ADAPT and FRAME guided implementation science approach to improve care transitions in older colorectal surgery patients: a qualitative study.

BACKGROUND: Older adults are disproportionately vulnerable to adverse outcomes during postoperative care transitions. While evidence-based care transition interventions exist, such as the Care Transitions Intervention (CTI), their core components have not been systematically adapted for surgical populations. Concurrently, geriatric surgery co-management models, such as the Optimization of Senior Care and Recovery (OSCAR) model, improve outcomes but do not address the post-discharge transition. To bridge this gap, we merged the geriatrics-informed, inpatient co-management model of OSCAR with th

Strengthening PrEP services at community-based organizations for Latinx men who have sex with men: an implementation science approach.

BACKGROUND: In Miami-Dade County Florida (Miami), the rate of newly diagnosed HIV per 100,000 population is four times the national rate with a large majority of transmission due to male-to-male sex. Latinx men are prescribed HIV preventative medication (PrEP) at a lower rate than White individuals; as such, efforts are being made by community health organizations to engage Latinx men who have sex with men (LMSM) in PrEP promotion programs. The objective of this study is to characterize the features, strengths, and weaknesses of the implementation of a PrEP promotion program (DiversiPrEP) at t

Charting a path forward toward integrating macrolevel phenomena, marginalization, and health equity principles in into dissemination and implementation science.

Dissemination and implementation (D&I) science is ascendant within public health in the United States, but D&I scientists have long voiced concern at their field's capacity to fulfill two essential and intertwined public health responsibilities: analyzing and intervening in macrosocial determinants and achieving health equity. To help break this impasse, we explore D&I's theoretical roots by critically reviewing three historical eras, eras that D&I's origin story has identified as foundational to D&I's development: 1890-1903: Tarde's Laws of Imitation. 1920-1960: Rural Sociology. 1960-1980: Kn

Emerging Interventions to Improve Health Outcomes for People Aging With HIV: Protocol for a Mixed Methods Implementation Science Evaluation.

BACKGROUND: In 2022, 54% of people with HIV were aged 50 years and older; however, clinical care for HIV in the United States often falls short of comprehensively integrating care for aging-related conditions. In response, the Health Resources and Services Administration HIV/AIDS Bureau Ryan White HIV/AIDS Program funded a new initiative comprising 10 demonstration sites to test emerging interventions to support people aging with HIV, as well as a capacity-building provider and an evaluation provider. NORC at the University of Chicago received an award for the evaluation provider. OBJECTIVE:

Successful Deployment of High Flow Nasal Cannula in a Peruvian Pediatric Intensive Care Unit Using Implementation Science-Lessons Learned.

Acute lower respiratory infections are the leading cause of death outside the neonatal period for children less than 5 years of age. Widespread availability of invasive and non-invasive mechanical ventilation in resource-rich settings has reduced mortality rates; however, these technologies are not always available in many low- and middle-income countries due to the high cost and trained personnel required to implement and sustain their use. High flow nasal cannula (HFNC) is a form of non-invasive respiratory support with growing evidence for use in pediatric respiratory failure. Its simple in

Using Systems Engineering and Implementation Science to Design an Implementation Package for Preoperative Comprehensive Geriatric Assessment Among Older Adults Having Major Abdominal Surgery: Protocol for a 3-Phase Study.

BACKGROUND: Older Americans, a growing segment of the population, have an increasing need for surgical services, and they experience a disproportionate burden of postoperative complications compared to their younger counterparts. A preoperative comprehensive geriatric assessment (pCGA) is recommended to reduce risk and improve surgical care delivery for this population, which has been identified as vulnerable. The pCGA optimizes multiple chronic conditions and factors commonly overlooked in routine preoperative planning, including physical function, polypharmacy, nutrition, cognition, mental h

Readiness assessment as a strategy, not an afterthought: leveraging implementation science to integrate tuberculosis comorbidity care in Pakistan's health system.

Health facility readiness assessment is a foundational element in designing and implementing integrated tuberculosis (TB) and comorbidity interventions, particularly in low- and middle-income countries (LMICs) like Pakistan. The integration of TB care with management of comorbid conditions like diabetes mellitus, depression, and tobacco use is complicated and challenged by diverse, multifaceted barriers at the health system, organizational, and individual levels. Evidence-based frameworks, such as Consolidated Framework for Implementation Research (CFIR), Reach, Effectiveness, Adoption, Implem

A call to action: dissemination and implementation science education and training is needed in physical therapy and athletic training health professions.

Prevention and appropriate management of musculoskeletal (MSK) conditions is a key contributor to a healthy and well population. Physical therapists (PTs) and athletic trainers (ATs) are allied health professionals at the forefront of prevention and management of MSK conditions, working in wide-ranging healthcare settings, from outpatient clinics, with specialized populations (e.g., athletes), within schools, and in industrial settings and public health. PTs and ATs receive extensive education in evidence-based practice for the prevention and treatment of common MSK conditions, but little to n

Context analysis as a foundational step to inform the design and implementation of an interprofessional primary care model for new long-term medication users - the myCare Start Implementation Science Project.

BACKGROUND: Implementation science recognises that health innovations must be adapted to local contexts. Context analysis is a critical foundational phase for tailoring interventions and implementation strategies, yet remains underutilised in pharmaceutical sciences, limiting successful translation and scale-up of proven services in interprofessional settings. AIM: This paper provides a methodological roadmap highlighting the foundational role of context analysis in guiding real-world adaptation of a known service - the New Medicine Service - to form the myCare Start service in Switzerland.

Implementation science approaches to optimizing venous thromboembolism prevention in patients with traumatic injuries: Findings from the 2022 Consensus Conference to Implement Optimal Venous Thromboembolism Prophylaxis in Trauma.

Disease burden of venous thromboembolism (VTE) without pharmacologic prophylaxis is high in trauma patients. Although VTE prophylaxis guidelines exist, studies on real-world use of VTE prophylaxis in trauma shows limited uptake of guideline recommendations. Despite existing guidelines, reports indicate that VTE prophylaxis implementation across trauma centers is lagging. Implementation barriers of VTE prophylaxis in trauma are multifactorial, and VTE prescribing practices require further optimization. Implementation science methods can help standardize and improve care; well-established approa

Advancing youth HIV prevention and care in Uganda: stakeholder-informed innovations through implementation science, protocol for a randomized controlled trial.

BACKGROUND: Compared with older adults, adolescents and young adults (AYA) face greater HIV incidence and poorer outcomes across the HIV prevention and care continuum. Despite the high HIV burden, youth in sub-Saharan Africa have suboptimal uptake rates of oral preexposure prophylaxis (PrEP) and viral suppression. Although effective interventions are available, AYA exhibit low uptake and persistence due to stigma, pill burden, and accessibility challenges. The HIV Prevention and Care Interventions for Youth in Uganda (HIP-CY) study aims to explore the adoption and effectiveness of cabotegravir

Comparative analysis of Egypt's malaria elimination strategies and implementation science: Pathways to achieve malaria-free status for other African countries.

BACKGROUND: Malaria elimination is a critical public health goal, particularly in Africa, where the disease disproportionately affects vulnerable populations. Egypt's success in achieving World Health Organization (WHO) malaria-free certification on October 20, 2024, through an evidence-based malaria elimination program, offers a valuable model for replication. Our study is identified as an implementation study, evaluating the evidence-based interventions deployed, the implementation strategy tested, and its outcomes to provide insights for scaling similar programs across Africa. Understanding adaptive responses in PrEP service delivery in Belgian HIV clinics: a multiple case study using an implementation science framework.

INTRODUCTION: In Belgium, oral HIV pre-exposure prophylaxis (PrEP) is primarily provided in specialized clinical settings. Optimal implementation of PrEP services can help to substantially reduce HIV transmission. However, insights into implementation processes, and their complex interactions with local context, are limited. This study examined factors that influence providers' adaptive responses in the implementation of PrEP services in Belgian HIV clinics. METHODS: We conducted a qualitative multiple case study on PrEP care implementation in eight HIV clinics. Thirty-six semi-structured int

Colliding Epidemics: Research Gaps and Implementation Science Opportunities for Tobacco Use and HIV/AIDS in Low- and Middle-Income Countries.

INTRODUCTION: Tobacco use is a leading cause of cancer death among people living with HIV (PLWH) worldwide, and smoking prevalence tends to be higher among PLWH. The burden of both HIV/AIDS and tobacco use is increasingly concentrated in low- and middle-income countries (LMICs), where resources to address these challenges are often limited. However, there has been limited effort to date to integrate tobacco cessation into HIV programs in LMICs. METHODS: We searched the literature (searching was conducted between October 1 and December 31, 2020) using PubMed including search terms "tobacco" an
