Transition experiences of patients with post stroke dysphagia and family caregivers: A longitudinal, qualitative study.

BACKGROUND: Stroke patients with dysphagia and family caregivers will experience multiple transitions during the whole process of the disease and various nursing needs will be generated. There is a lack of knowledge about their experiences at different transition stages. Thus, we aimed to explore the transition experiences of patients with post stroke dysphagia and family caregivers from admission to discharge home. METHODS: A semi-structured interview based on Meleis's transition theory was used during hospitalization and telephone follow-up interviews were conducted in the first, third, and

Effects of home-based cardiac rehabilitation integrated in the cardiac care bridge transitional care program on the physical functioning of older patients who are frail: secondary analysis of a randomized trial.

IMPORTANCE: Older patients hospitalized for cardiovascular disease (CVD) are at risk of physical function decline and adverse health outcomes. Cardiac rehabilitation (CR) improves physical functioning but is underutilized by older patients. Home-based CR potentially improves utilization, yet its effectiveness in older patients who are frail remains understudied. OBJECTIVE: The objective of this study was to investigate the effects of a transitional-care integrated home-based CR program on physical functioning in older patients who are frail after CVD hospitalization. DESIGN: This was a presp

Readmission is experienced as inevitable among older adults receiving homecare: A qualitative interview study.

BACKGROUND: Older adults receiving homecare have an increased risk of readmission. The transition from hospital to home can be experienced as unsafe, and older adults describe themselves as vulnerable during the post-discharge period. Thus, the objective was to explore the experiences of unplanned readmissions among older adults who receive homecare. METHODS: We conducted qualitative individual semi-structured interviews with older adults, 65 years or above, receiving homecare and being readmitted to an emergency department (ED) between August and October 2020. Data were analysed by systemati

Transition to a Safe Home Sleep Environment for the NICU Patient.

Of the nearly 3.8 million infants born in the United States in 2018, 8.3% had low birth weight (ie, weight <2500 g) and 10% were born preterm (ie, gestational age of <37 weeks). Ten to fifteen percent of infants (approximately 500 000 annually), including low birth weight and preterm infants and others with congenital anomalies, perinatally acquired infections, and other diseases, require admission to a NICU. Every year, approximately 3600 infants in the United States die of sudden unexpected infant death (SUID), including sudden infant death syndrome (SIDS), unknown and undetermined causes, a

Using the RE-AIM Framework to Understand Pathway Home's Impact, Reach, and Long-Term Sustainability Within the Healthcare System.

Consistent evidence demonstrates that care transition programs can significantly improve mental health outcomes, treatment engagement, satisfaction, and overall quality of life. However, the extent and fidelity of implementation across behavioral health systems remain underexamined. This article applies the RE-AIM (Reach, Effectiveness, Adoption, Implementation, and Maintenance) framework to evaluate the Pathway Home model, a community-based care transition program in New York that supports individuals returning to the community after psychiatric hospitalization or other institutional settings

Comparison of mortality and hospitalizations of older adults living in residential care facilities versus nursing homes or the community. A systematic review.

Residential care facility may provide a transition between living at home and a nursing home for dependent older people or an alternative to nursing homes. The objective of this review was to compare mortality and hospitalizations of older adults living in residential care facilities with those living in nursing homes or in the community. We searched Medline, Scopus and Web of Science from inception to December 2022. Fifteen cohort studies with 6 months to 10 years of follow-up were included. The unadjusted relative risk (RR) of mortality was superior in nursing homes than in residential care

Social Work Support for Anticipatory Grief in Rural Palliative Care Transitions: A Narrative Review for South Africa.

Family caregivers play a central role in palliative care, especially during hospital-to-home transitions, when rural families may assume complex care with limited preparation and uneven professional support. Anticipatory grief, the grief experienced before a loved one's death, can intensify emotional, practical, social and financial strain. A SANRA-informed, agenda-setting narrative review integrated literature on palliative care, caregiver burden, anticipatory grief, transitional care, rural healthcare and social work to develop a research and practice agenda for rural South African public he
