[Biomechanical aspects in the treatment of periprosthetic humeral fractures].

Periprosthetic humeral fractures represent a relevant complication of shoulder arthroplasty and the incidence of 0.5-3% is expected to increase given current demographic trends. Biomechanical aspects are considered in therapeutic decision-making and planning, including fracture location and morphology as well as patient-specific factors. These include among others osteoporosis and age, but also other comorbidities of the patient, such as nicotine abuse or obesity. The fracture classification follows the internationally recognized system developed by Sanchez-Sotelo und Athwal. Conservative trea

Pediatric Medial Humeral Condyle Fractures: Classification, Treatment, and Outcomes.

BACKGROUND: The purpose of this study was to develop and validate a classification system that describes the injury pattern of pediatric medial humeral condyle fractures and provide treatment guidelines. METHODS: Patients less than 18 years old with medial humeral condyle fractures from 2012 to 2022 were identified. A classification system was developed based on fracture pattern and putative mechanism of injury. Type 1 fractures were characterized by a valgus/avulsion type injury while type 2 fractures were characterized by a vertical shear pattern. Each type was subdivided by amount of fragm

Humeral shaft fractures: how effective really is functional bracing?

BACKGROUND: The aim of this study was to assess the effectiveness of functional bracing in the treatment of humeral shaft fractures and identify factors that increased the risk of delayed or non-union. METHODS: All patients with humeral shaft fractures treated at Musgrove Park Hospital between 2010 and 2017 were identified. Patient electronic records were reviewed to identify demographic data, as well as outcomes and complications following treatment. Radiographs were reviewed to identify fracture pattern and location as well as fracture displacement and angulation. RESULTS: In total, 65 pat

Humeral shaft fractures: union outcomes in a large cohort.

BACKGROUND: Humeral shaft fractures can be managed conservatively or operatively. Fracture characteristics were analyzed to identify patients who would benefit from early operative fixation. METHODS: We performed a retrospective cohort study of 126 consecutive humeral shaft fractures (2008-2015). Fractures were classified according to fracture type, location, separation, and comminution. RESULTS: Of 126 patients, 96 were managed conservatively. In 54%, union occurred before 26 weeks, and 13% had delayed union after 26 weeks, whereas 33% did not achieve union. Of 30 patients managed surgicall

Long endomedullary nail in proximal third humeral shaft fractures.

INTRODUCTION: Proximal humeral fractures with a spiral line of fracture extending from the humeral head to the diaphyseal region are increasing. Treatment for these fractures is comparable to that for shaft fractures. The purpose of this study was to evaluate the use of a new "Long" humeral nail for this type of lesion and identify the best distal locking. MATERIALS AND METHODS: Forty-three patients treated with a Long Diphos Nail® were selected for this study: main exclusion criteria were poor cognitive and responsive ability to physical therapy, four-part fracture requiring humeral head rep

Proximal humeral fractures in Sweden-a registry-based study.

UNLABELLED: Proximal humeral fracture is the third most common osteoporotic fracture. To our knowledge, this is the first nationwide population-based registry study in adults that includes both inpatient and outpatient visits. Thus, we were able to report the true incidence rates and trends in the treatment of proximal humeral fractures. INTRODUCTION: Proximal humeral fractures are among the most common osteoporotic fractures. Valid epidemiologic population-based data, including both inpatient and outpatient visits, however, are lacking. METHODS: To investigate the Swedish national incidence

[Periprosthetic humeral fractures: Strategies and techniques of revision arthroplasty].

The primary aims when performing revision arthroplasty of periprosthetic humeral fractures (PHF) are preservation of bone stock, achieving fracture healing and preserving a stable prosthesis with the focus on regaining the preoperative shoulder-arm function. The indications for revision arthroplasty are given in PHF in combination with loosening of the stem. In addition, further factors must be independently clarified in the case of an anatomical arthroplasty. In this context secondary glenoid erosion as well as rotator cuff insufficiency are potential factors for an extended revision procedur

Treatment of Proximal Humeral Fractures: A Critical Analysis Review.

There is substantial variability in the treatment of proximal humeral fractures because of classification systems with poor interobserver reliability, rapid advances in technology (e.g., anatomically designed locking plates and reverse shoulder arthroplasty), and limited Level-I and II evidence for best treatments based on fracture patterns and physiological age. Almost three-quarters of proximal humeral fractures can be treated nonoperatively with good functional results as nearly 50% are nondisplaced or Neer one-part fractures. Another quarter occur in infirm patients or are characterized by

Secondary posterior interosseous nerve lesions associated with humeral fractures.

INTRODUCTION: Radial nerve lesions associated with humeral shaft fractures are the most common traumatic nerve lesions observed with long bone fractures. Secondary indirect posterior interosseous nerve (PIN) lesions can be associated with traumatic radial nerve palsy. The aim of this study was to identify cases of traumatic double-site radial nerve involvement through ultrasound (US). METHODS: Patients with traumatic radial nerve lesions referred to our laboratory from January 2010 to January 2014 were evaluated. RESULTS: Of the 35 patients, 18 had US evidence of a radial nerve lesion at the

Management of Humeral Shaft Fractures; Non-Operative Versus Operative.

CONTEXT: Functional humeral bracing remains the gold standard for treatment of humeral shaft fractures. There is an increasing trend in the literature to perform operative fixation of these fractures. EVIDENCE ACQUISITION: The aim of this systematic review was to compare the level one evidence for the outcome of non-operative with operative management of humeral shaft fractures in adults. A comprehensive electronic literature search of Medline and PubMed was performed with specific inclusion criteria to identify randomized controlled trials. RESULTS: In total, seventeen different studies wer

Operative versus nonoperative treatment in complex proximal humeral fractures.

This updated meta-analysis investigated whether operative treatment is superior to nonoperative treatment in complex proximal humeral fractures. The authors searched the Cochrane Central Register of Controlled Trials, PubMed, and EMBASE. Randomized controlled trials that evaluated operative vs nonoperative treatment for exclusively 3- or 4-part proximal humeral fractures were considered. Six studies with a total of 287 patients who had proximal humeral fractures were included. According to the meta-analysis, no statistically significant differences were found between operative and nonoperative

Surgical interventions for treating distal humeral fractures in adults.

BACKGROUND: Distal humeral fractures in adults are relatively uncommon injuries that require surgical intervention in most cases. There is a lack of consensus regarding the best management of distal humeral fractures in adults, including the role of conservative treatment, appropriate surgical approach, fixation strategies, the role of total elbow arthroplasty and handling of nerves such as the ulnar nerve. OBJECTIVES: To assess the effects (benefits and harms) of surgical interventions for distal humeral fractures in adults. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Tr

Emerging concepts in upper extremity trauma: humeral shaft fractures.

Fractures of the humeral shaft are common in low-energy and high-energy trauma, and optimal clinical management remains controversial. Nonsurgical management has been supported as the preferred treatment based on high union rates and minimal functional deficit due to a rich vascular supply from overlying muscle and the wide motion available at the glenohumeral joint. Recent studies of nonoperative management have challenged surgeons' understanding of these fractures and the perception of favorable outcomes. Current considerations support expanded operative indications with traditional open-pla

External fixation for displaced 2-part proximal humeral fractures.

Studies have reported conflicting results regarding external fixation for displaced proximal humeral fractures. Compared with open reduction and internal fixation, external fixation for displaced proximal humeral fractures avoids dissection and soft tissue stripping and leads to higher union rates, a lower incidence of avascular necrosis, less scaring of the scapulohumeral interface, and faster rehabilitation. Some authors have reported good or excellent results and minimum complications compared with open reduction and internal fixation; however, others have reported that external fixation do

Minimally invasive plate osteosynthesis for metastatic pathologic humeral fractures.

Because patients with metastatic bone disease may survive only 3 to 12 months, the goal of surgery for pathologic fractures is to attain rigid and durable internal fixation and immediate postoperative use of the upper extremity. Surgical options such as intramedullary nailing, plating, and insertion of prosthesis usually reinforced with bone cement have been proposed for pathological humeral fractures. We describe a 42-year-old man with a pathologic distal humeral fracture. The fracture location and lack of distal bone stock precluded the use of intramedullary nails. We performed minimally inv

Transosseous suture fixation of proximal humeral fractures. Surgical technique.

BACKGROUND: The optimal treatment of displaced fractures of the proximal part of the humerus remains controversial. We evaluated the long-term functional and radiographic results of transosseous suture fixation in a series of selected displaced fractures of the proximal part of the humerus. METHODS: Over an eleven-year period, a consecutive series of 188 patients with a specifically defined displaced fracture of the proximal part of the humerus underwent open reduction and internal fixation with transosseous sutures. Twenty patients were lost to follow-up and three died before the time of fol

Functional outcome after proximal humeral fractures treated with hemiarthroplasty.

OBJECTIVE: To evaluate functional outcome after hemiarthroplasty for displaced proximal humeral fractures and to review whether prosthesis type, intraoperative technique or previous ipsilateral shoulder surgery could affect the outcome. METHODS: We reviewed the medical records and radiographs of patients who had undergone hemiarthroplasty for proximal humeral fractures between 1992 and 2000. We identified 45 patients, 39 with acute fractures and 6 with fracture-related complications. One surgeon performed 17 surgeries (38%), and the rest were carried out by 11 other orthopedic surgeons. Minimally Invasive Anterior Plate Osteosynthesis in Humeral Shaft Fractures.

Twelve patients with shaft fractures of the humerus were treated with percutaneous anterior plate osteosynthesis using a deltoid-pectoral respectively brachialis splitting approach without exposure of the radial nerve. The implants used were PHILOS plates, locking compression plates 3.5 and 4.5, and a tibial metaphyseal plate (all by Synthes). There were no intraoperative complications, no infections and no iatrogenic injuries of the radial or axillary nerve. Nine fractures healed entirely. There was one pseudarthrosis caused by a plate that was too short; another fracture probably healed but

Isolated tuberosity fractures of the proximal humeral: current concepts.

Despite the relatively common occurrence of fractures of the proximal humeral amongst the elderly, the subgroup of isolated greater and lesser tuberosity fractures have remained less well understood. While the majority of two-part fractures result from a standing-height fall onto an outstretched hand, isolated tuberosity fractures are also commonly associated with glenohumeral dislocations or direct impact to the shoulder region. Inasmuch as isolated greater tuberosity fractures are considered uncommon, isolated lesser tuberosity fractures are generally considered exceedingly rare. Non-operati
