Ventilatory responses of the clown knifefish, Chitala ornata, to hypercarbia and hypercapnia.

The aim of the present study was to determine the roles of externally versus internally oriented CO2/H+-sensitive chemoreceptors in promoting cardiorespiratory responses to environmental hypercarbia in the facultative air-breathing fish, Chitala ornata (the clown knifefish). Fish were exposed to environmental acidosis (pH ~ 6.0) or hypercarbia (≈ 30 torr PCO2) that produced changes in water pH equal to the pH levels of the acidotic water to distinguish the relative roles of CO2 versus H+. We also injected acetazolamide to elevate arterial levels of PCO2 and [H+] in fish in normocarbic water to

Effects of hypoxia and hypercapnia on human HRV and respiratory sinus arrhythmia.

PURPOSE: Hypercapnia increases minute ventilation (V’E) with little effect on heart rate (HR), whereas hypoxia may increase HR without affecting V’E. However, the effects of hypercapnia and hypoxia on both heart rate variability(HRV) and the clustering of heart beats during spontaneous breathing (respiratory sinus arrhythmia – RSA), are not clear. METHODS: In this study, 10 human volunteers breathed room air (RA), hypercapnic (5% CO2) or hypoxic (10%O2) gas mixtures, each for 6 min, while resting supine. ECG, mean arterial pressure (MAP), ventilatory flow, inhaled and exhaled fractions of CO2

Therapeutic hypercapnia is not protective in the in vivo surfactant-depleted rabbit lung.

Permissive hypercapnia because of reduced tidal volume is associated with improved survival in lung injury, whereas therapeutic hypercapnia-deliberate elevation of arterial Pco2-protects against in vivo reperfusion injury and injury produced by severe lung stretch. No published studies to date have examined the effects of CO2 on in vivo models of neonatal lung injury. We used an established in vivo rabbit model of surfactant depletion to investigate whether therapeutic hypercapnia would improve oxygenation and protect against ventilator-induced lung injury. Animals were randomized to injurious

Neuroimaging evidence implicating cerebellum in the experience of hypercapnia and hunger for air.

Recent neuroimaging and neurological data implicate cerebellum in nonmotor sensory, cognitive, vegetative, and affective functions. The present study assessed cerebellar responses when the urge to breathe is stimulated by inhaled CO(2). Ventilation changes follow arterial blood partial pressure CO(2) changes sensed by the medullary ventral respiratory group (VRG) and hypothalamus, entraining changes in midbrain, pons, thalamus, limbic, paralimbic, and insular regions. Nearly all these areas are known to connect anatomically with the cerebellum. Using positron emission tomography, we measured r

Rapid response to inhaled frusemide in severe acute asthma with hypercapnia.

We report 7 patients with severe acute asthma unresponsive to standard medication, including sympathomimetic agents, aminophylline and corticosteroids, who responded to inhaled frusemide. All were hypercapneic with a mean PaCO2 of 7.7 kPa (57.7 mm Hg) [range 6.2-8.8 kPa (46.2-66.3 mm Hg)]. Following nebulization of 20 mg frusemide, clinical response was rapid, and the mean PaCO2 fell significantly to 5.4 kPa (40.6 mm Hg) [range 5.0-6.2 kPa (37.5-46.5 mm Hg)] within 20-60 min. No adverse effect was recognized. Inhaled frusemide should be considered for treatment of acute asthma refractory to co

Effects of hypercapnia on metabolism, temperature, and ventilation during heat and fever.

We wished to determine whether 1) acute hypercapnia in cats changes metabolic heat production to affect temperature regulation and 2) heat exposure and fever affect the normal response. The effects of breathing 4% CO2 on O2 consumption (VO2), rectal temperature (Tre), and ventilation (V) were measured in five conscious resting cats. Cats were exposed to a normal (24-27 degrees C) chamber temperature (Ta) and a warm (33-34 degrees C) chamber Ta. Fever was produced by intravenous injection of Escherichia coli endotoxin (0.02 microgram/kg) at a normal Ta. In normothermic cats, hypercapnia decreas

[Hypercapnia and mixed acidosis as the only sign of malignant hyperthermia (MH)?].

CASE REPORT: A 34-year-old male (190 cm/100 kg) was scheduled for surgery of the nasal septum. He had had uneventful anaesthesia for appendicectomy 14 years earlier: following 600 mg thiopentone, 180 mg suxamethonium and up to 2 vol.% halothane for 20 min had been used and no symptoms of malignant hyperthermia (MH) were recorded. Following oral premedication with 2 mg flunitrazepam at 7.00 a.m. anaesthesia was induced with a priming dose of atracurium at 8.45 a.m. followed by 0.2 mg fentanyl, 500 mg thiopentone, and 100 mg suxamethonium. Endotracheal intubation was accomplished easily, and the

Effect of hypercapnia on laryngeal airway resistance in normal adult humans.

Laryngeal airway resistance (Rlar) was measured in eight normal adult humans during progressive hyperoxic hypercapnia. In most subjects, the translaryngeal pressure-flow relationship appeared linear under normocapnic conditions. During hypercapnia, the pressure-flow relationship on inspiration and expiration was curvilinear with increasing translaryngeal pressure associated with progressively smaller increments in flow. Translaryngeal pressure-flow relationships at different CO2 levels were compared over their common flow ranges by performing a least-squares linear regression on data throughou

[A case of pulmonary embolism following acute respiratory failure with hypercapnia].

A 18-year-old boy was admitted to hospital in an unconsciousness state as a result of taking a large dose of several psychotropic drugs simultaneously in an attempt to commit suicide. Blood studies revealed hypoxia (55.7 mmHg) and hypercapnia (59.7 mmHg). Hypoxia (74.3 mmHg) and hypercapnia (46.7 mmHg) were still present on the fourth day after admission, and the patient was becoming lethargic. Reduced vascular markings in the right upper lung field on chest roentgenogram in spite of hypercapnia suggested that the persistent hypoxia was the result of a pulmonary embolism. This diagnosis was su

Carotid body amine levels in goats exposed to hypoxia or hypercapnia.

The carotid body (CB) contains large amounts of several monoamines. There is considerable evidence that carotid body (CB) chemoreceptor function may be regulated by one or several of these monoamines. In order to test whether conditions stimulating the CB might change the CB content of these monoamines, we measured the norepinephrine, dopamine, 5-hydroxyindoleacetic acid, and 5-hydroxytryptamine content of carotid bodies collected from goats exposed to 4 h of either normoxic-normocapnic, hypoxic-normocapnic, or normoxic-hypercapnic gas mixtures. We found that there were no consistent changes i

Ventilatory responses to hypoxia and hypercapnia in asthmatics with previous respiratory failure.

The ventilatory response to hypoxia and the ventilatory and mouth occlusion pressure response to hypercapnia was measured in 13 subjects who had previously developed respiratory failure or respiratory arrest during an acute asthma attack. In 11 of 12 subjects tested there was a normal response to hypercapnia. Six of the 13 subjects had an impaired response to hypoxia. Impaired hypoxic responsiveness may contribute to the early onset of hypercapnic respiratory failure during acute severe asthma.

Effects of naloxone on breathing movements during hypercapnia in the fetal lamb.

To determine whether endogenous opioids influence the fetal breathing response to CO2 we have investigated the effect of the opiate antagonist, naloxone on the incidence, rate, and amplitude of breathing movements during hypercapnia in fetal lambs in utero. In 20 experiments in six pregnant sheep (130-145 days gestation) hypercapnia was induced by giving the ewe 4-6% CO2-18% O2 in N2 to breathe for 60 min. After 30 min of hypercapnia either naloxone (13 experiments) or saline (7 experiments) was infused intravenously for the remaining 30 min. During hypercapnia breath amplitude increased from

The purpose of this study was to determine the content of catecholamines (CA) in the cat carotid body before and after 0.5 h exposures to normoxic normocapnia [arterial O2 partial pressure (Pao2) 126 +/- 28 Torr, arterial CO2 partial pressure (Paco2) 36.4 +/- 1.5 Torr], hypoxic normocapnia (Pao2 25 +/- 3 Torr, Paco2 36.7 +/- 3.3 Torr), and normoxic hypercapnia (Pao2 132 +/- 13 Torr, Paco2 = 98.2 +/- 7.6 Torr). CA synthesis was blocked using alpha-methylparatyrosine methyl ester (AMPT) prior to alterations in the inspired air. There was a significant decrease in carotid body content of dopamine

Neural responses of the cat carotid and aortic bodies to hypercapnia and hypoxia.

The response (imp . s-1) of single- or few-fiber preparations from the carotid body (10 experiments) and the aortic body (5 experiments) to various levels of hypercapnia on different backgrounds of hypoxia were analyzed by two statistical techniques--analysis of variance and the Duncan's new multiple-range test. These analyses showed an initial statistically significant increase in the slope of the response to increasing arterial pressure of CO2 (PaCO2) as PaO2 fell. But the slope of the response to carbon dioxide later showed a clear tendency to become less; i.e., no significant increase in i

Effects of hypercapnia and hypoxia on phrenic nerve activity and respiratory timing.

In the spontaneously breathing animal, respiratory responses to chemical stimuli are influenced by phasic proprioceptive inputs from the thorax. We have compared the effects of hypercapnia and hypoxia on the level and timing of phrenic nerve activity while these phasic afferent signals were absent. Progressive hyperoxic hypercapnia and isocapnic hypoxia were produced in anesthetized paralyzed dogs by allowing 3-5 min of apnea to follow mechanical ventilation with 100% O2 or 35% O2 in N2, respectively; during hypoxia, isocapnia was maintained by intravenous infusion of tris(hydroxymethyl)aminom

[Investigation of orthostatic hypotension by the provoked hypercapnia test (author's transl)].

Orthostatic hypotension results from an alteration in the baroreflex at any level with subsequent disturbance in blood pressure homeostasis in response to variations. The second part of this reflex arc, from the bulbar vasopressor centre to the peripheral effector sites, can be explored directly by studying the hemodynamic response to an increase in arterial CO2 levels. Normally the response is an increase in blood pressure, but under abnormal conditions there is a reduction. Orthostatic hypotension with a normal hemodynamic response to increased CO2 levels indicates that the first part of the

Assessment of the central chemosensitivity in man under transient or progressive hypercapnia.

In healthy man, the central chemosensitivity to CO2 was studied after depression of the arterial chemoreflex drive by inhalation of pure oxygen. The effectiveness of the functional decrease of arterial chemoreceptor function was assessed by the delayed hyperventilation which followed transient inhalation of hypercapnic gas mixtures for 3 or 5 breaths in hyperoxic conditions. In such a case the first significant increase in tidal volume (VT) occurred 13.9 +/- 3.2 (SE) sec later than the early change in this variable measured in normoxic conditions. The stimulus strength was estimated by the cha

Acid-base changes in acute exacerbation of chronic hypercapnia in man.

Acid-base parameters were studied in 10 adult patients with chronic respiratory failure during spontaneous acute exacerbations of chronic hypercapnia and after return and stabilization of PaCO2 values at previous levels with improvement of clinical conditions. All factors interfering with acid-base equilibrium were carefully avoided. The results of this investigation confirm qualitatively those obtained by GOLDSTEIN et al. [7] in dog studies and those of INGRAM et al. [9]in human studies: increasing degrees of chronic hypercapnia tend to minimize the pH reduction induced by acute PACO2 changes
