The authors, based on their own material, evaluated the value of hysteroscopic treatment of endometrial polyps in a group of infertile women. Polypectomy was performed in 25 patients using either endoscopic microscissors or electric loop. Follow-up hysteroscopy was performed after two months as an integral part of the treatment. All the patients were observed for 12 months. The primary intrauterine investigation was complete in all patients without regard to kind of instrument which was confirmed by second-look hysteroscopy. About 80% of the patients who underwent surgery conceived. Restoratio

Hysteroscopic surgery replaced abdominal metroplasty and is today the treatment of choice for congenital uterine malformations. This is not just because of its reproductive results, which are comparable to those achieved with the abdominal approach, but mainly because of several post-operative benefits (reduced morbidity, convalescence and costs, and no scar tissue on the abdominal and uterine walls), improved reproductive performance (no reduction in uterine volume, shorter interval to conception after operation) and the mode of delivery (avoiding Caesarean section). Decisions on when and how

OBJECTIVES: Determine the effectiveness of hysteroscopic surgery in persistent postmenopausal bleeding.
PATIENTS AND METHODS: Transcervical resection was performed in 176 women between the ages of 46-74 years. A benign cause was noted in 135 cases (polyps in 80 and submucous fibroids in 55); 37 patients had no significant disease and 4 had endometrial atypical hyperplasia missed in the preoperative evaluation. Patients underwent resection of polyp, resection of fibroid or endometrial ablation. Major operative complications were rare and included 3 perforations and one case of fluid overload.

BACKGROUND: Endometrial ossification is a rare condition. The common feature in most reported cases is a previous history of abortion and retention of fetal bones. Few cases of endometrial ossification can be explained by osseous metaplasia. Clinical presentation may include abnormal vaginal bleeding or discharge, dysmenorrhoea, pelvic pain, and secondary infertility. Hysterectomy or dilation and curettage have been the usual therapeutic methods used. Recently, some cases have been treated by means of hysteroscopic resection.
CASE: We report a case of endometrial ossification in a woman who p

OBJECTIVE: To review the experience with hysteroscopic treatment of uterine septa, particularly those involving the cervix.
METHODS: One hundred twenty-four patients with septate uterus were treated hysteroscopically. Seven of these women had a uterine septum involving the uterus and cervix. All patients but nine who were infertile had demonstrated repetitive pregnancy losses with nothing to explain the reproductive failure but the uterine septation. The preoperative reproductive performance included 299 pregnancies, of which 258 were spontaneous abortions (86.6%) and 28 preterm pregnancies (

A series of 17 patients with significant intrauterine synechiae is presented with a demonstration of various adjunctive therapeutic measures which may be helpful. The procedure is ideally performed under ultrasound control and this is essential if the adhesions are extensive. There may be a place for postoperative use of an intrauterine contraceptive device or of hormone therapy, but these modalities are not universally accepted. However, 11 of 13 patients regained normal menstruation and 8 of the 10 desiring pregnancy achieved livebirths with these methods. Four of the latter had had severe a

OBJECTIVE: To examine the reproductive outcome after operative hysteroscopic resection of submucous myomas in women for whom no other infertility factor was identified.
DESIGN: Fifteen infertile women with submucous myomas underwent an intensive workup to exclude other causes of infertility. Operative hysteroscopy for resection of the submucous myomas was performed using a rigid 26 French resectoscope (Karl Storz GmbH & Co., Tuttlingen, Germany).
SETTING: Academic tertiary referral center.
RESULTS: The mean +/- SD duration of the procedure was 25.5 +/- 5.6 minutes. No operative or postopera

At the Nepean Hospital transabdominal ultrasound has been used to assist the hysteroscopic surgeon when performing complicated transcervical operations. The 2 main areas of use appear to be either to direct the surgeon within the uterus to the site of pathology or to prevent inadvertent perforation of the uterine wall. The cases vary from haematometra following endometrial ablation to Asherman syndrome and subseptate uteri.

Hysteroscopic resection of submucous myomas was done in 27 patients with the Karl Storz equipment. Among them 20 were peduncular myomas, 5 of which dropped out of the external of cervix, 7 were sessile myomas. Under ultrasonography the largest sessile myoma was 5cm in diameter. The depth of the uterine cavity was less than 10cm in all cases. For the sessile submucous myomas, the surface vessels were electro-coagulated first, then the myomas were electro-resected chip-by-chip. Peduncular for the myomas, if the pedicle was larger than 1cm, it was electro-resected, if it was less than 1cm, the my

Fifty submucous fibroids were removed by hysteroscopic resection from 37 patients with abnormal uterine bleeding. Thirty-seven fibroids were completely resected and 13 incompletely resected. Nine patients also had rollerball endometrial ablations. The average follow-up period was 14 months (4 to 51 months) in 33 patients available for follow-up. All of these patients had cure of their symptoms, 3 after repeat operations. The 1 immediate complication was a postoperative infection. A normal uterine cavity was found in 13 of 14 patients examined by office hysteroscopy at follow-up.

OBJECTIVES: The purpose of this study was to evaluate the effects of pretreatment with GnRH analogs and hysteroscopic resection of submucous myomas previously deemed to be contraindicated for this approach.
METHODS: Twenty-five patients were included in this study. Patients were assessed by ultrasonography and diagnostic hysteroscopy. They were treated with depot GnRH analogs for 3 months and then scheduled for hysteroscopic surgery.
RESULTS: After GnRH therapy the diameter was 61 +/- 10% of the initial diameter. In one patient, therapy was almost ineffective. Hysteroscopic surgery was possi

OBJECTIVE: To assess the efficacy of hysteroscopic endometrial ablation with the rollerball resectoscope.
METHODS: From April 1989 to March 1991, 64 women underwent hysteroscopic endometrial ablation using electrosurgery. Telephone follow-up was obtained for 61 patients at least 6 months after the procedure. The majority of patients requested endometrial ablation because of irregular heavy menses, and two patients presented with postmenopausal bleeding. All patients had preoperative endometrial sampling that demonstrated benign endometrial histology. Five women had previous endometrial ablati
