia. The purpose of this article is to review the prevalence and spectrum of rheumatic disorders and autoimmune phenomena in HCV-infected patients. It evaluates and current treatment options including nonsteroidal anti-inflammatory drugs, low-dose corticosteroids, hydroxychloroquine, methotrexate, penicillamin…

Although asymptomatic joint involvement and arthralgias are frequent in patients with hepatitis C virus chronic infection (HCV), a true arthritis affects only up to 4% of the subjects. HCV-related arthritis (HCVrA) is usually distinguished in two clinical subsets: a more frequent symmetrical polyarthritis (SP), similar to rheumatoid arthritis but much less serious, and an intermittent mono-oligoarthritis (IMO) that involves medium and large sized joints, mainly the ankle. This latter subset is strictly related to the presence of HCV-induced mixed cryoglobulinemia and its cutaneous manifestatio…

Around 25% of people infected with hepatitis C virus (HCV) are able to clear the infection spontaneously, while the majority become chronically infected, with a subsequent risk for the individual patient of progressive inflammatory liver disease, cirrhosis, hepatocellular carcinoma and liver-related death (Figure 1). Much is known about the epidemiology, pathogenesis, diagnosis and management of chronic HCV infection. In comparison, knowledge about acute HCV infection is patchy. In this article, we will highlight concerns relating to acute HCV infection and suggest that public health bodies re…

Hepatitis C virus (HCV) is well known for its aetiological role in chronic non-A, non-B viral hepatitis, liver cirrhosis and hepatocellular carcinoma; in addition, the virus has also been implicated in a number of extra-hepatic "autoimmune" disease manifestations. A causative association between HCV and non-Hodgkin lymphoma (NHL) was postulated relatively recently and has been the subject of intense investigation, as well as some debate. On the strength of epidemiological data, emerging biological investigations and clinical observations, HCV appears to be involved in the pathogenesis of at le…

Since the discovery of the hepatitis C virus (HCV) in 1989, attention has been paid to the association of chronic HCV infection and the development of diabetes. The risk factors for diabetes include older age, HCV genotype 3, severe liver fibrosis, family history of diabetes, and liver/kidney transplantation. Emerging evidence in animals and humans has shown that HCV infection induces hepatic steatosis and increases tumor necrosis factor-alpha level, both resulting in the development of insulin resistance and subsequent type 2 diabetes. It is suggested that the presence of diabetes and hepatic…

Chronic hepatitis C induces a state of hepatic oxidative stress that is more pronounced than that present in many other inflammatory liver diseases. This review summarizes recent information that the hepatitis C virus (HCV) core protein plays an important role in this phenomenon. Core protein localizes to mitochondria, particularly at the points of contact between mitochondrial outer membrane and endoplasmic reticulum. Its expression causes inhibition of electron transport at complex I, increased complex I reactive oxygen species (ROS) production, decreased mitochondrial glutathione, and incre…

Estimating a prevalence of 1.8% of hepatitis C in the general population, there are in Louisiana about 81,000 individuals who have been infected with the hepatitis C virus. In 2002, the incidence of reported cases of acute hepatitis C was 2.2/100,000 versus 0.64 in the USA. The case register maintained by the Louisiana Office of Public Health has 22,000 cases. Building up a register of hepatitis C past or present infections is an important tool that may be used to resolve some important questions about the epidemiology of hepatitis C or be used for future preventive actions. Physicians and hos…

Acute infection with hepatitis C virus (HCV) is a frequent but underrecognized problem among substance users, because it produces few symptoms. Despite this fact, a good deal has been learned recently from studies of cohorts of persons with acute HCV infection. Intensive study of these cohorts has suggested that there is a higher frequency of spontaneous clearance among persons with symptomatic infection and persons with vigorous adaptive immune responses. Similarly, polymorphisms for genes involved in innate immunity also appear to influence the outcome of acute HCV infection. Data on injecti…

There have been numerous new data in the field of acute hepatitis C in the past few years. The goal of this study was to gather recent results in the literature to determine the best current therapeutic strategy of acute hepatitis C. Epidemiology has made enormous progress with the disappearance of acute post transfusion hepatitis C. Infection by intravenous drug-addiction has became prevalent, and nosocomial and sexual modes of transmissions are now seen. At the same time, the average age of the infected patients has dropped by 13 years, and the proportion of acute forms with a spontaneously …

Acute hepatitis C virus infection produces clinical and biochemical features that is non-specific and indistinguishable from those caused by other hepatotropic viruses. The specific diagnosis of acute hepatitis C virus infection is based on the detection of serum RNA-HCV through a technique of PCR whose result will be positive after 1-2 weeks of the initial contact with the virus. The anti-bodies against HCV are detected later (after 7-8 weeks on average), and are not useful, as an isolated determination, in distinguishing acute infection from chronic infection or in clearing the virus (sponta…

Arthritis is one of the several autoimmune disorders induced by HCV infection. There is not a specific clinical pattern of HCV-related arthritis, but two nonerosive subsets have more frequently been described: a RA-like polyarthritis and a less common mono-oligoarthritis involving medium-sized and large joints, often showing an intermittent course. This latter form is associated with the presence of serum cryoglobulins. Because of its variable characteristics, HCV-related arthritis must be considered in the differential diagnosis of many patients having inflammatory joint involvement. Antikera

BACKGROUND: We presented the results of treatment in patients with AHC and compared these findings with natural outcome of AHC in untreated patients.
MATERIAL/METHODS: Ten treated AHC patients (5F/5M, mean age: 35.5 yrs; all HCV-RNA-positive including 9 anti-HCV-positive). Antiviral treatment was started within 3 to 18 (median 9) weeks after the onset of acute hepatitis including eight patients with monotherapy of IFN and two patients with combined (IFN + ribavirin) therapy.
CONTROL GROUP: Fifty consecutive untreated AHC patients (26F/24M, mean age: 40.8 yrs; all HCV-RNA-positive, including

It has been recognized that, with certain limitations, interferon (IFN) alpha and beta, whether alone or combined with ribavirin, is the first choice therapy for chronic hepatitis C. The administration of standard interferon during 12 months reaches an acceptable effectiveness, although it's not the best. Interferon only achieves a sustained response in 20% of the cases. When it's combined with ribavirin, the sustained response reaches 35%. Recently, the development of pegylated interferon combined with ribavirin increased the sustained viral response to 55 percent. With respect to duration of…

Hepatitis C virus (HCV) has been implicated in the development of a variety of autoimmune phenomena, some of which are well documented and include a panel of auto-antibodies shared with autoimmune hepatitis (AIH). Anti-nuclear (ANA) and smooth muscle (SMA) antibodies (markers of AIH type 1 [AIH-1]), have been demonstrated in 9-38% and 5-91% of cases respectively, whereas anti-liver/kidney microsomal type 1 (anti-LKM-1) and anti-liver cytosol type 1 antibodies (anti-LC1) (markers of AIH type 2 [AIH-2]), are definitely rarer, especially in adults. The presence of these auto-reactivities in chron…
