Immunomodulatory therapy for sepsis: from pathophysiological mechanisms to precision treatment.

Sepsis is a life-threatening syndrome marked by immune dysregulation, progressing from hyperinflammation to immunosuppression. The translation of immunomodulatory therapies has been hampered by the disease's extreme heterogeneity. This review synthesizes current progress and future perspectives in sepsis immunotherapy. We outline key immunopathological mechanisms and critically discuss evolving diagnostic tools, including dynamic biomarker monitoring and immune endotyping for personalized management. We then highlight novel therapeutic targets and explore how integrating single-cell technologi

Targeting Host Dependency Factors: A Paradigm Shift in Antiviral Strategy Against RNA Viruses.

RNA viruses, such as SARS-CoV-2 and influenza, pose a persistent threat to global public health. Their high mutation rates undermine the effectiveness of conventional direct-acting antivirals (DAAs) and facilitate drug resistance. As obligate intracellular parasites, RNA viruses rely extensively on host cellular machinery and metabolic pathways throughout their life cycle. This dependency has prompted a strategic shift in antiviral research-from targeting the mutable virus to targeting relatively conserved host dependency factors (HDFs). In this review, we systematically analyze how RNA viruse

Repurposing Tamoxifen as Potential Host-Directed Therapeutic for Tuberculosis.

The global burden of tuberculosis (TB) is aggravated by the continuously increasing emergence of drug resistance, highlighting the need for innovative therapeutic options. The concept of host-directed therapy (HDT) as adjunctive to classical antibacterial therapy with antibiotics represents a novel and promising approach for treating TB. Here, we have focused on repurposing the clinically used anticancer drug tamoxifen, which was identified as a molecule with strong host-directed activity against intracellular Mycobacterium tuberculosis (Mtb). Using a primary human macrophage Mtb infection mod

The role of DPP4 in viral infections: from receptor biology to antiviral target.

Dipeptidyl Peptidase 4 (DPP4) is a type II transmembrane serine protease with diverse physiological roles in metabolic regulation and immune modulation. Beyond its native functions, DPP4 has emerged as a critical host factor exploited by a range of viruses to drive infection and pathogenesis. This review synthesizes the multifaceted interactions between DPP4 and viral pathogens. We examine the structural basis for direct receptor binding by coronaviruses such as MERS and PHEV, as well as the dual-receptor entry mechanism used by human astroviruses. Additionally, the review explores how viruses

From Epigenetic Regulation to Protein Degradation: Emerging Strategies for Anti-Infective Drug Discovery.

Infectious diseases remain a major global health challenge, driven by antimicrobial resistance, pathogen persistence, and the limited integration of mechanistically innovative therapeutic approaches. Emerging evidence indicates that epigenetic regulation is fundamental to host-pathogen interactions, influencing transcriptional programmes associated with virulence, immune evasion, stress adaptation, and phenotypic plasticity. In organisms such as bacteria, parasites, and intracellular pathogens, including Mycobacterium tuberculosis and Plasmodium falciparum, chromatin-associated regulators and

Macrophage states and polarization during Mycobacterium tuberculosis infection: mechanisms, granuloma adaptation, and host-directed therapeutic opportunities.

Tuberculosis is a chronic infectious disease caused by Mycobacterium tuberculosis (Mtb). As primary host cells targeted by Mtb, macrophages play central roles in both innate and adaptive immunity. Accumulating evidence indicates that macrophage polarization is a key determinant of tuberculosis pathogenesis. In response to diverse microenvironmental cues, macrophages adopt functionally distinct polarization states, ranging from pro-inflammatory, microbicidal programs (M1-like) to anti-inflammatory, tissue-reparative programs (M2-like). These states differentially shape tuberculosis progression.

Transient, inhaled gene therapy with gamma interferon mitigates pathology induced by host response in a mouse model of tuberculosis.

Transient transfection of the respiratory mucosa of mice infected with Mycobacterium tuberculosis (Mtb) with gamma interferon (IFN-γ) promises benefits in disease therapy. We investigated preclinical efficacy of a dry powder inhalation (DPI) as a stand-alone versus adjunct to oral anti-tuberculosis (TB) chemotherapy in mice. We observed that this host-directed therapy mitigates the gross organ pathology and histopathology of lung and spleen tissue of infected mice receiving the DPI, either alone or as adjunct therapy. However, no statistically significant reduction in Mtb colony forming units

Harnessing miRNAs: A Novel Approach to Diagnosis and Treatment of Tuberculosis.

Mycobacterium tuberculosis (Mtb) complex, responsible for tuberculosis (TB) infection, continues to be a predominant global cause of mortality due to intricate host-pathogen interactions that affect disease progression. MicroRNAs (miRNAs), essential posttranscriptional regulators, have become pivotal modulators of these relationships. Recent findings indicate that miRNAs actively regulate immunological responses to Mtb complex by modulating autophagy, apoptosis, and immune cell activities. This has resulted in increased interest in miRNAs as prospective diagnostic indicators for TB, especially

Use of antibiotics in the management of postirradiation wound infection and sepsis.

Ionizing gamma irradiation depresses the host defenses and enhances the susceptibility of the immunocompromised host to local and systemic infection due to endogenous or exogenous microorganisms. Trauma and wounding act synergistically and decrease the survival after exposure to irradiation. The current antimicrobial agents suitable for controlling serious infections and their use in post irradiation local and systemic infection with and without trauma are discussed. The experience gained in managing immunocompromised patients following chemotherapy is reviewed. Empiric single agent or combina

Parasite and host kinases as targets for antimalarials.

INTRODUCTION: The deployment of Artemisinin-based combination therapies and transmission control measures led to a decrease in the global malaria burden over the recent decades. Unfortunately, this trend is now reversing, in part due to resistance against available treatments, calling for the development of new drugs against untapped targets to prevent cross-resistance. AREAS COVERED: In view of their demonstrated druggability in noninfectious diseases, protein kinases represent attractive targets. Kinase-focussed antimalarial drug discovery is facilitated by the availability of kinase-target

Updates in the clinical management of tuberculous meningitis.

INTRODUCTION: Tuberculous meningitis is frequently associated with significant mortality and persistent neurological sequelae. Diagnosis is often delayed due to nonspecific symptoms and the low sensitivity of cerebrospinal fluid (CSF) tests. Standard pulmonary tuberculosis regimens are suboptimal for central nervous system involvement due to poor drug penetration and prolonged treatment. Emerging resistance, particularly to rifampicin and isoniazid, further complicates the management. AREAS COVERED: This review synthesizes recent data particularly on diagnostic tools, drug treatment, host-dir

Host-directed antimicrobial drugs with broad-spectrum efficacy against intracellular bacterial pathogens.

We sought a new approach to treating infections by intracellular bacteria, namely, by altering host cell functions that support their growth. We screened a library of 640 Food and Drug Administration (FDA)-approved compounds for agents that render THP-1 cells resistant to infection by four intracellular pathogens. We identified numerous drugs that are not antibiotics but were highly effective in inhibiting intracellular bacterial growth with limited toxicity to host cells. These compounds are likely to target three kinds of host functions: (i) G protein-coupled receptors, (ii) intracellular ca

From acute infection to chronic osteoimmune remodeling: immunopathogenesis and precision immunotherapeutic strategies in osteomyelitis.

Osteomyelitis is a severe infectious and inflammatory disease that significantly affects bone tissue. It progresses from an acute inflammatory response to refractory chronic osteomyelitis and bone defects, constituting a core challenge in current clinical therapeutics and functional reconstruction. In the acute osteomyelitis stage, innate immune cells such as neutrophils, monocytes-macrophages, and dendritic cells are rapidly recruited and release large amounts of cytokines, chemokines, and extracellular traps. These work in synergy with adaptive immune responses, including T/B cells, to form
