[Sequestered bone infection after Kirschner wire osteosynthesis of humeral head fracture].

Percutaneous stabilization of fractures of the humeral head by K-wires is a proved therapy. This proceeding provides enough stability for early moving of the joint and has a low incidence for complications. In the case described here the special is the cause for local infection: local osteonecrosis induced by drilling-holes too close together.

Long-term outcomes after osteochondral allograft transplantation to the humeral head.

BACKGROUND: Long-term outcomes of osteochondral allograft (OCA) transplantation to the humeral head have been sparsely reported in the literature. PURPOSE: To evaluate outcomes and survivorship of OCA transplantation to the humeral head in patients with osteochondral defects at a minimum of 10 years of follow-up. METHODS: A registry of patients who underwent humeral head OCA transplantation between 2004 and 2012 was reviewed. Patients completed pre and postoperative surveys including the American Shoulder and Elbow Surgeons score, Simple Shoulder Test, Short Form 12 (SF-12), and the visual a

Walch B0 glenoid: pre-osteoarthritic posterior subluxation of the humeral head.

The management of primary osteoarthritis of the shoulder has been well investigated. However, the etiology and management of posterior humeral head subluxation in the context of primary glenohumeral osteoarthritis remain controversial. The finding of static posterior subluxation of the humeral head before the development of posterior bone erosion of the glenoid in young men with radiographic findings of primary osteoarthritis has been described as arthrogenic posterior subluxation of the humeral head. It demonstrates the earliest form of the osteoarthritic evolution, and an excessive glenoid r

Partial Resurfacing for Humeral Head Defects Associated With Recurrent Shoulder Instability.

Recurrent traumatic shoulder instability is a complex clinical entity that commonly affects young, active patients. Humeral head defects are frequently associated with this condition, but specific treatment to stabilize the shoulder is rarely needed. Management options for defects of the humeral head that do necessitate treatment carry various risks and disadvantages, including the need for bone or soft tissue healing, complications related to hardware, and loss of motion. Partial prosthetic resurfacing has been reported as a treatment option. The current study retrospectively reviewed a cohor

Locked plate osteosynthesis of humeral head-splitting fractures in young adults.

BACKGROUND: Humeral head-splitting fractures occur in younger patients and can be associated with poor outcome. We decided to study the functional outcome and complications in simple and complex humeral head-splitting fractures. We hypothesized that simple head-splitting fractures will perform better compared with complex head-splitting fractures. PATIENTS AND METHODS: Records of 16 patients <55 years who underwent locked plating for humeral head-splitting fractures were reviewed. Five fractures were classified as simple (isolated head-splitting fractures) and 11 as complex fractures (associa

Humeral head cysts and rotator cuff tears: an MR arthrographic study.

OBJECTIVE: Humeral tuberosity cysts are a common finding, with previous reports suggesting they are related to rotator cuff tear or aging. The aim of this study was to investigate the characteristics of cysts in the tuberosities of the humeral head and their relationship with rotator cuff tear and age. DESIGN AND PATIENTS: Shoulder MR arthrograms were reviewed in 120 consecutive patients-83 males (mean age 38.0, range 19-59 years) and 37 females (mean age 41.2, range 15-59 years). Patients were referred for investigation of a variety of conditions, and instability was suspected in only a mino

Minimal invasive plate osteosynthesis for the dislocated humeral head fracture associated with ipsilateral humeral shaft fracture: A case report.

The condition combining a dislocated humeral head fracture and an ipsilateral humeral shaft fracture is seen to be rare in literature, which is limited to case report or case series. Furthermore, effective management of these coexistent injuries is still a subject of debate. The essential purpose of this article is to report another treatment method for this condition. We present a case of a 79-year-old female patient who suffered a three-part humeral head fracture-dislocation associated with the ipsilateral humeral shaft fracture. The combined injuries were managed by minimal invasive plate o

Preoperative measurement of the thickness of the center of the humeral head predicts screw cutout after locked plating of proximal humeral fractures.

HYPOTHESIS AND BACKGROUND: Preoperative computed tomography (CT) scans can be used to measure the thickness of the center of the humeral head to identify patients at a higher risk of screw cutout after open reduction-internal fixation. METHODS: At an academic medical center, we performed a retrospective review of all patients aged ≥ 18 years who had sustained a proximal humeral fracture that was treated with open reduction-internal fixation between January 1, 2005, and December 31, 2014, and who underwent preoperative shoulder CT. Ninety-four patients were included. Patient charts were review

Risk factors for osteonecrosis of the humeral head after internal fixation of proximal humeral fractures: a systematic review and meta-analysis.

BACKGROUND: Osteonecrosis of the humeral head (ONHH) is a severe complication after the internal fixation of proximal humeral fractures (IFPHF). The risk factors remain controversial though many studies have reported. In this research, meta-analysis was used to evaluate which surgeon-level factors can be modified to lower the risk and we hope to provide evidence-based support for preventing ONHH. METHODS: Literature was retrieved from PubMed, Cochrane Library, Embase, Web of Science, and Scopus for eligible studies published up to January 2023. The pooled odds ratios (ORs) were calculated wit

Proximal humeral osteolysis and glenoid radiolucent lines in an anatomic shoulder arthroplasty: a comparison of a ceramic and a metal humeral head component.

BACKGROUND: In vitro wear studies demonstrate decreased polyethylene wear with a ceramic compared with a metal humeral head. In this in vivo study, we analyzed prospectively collected 5-year data, comparing the outcomes of 2 types of humeral implants. METHODS: Sixty-two anatomic total shoulder arthroplasties (TSAs) (59 patients) were performed using the same glenoid component and 2 types of modular humeral implants (metal-head long-stem prosthesis in group A vs. ceramic-head stemless prosthesis in group B). Traumatic anterior fracture dislocation of the humeral head. A rare case report.

INTRODUCTION: Anterior fracture dislocation of the humerus with an anatomical neck fracture is an uncommon injury. These injuries pose a significant risk of devastating complications, such as avascular necrosis of the humeral head, due to the poor vascularization of the area, and stiffness resulting from prolonged immobilization. CLINICAL PRESENTATION: We report a case of a 20-year-old female who was involved in a motor vehicle crash and sustained an anterior fracture dislocation of the humerus with an anatomical neck fracture. The injury was successfully managed with open reduction and inter

Addressing the Hill-Sachs Lesion Humeral Head Bone Loss and Glenohumeral Instability.

Osseous injuries affecting the posterosuperolateral aspect of the humeral head, also known as the Hill-Sachs lesion, are common and can pose a difficult problem for the operating surgeon. Hill-Sachs lesions can lead to recurrent shoulder instability if not identified and addressed. This review will focus on osseous defects of the humeral head and address the pathoanatomy, diagnosis, indications, and surgical techniques for the treatment of these lesions.

Initial post-fracture humeral head ischemia does not predict development of necrosis.

We evaluated the functional outcome and the occurrence of avascular necrosis in 51 consecutive patients (26 women) with intracapsular fractures of the proximal humerus treated with open reduction and internal fixation between February 1998 and December 2001. Mean age was 44 years (range, 21-60 years). Forty-one heads were initially perfused (group A), and 10 were ischemic (group B). Seven patients were lost to follow-up. Forty-four were evaluated using the Constant-Murley score and the Subjective Shoulder Value; 40 patients consented to a radiographic evaluation. The mean follow-up was 5 years

Combined glenoid and humeral head allograft reconstruction for recurrent anterior glenohumeral instability.

Recurrent glenohumeral instability can be a challenging therapeutic problem to orthopedic surgeons. Oftentimes, recurrent anterior glenohumeral instability is associated with a host of capsular, labral, and occasionally, osseous deficiencies. Current treatment strategies for significant bony deficiency within the shoulder are aimed at nonanatomic restoration of glenohumeral anatomy with resultant limitation in range of motion parameters. While these techniques may prevent recurrence of instability, the resulting loss of motion and decreased function may not be suitable for young, active indivi

Predictors of humeral head ischemia after intracapsular fracture of the proximal humerus.

The purpose of this study was to evaluate predictors of fracture-induced humeral head ischemia. Between February 1998 and December 2001, 100 intracapsular fractures of the proximal humerus, treated by open surgery, were included in a prospective surgical evaluation protocol (mean age, 60 years; minimum, 21 years; maximum, 88 years; 45 men; 57 right shoulders). Fracture morphology was assessed following a structured questionnaire and based on radiographic and intraoperative findings. Perfusion was assessed intraoperatively by observation of backflow after a borehole was drilled into the central

[Primary humerus head replacement in dislocated proximal humeral fracture. Indications, technique, results].

Displaced three- and four-part fractures of the humeral head detached from their blood supply are at high risk for a poor functional outcome after internal fixation. Primary prosthetic replacement can restore the pretraumatic anatomy, and early function is possible. Head replacement within 2 weeks after trauma, reapproximation of the tuberosities, and reconstruction of the rotator cuff tear establishes early stability and good functional results in up to 80%.
