Hospitalists often care for patients with liver disease, including those with acute liver injury and failure and patients with complications of decompensated cirrhosis. Acute liver failure is a true emergency, requiring intensive care and oftentimes transfer of the patient to a liver transplant center. Patients with decompensated cirrhosis have complications of portal hypertension, including variceal hemorrhage, ascites, spontaneous bacterial peritonitis, and hepatic encephalopathy. These complications increase the risk of mortality among patients with decompensated cirrhosis. Comanagement by

Hepatic encephalopathy (HE) is an important complication of decompensated liver disease. Hospital admission for episodes of HE are very common, with these patients being managed by the hospitalists. These admissions are costly and burdensome to the health-care system. Diagnosis of HE at times is not straightforward, particularly in patients who are altered and unable to provide any history. Precipitants leading to episodes of HE, should be actively sought and effectively tackled along with the overall management. This mandates timely diagnostics, appropriate initiation of pharmacological treat

BACKGROUND: Asthma is one of the most common chronic diseases affecting children in the United States, with millions impacted annually. There is ongoing concern and disparity in the pediatric workforce. However, there is limited research evaluating how the geographic distribution of these providers aligns with pediatric asthma burden across states.
OBJECTIVE: To assess the degree of geographic alignment between pediatric asthma prevalence and the distribution of general pediatricians and pediatric hospitalists in the United States using inequality metrics and spatial analysis.
METHODS: This

As pediatric and neonatal hospitalists assume increasing care for newborns in the delivery room and during the birth hospitalization, preparing pediatric trainees with foundational skills in neonatal resuscitation is increasingly critical. However, residency programs may face challenges in providing sufficient delivery room exposure and procedural opportunities given newer training requirements and updated care practices that reduce procedural interventions during neonatal resuscitation. In this Perspectives article, we examine factors contributing to these gaps, describe our institution's hos

Background: Skin inflammation causes vasodilation and increased vascular permeability, which may result in decreased blood pressure and peripheral edema. Patients with erythroderma usually compensate for low blood pressure with fluid retention and increased cardiac output. However, if the heart cannot support increased cardiac output, blood pressure will stay low, resulting in severe compensatory fluid retention, which leads to decompensated congestive heart failure, and pulmonary edema. Treatment for high-output congestive heart failure focuses on the primary pathology, which is skin inflamma

BACKGROUND: Inappropriate antibiotic prescribing upon hospital discharge has been identified as a significant problem. Despite high rates of antibiotic prescription errors, there is no widely accepted antimicrobial stewardship initiative to prevent such errors.
AIM: The primary objective of this study was to determine the impact of hospital-based clinical pharmacist discharge prescription review on the appropriateness of antibiotic prescriptions.
METHOD: This was an observational study comparing the appropriateness of hospital discharge antibiotic prescriptions between two similar internal m

BACKGROUND: In the care of feverish children, symptomatic management is pivotal. Thus, the Italian Pediatric Society has recently published guidelines on fever management in children. Our aim was to investigate whether pediatric hospitalists, community pediatricians and pediatric residents differ in their every-day clinical practice with respect to symptomatic management of feverish children.
METHODS: 79 out of 118 physicians involved in pediatric care in an area of Northern Lombardy (Italy) filled in a modified version of the questionnaire derived from the Swiss national survey on symptomati

Hospitalists are increasingly responsible for the management of infectious consequences of opioid use disorder (OUD), including increasing rates of hospitalization for injection drug use (IDU)-associated infective endocarditis, osteomyelitis, and soft tissue infections. Management of IDU-associated infections poses unique challenges: symptoms of the underlying addiction can interfere with care plans, patients often have difficult psychosocial circumstances in addition to their addiction, and they are often stigmatized by the healthcare system. Although there are few randomized trial data to su

OBJECTIVES: Fewer than 2500 physicians nationwide are certified in addiction medicine. Inpatient addiction hospitalist services have been shown to improve patient outcomes, but their capabilities and impact are underexplored in community hospitals. This study evaluates the role of an addiction medicine hospitalist service in enhancing patient outcomes in two community-based hospitals.
METHODS: A retrospective chart review was conducted on patients who received care from addiction medicine service in two hospitals. The data were collected within the first 120 days of the establishment of servi

Approximately one million people in the US have permanent pacemakers. These patients are usually seen in general practice and referred to a cardiologist. Therefore, it is important for the internists and hospitalists to be aware of the indications and complications of cardiac pacing.

This anonymous survey of hospitalists and acute care nurses evaluated awareness of peripheral intravenous catheters (PIVC) presence and decision-making regarding PIVC insertion, maintenance, and removal. Nurses were most aware of PIVC presence and regarded as best to make decisions about PIVC, yet <50% of respondents felt nurses should remove PIVC without an order.

A medical doctor occasionally experienced itching papular exanthema during work hours. Careful history taking and a diary of symptoms demonstrated an association between the urticaria and stressful work situations. Urticaria can have many triggers, and stress is among the well-known triggers. The usual treatment of urticaria consists of avoiding triggers and, if necessary, antihistamines. Whereas it might be difficult to follow these recommendations in the case of work-related urticaria, the disorder may be perceived as a reminder of work situations which are going awry.

Hospitalist-focused training (electives, rotations, pathways, and tracks) evolved to address gaps in residency training pertinent to hospital medicine (HM). The immersion in hospital medicine elective (IHME) at Beth Israel Deaconess Medical Center immerses residents in curricular elements fundamental to HM, particularly clinical operations and healthcare economics. We surveyed IHME participants to evaluate the curriculum and characterize its impact on participants' professional development. Throughout the IHME's 20-year course, 120 residents participated, and 76 (63%) completed our survey. The

Disagreements, including those between residents and attending physicians, are common in medicine. In this Ethics Rounds article, we present a case in which an intern and attending disagree about discharging the patient; the attending recommends that the patient be hospitalized longer without providing evidence to support his recommendation. Commentators address different aspects of the case. The first group, including a resident, focus on the intern's potential moral distress and the importance of providing trainees with communication and conflict resolution skills to address inevitable confl

Hyperbaric oxygen (HBO2) is the inhalation of 100% oxygen at pressures > 1.4 times atmospheric pressure. Hyperbaric oxygen can be delivered in monoplace (single person) or multiplace (multi-person) chambers. Most clinical HBO2 exposures are between 2 and 2.4 atm abs for approximately 2 hours. Hyperbaric oxygen causes the blood and tissue oxygen levels to increase, reduces the volume of intravascular and tissue bubbles (to treat decompression sickness [DCS] and arterial gas embolism [AGE]), and accelerates wash-out of other gases, such as nitrogen or carbon monoxide (CO), which is important for

Balancing trainee autonomy with patient safety during clinical emergencies is an enduring challenge faced by hospitalists. In this article, we propose a framework to be used in urgent medical situations, such as hypotension or acute hypoxic respiratory failure. These strategies can be used to enhance early career trainee learning, confidence, and independence in such challenging situations by fostering a supportive learning environment while emphasizing both patient safety and trainee growth.

BACKGROUND: Adhesive small bowel obstruction (ASBO), although a potential surgical emergency, is increasingly being managed by medical hospitalists due to the likelihood these patients will not require operation. However, the value of care delivered by medical hospitalists to patients with ASBO has not been reported.
STUDY DESIGN: We hypothesized that patients admitted to the medical hospitalist service (MHS) for presumed ASBO have increased length of stay (LOS) and charges compared with patients admitted to the surgical service (SS). There were 555 consecutive admissions with presumed ASBO f

Disseminated candidiasis and other invasive fungal infections are a substantial cause of morbidity and mortality in immunocompromised and critically ill patients. Diagnosis of disseminated candidiasis via blood culture and skin biopsy can be unreliable and may delay treatment. (1,3)-β-D-glucan (BDG) assay is a rapid, cost-effective, noninvasive diagnostic screening tool for the dermatology hospitalist to consider.
