Progression of hydroxychloroquine-induced retinopathy after drug cessation.

PURPOSE: Hydroxychloroquine (HCQ) is a frequently used drug for a variety of rheumatological conditions. One significant adverse effect is HCQ retinopathy, which irreversibly affects the photoreceptor layer and retinal pigment epithelium, causing paracentral scotomas and potential visual loss. Retinal impairment is irreversible, even after termination of therapy. This study investigates the progression of the retinal destruction after drug cessation. SETTING/VENUE: The study was performed at the Departments of Ophthalmology at Zurich University Hospital and Tel Aviv Sourasky Medical Center fr

Efficient pair, hydroxychloroquine monotherapy for tattoo-associated sarcoidosis.

Sarcoidosis is a systemic granulomatous disease which may affect the skin in about one-third of patients. It is characterised by the histopathological finding of non-caseating granulomas. There is a predilection for cutaneous sarcoidosis towards prior trauma sites. Tattoo-associated sarcoidosis may be caused by the injected foreign pigment or its by-products. These reactions are commonly observed with black and red dyes, although other colours have been implicated. Currently, there are no evidence-based treatment regimens of tattoo sarcoidosis; however, the most commonly used treatments leadin

Hydroxychloroquine reduces T cells activation recall antigen responses.

BACKGROUND: In the context of the current COVID-19 pandemic, there is still limited information about how people suffering from autoimmune diseases respond to the different COVID vaccines. The fact that they are taking an immunosuppressant or other drugs that aim to decrease the immune system activities, such as hydroxychloroquine (HCQ), could also impact their ability to respond to a COVID vaccine and vaccines in general. METHODS: Heathy donors were given 200mg of HCQ daily for 6-weeks to assess HCQs impact on the systemic T cells and humoral immune response. Peripheral blood mononuclear cel

ASYMMETRIC HYDROXYCHLOROQUINE MACULAR TOXICITY WITH APHAKIC FELLOW EYE.

BACKGROUND/PURPOSE: Retinal toxicity associated with antimalarial drug use in inflammatory conditions is well described and may be more common than previously recognized. Antimalarial drugs bind to melanin in ocular tissues, particularly the retinal pigment epithelium, but the mechanism of toxicity and its relation to light is unclear. METHODS: Case report. RESULTS: A 62-year-old white woman with erosive rheumatoid arthritis developed hydroxychloroquine toxicity in her phakic eye, with her aphakic fellow eye only mildly affected. CONCLUSION: We report the clinical evaluation of this rare ca

Hydroxychloroquine in COVID-19 Therapy: Protection Versus Proarrhythmia.

In recent months, the new coronavirus SARS-CoV-2 has emerged as a worldwide threat with about 4.2 million confirmed cases and almost 300 000 deaths. Its major clinical presentation is characterized by respiratory symptoms ranging from mild cough to serve pneumonia with fever and potentially even death. Until today, there is no known medication to improve clinical symptoms or even prevent or fight the infection. The search for a useful vaccination is ongoing and it will probably not be available before the end of 2020. In this review, we highlight hydroxychloroquine (HCQ) as a potential agent t

Hydroxychloroquine antiparkinsonian potential: Nurr1 modulation versus autophagy inhibition.

The nuclear orphan receptor (Nurr1) has recently received a perceivable solicitude as a target for the therapeutic intervention against PD. Meanwhile, the dysregulation of autophagy, along with other processes is believed to contribute massively to PD pathophysiology. Hydroxychloroquine, a hydroxy derivative of chloroquine, is an antimalarial agent which is also used as an anti-rheumatic drug. The neuroprotective potential of hydroxychloroquine and chloroquine remained controversial until recently a study showed that chloroquine exhibited an antiparkinsonian activity through Nurr1 modulation.

Primary Sjögren's syndrome: Extraglandular manifestations and hydroxychloroquine therapy.

The use of hydroxychloroquine (HCQ) in Primary Sjögren's Syndrome (pSS) has been assessed in different studies over the last years, with conflicting results regarding its efficacy in sicca syndrome and extraglandular manifestations (EGM). The goal of this study was to compare the incidence rate of EGM in pSS patients with and without HCQ therapy. We performed a multicenter retrospective study, including patients with pSS (European classification criteria) with at least 1 year of follow-up. Subjects with concomitant fibromyalgia, autoimmune hepatitis, primary biliary cirrhosis, and primary scler

Optimizing Hydroxychloroquine Blood Levels and Long-Term Hydroxychloroquine Intake Could Lower Atherosclerotic Cardiovascular Disease Risk and Prevent Polypharmacy in Systemic Lupus Erythematosus.

OBJECTIVE: Hydroxychloroquine (HCQ) is the cornerstone of systemic lupus erythematosus (SLE) management with benefits extending beyond SLE control, including protection against atherosclerotic cardiovascular disease (ASCVD). Although HCQ blood levels reflect recent exposure and long-term intake reflects medication adherence, the impact of longitudinal changes in both measures on ASCVD risk over time remains unclear. We evaluated how HCQ blood levels and long-term intake patterns relate to estimated ASCVD risk at baseline and after one year. METHODS: In this prospective longitudinal study, 248

Hydroxychloroquine Attenuates hERG Channel by Promoting the Membrane Channel Degradation: Computational Simulation and Experimental Evidence for QT-Interval Prolongation with Hydroxychloroquine Treatment.

INTRODUCTION: The coronavirus disease 2019 (COVID-19) pandemic has led to millions of confirmed cases and deaths worldwide and has no approved therapy. Currently, more than 700 drugs are tested in the COVID-19 clinical trials, and full evaluation of their cardiotoxicity risks is in high demand. METHODS: We mainly focused on hydroxychloroquine (HCQ), one of the most concerned drugs for COVID-19 therapy, and investigated the effects and underlying mechanisms of HCQ on hERG channel via molecular docking simulations. We further applied the HEK293 cell line stably expressing hERG-wild-type channel

A case of severe hydroxychloroquine-induced retinal toxicity in a patient with recent onset of renal impairment: a review of the literature on the use of hydroxychloroquine in renal impairment.

We present a case of a 67-year-old female who presented with a twelve-month history of progressive blurred vision in both eyes. The patient was on hydroxychloroquine 200 mg twice a day for eight years for the treatment of scarring alopecia. Two years prior to presenting, the patient was found to have chronic kidney disease stage 3 secondary to hypertension. Examination revealed bilateral reduced visual acuities with attenuated arterioles and pigmentary changes on retinal assessment. Goldmann visual fields showed grossly constricted fields in both eyes. The patient was diagnosed with retinal to
