The Medicalization of Poverty in the Lives of Low-Income Black Mothers and Children.

Scholars are beginning to use the concept medicalization of poverty to theorize how the United States spends large amounts of money on illnesses related to poverty but invests much less in preventing these illnesses and the conditions that create them (e.g., economic insecurity, housing instability, continuous exposure to violence, and racism). This study examines the connection between poverty, disease burden and health-related costs through the in-depth interviews of 86 Black mothers living in neighborhoods with high levels of violence on the South Side of Chicago. The rippling costs of pove

[Analysis of equity and budgetary sustainability of the pharmaceutical co-payment system in Spain].

OBJECTIVE: To estimate the impact of a more equitable pharmaceutical co-payment system by eliminating the distinction between active workers and pensioners, using only personal income as an adjustment parameter, defining more detailed income brackets, and introducing protective limits on personal expenditure. METHOD: Data from a random sample of 4,505,483 individuals residing in Spain were used, matching pharmaceutical consumption information from the Ministry of Health with economic data from the Tax Agency. Five microsimulation scenarios were designed, modifying co-payment percentages and m

Socioeconomic Status and Choice of Primary Management of Stress Urinary Incontinence: A Retrospective Cohort Study.

OBJECTIVES: Stress urinary incontinence (SUI) affects 25% of Canadian women. Characterizing socioeconomic barriers is essential for advocacy and patient-centred counselling. Our objective was to estimate the association between socioeconomic status and the primary management strategy among patients presenting for consultation for management of SUI. METHODS: This retrospective cohort study used billing and diagnostic data from 2019 to 2022 to identify all patients with SUI presenting to a tertiary urogynaecology clinic in Nova Scotia, Canada. Socioeconomic status was estimated using after tax

US Taxation of Gambling Winnings and Incentives to Bet.

Sports betting is growing rapidly in the US after its legalization by the Supreme Court in 2018. This paper describes the treatment of gambling winnings and losses in the federal tax code and shows how the system may incentivize some gamblers to substantially increase the scale of their betting in order to have a chance to win. This incentive stems from the fact that gambling losses can only be deducted if taxpayers are filing for itemized deductions, meaning the scale of gambling losses has to be large enough to push a taxpayer's eligible deductions over the standard deduction. This incentive

Evaluating post 2024 election scenarios for the UK based on political party manifestos.

Manifestos provide a vision for political parties to enact if elected to government. While manifestos may not be enacted in full, they provide some of the best information for the public in deciding how to vote. However, manifestos tend to contain both enactable policies (e.g., tax cuts) and outcomes or visions of what these policies may achieve for society (e.g., higher disposal income). These outcomes may be deliberately misleading, inaccurate or only a partial picture of how policies will materialise, as it is unlikely a single policy will influence a single outcome. This work creates a com

A pilot study to evaluate the feasibility of using willingness to pay as a measure of value in cancer supportive care: an assessment of amifostine cytoprotection.

The most commonly used method for pharmacoeconomic studies has been the cost-effectiveness analysis (CEA), where the outcome is expressed as an incremental cost per unit of effectiveness (e.g. quality-adjusted life years). Although CEA is a valuable tool for identifying therapies that are more effective and less expensive, deficiencies develop when a given treatment is both more expensive and more effective. An alternative that has not been investigated in the oncology setting is the willingness-to-pay (WTP) method. In this pilot study, a WTP strategy was utilized to estimate the value that th

Is the affordability of cigarettes associated with the prevalence of smoking and quitting at the regional level in Russia?

INTRODUCTION: Tobacco prices and income determine the affordability of cigarettes. Affordability in turn affects smoking prevalence. Both prices and incomes can vary widely across a large country. The aim of this paper is to find out whether there is an association between affordability of cigarettes and smoking/cessation prevalence across Russian regions. We assess the proportions of people who smoke and those who smoked and quit. METHODS: We use microdata from a repeated survey on the living conditions of the Russian population that is representative at the regional level (2014, 2016, 2018,

Parents' employment and children's wellbeing.

Since modern welfare reform began in the 1980s, we have seen low-income parents leave the welfare rolls and join the workforce in large numbers. At the same time, the Earned Income Tax Credit has offered a monetary incentive for low-income parents to work. Thus, unlike some of the other two-generation mechanisms discussed in this issue of Future of Children, policies that encourage low-income parents to work are both widespread and well-entrenched in the United States. But parents' (and especially mothers') work, writes Carolyn Heinrich, is not unambiguously beneficial for their children. On t

Interventions to bolster benefits take-up: Assessing intensity, framing, and targeting of government outreach.

Behaviorally informed "nudges" are widely used in government outreach but are often seen as too modest to address poverty at scale. In four field experiments over 2 y (n = 542,804 low-income households), we test whether more proactive communication, varying message framing, and more precise targeting can boost take-up of tax-based benefits in California above and beyond traditional light-touch approaches. Our interventions focused on extremely vulnerable households, most with no prior-year earnings, who were at risk of missing out on two crucial benefits: the 2021 expanded Child Tax Credit and

"You Need to Keep It Going, Mind, Body, and Spirit": Older Adults' Perspectives on Aging in Place in Subsidized Housing.

In the U.S., more than one million older adults with low incomes live in apartment buildings subsidized by the Low-Income Housing Tax Credit. Although this population experiences disproportionate rates of nursing home admission, little is known about residents' perspectives on factors that influence their ability to live independently in these settings. Fifty-eight residents aged 62 and older and eight study partners participated in qualitative interviews about their perspectives on living independently in subsidized housing, including barriers and facilitators. We analyzed transcripts using a

The association of safety-net program participation with government perceptions, welfare stigma, and discrimination.

Safety-net programs in the United States offered critical support to counter food insecurity and poverty during the first year of the COVID-19 pandemic. The Supplemental Nutrition Assistance Program (SNAP) and the Earned Income Tax Credit (EITC) are both means-tested programs with significant benefits. Take-up of SNAP and EITC is lower in California than nationwide and reasons for this difference are unclear. We examined associations of participation in SNAP and receipt of the EITC and perceptions of the US government, 2 types of welfare stigma (program stigma and social stigma), and perceived

Institutional assembly under constraints: pathways to universal health coverage in Brazil, Rwanda, and China.

This paper examines how Brazil, Rwanda, and China have pursued universal health coverage (UHC) beyond classical health system models developed in high-income settings. We introduce the concept of institutional assembly under constraints, offering a complementary framework for understanding UHC pathways in developing countries, where no single trajectory universally applies. Brazil's health system ensures free access at the point of care through tax-based public financing, but limited government spending has produced a dualised system of public and private provision. Rwanda extends coverage to

Voter Support for Policies Associated With Child Health as National Campaign Priorities.

IMPORTANCE: Policies that are associated with child health are rarely included in platforms of candidates for national political office. Candidates may underrecognize voter support for such priorities or perceive that such policy issues are not sufficiently divisive to appeal to partisan voters. Key policy questions associated with child health may be considered by the next Congress, including the consistency of Medicaid coverage across states and restoring the recently lapsed refundable child tax credit. OBJECTIVE: To examine voter support for candidates regarding policies that are associate

Policy and market forces delay real estate price declines on the US coast.

Despite increasing risks from sea-level rise (SLR) and storms, US coastal communities continue to attract relatively high-income residents, and coastal property values continue to rise. To understand this seeming paradox and explore policy responses, we develop the Coastal Home Ownership Model (C-HOM) and analyze the long-term evolution of coastal real estate markets. C-HOM incorporates changing physical attributes of the coast, economic values of these attributes, and dynamic risks associated with storms and flooding. Resident owners, renters, and non-resident investors jointly determine coas

Punishing and toxic neighborhood environments independently predict the intergenerational social mobility of black and white children.

We use data on intergenerational social mobility by neighborhood to examine how social and physical environments beyond concentrated poverty predict children's long-term well-being. First, we examine neighborhoods that are harsh on children's development: those characterized by high levels of violence, incarceration, and lead exposure. Second, we examine potential supportive or offsetting mechanisms that promote children's development, such as informal social control, cohesion among neighbors, and organizational participation. Census tract mobility estimates from linked income tax and Census r

Examining the affordable connectivity program and telehealth use: a pilot survey of the affordable connectivity program, telehealth, video and audio visits in a racially diverse, lower-income population.

The Affordable Connectivity Program (ACP) aimed to narrow the digital divide by providing discounted internet services for millions of low-income households during the COVID-19 pandemic. This study examined associations between enrollment in the ACP and Telehealth visits in a racially diverse low-income population. Data were obtained via a cross-sectional survey of 213 respondents. Three multivariable regression models examined associations between ACP and three dependent variables, separately: (i) Used telehealth in the past 12 months, (ii) Had 1 or more video visits/consults in the past 12 m

Cross-country analysis of aggregate markups and their impact on income inequality.

BACKGROUND: This study investigates the connection between aggregate markups and income inequality, exploring how market power affects inequality in developed, emerging, and developing economies. METHOD: The study examines data from 12 countries from 1997 to 2016 using a panel data approach. A two-way fixed-effects specification is estimated after panel diagnostics for cross-sectional dependence, model selection, unit roots and cointegration. The analysis focuses on the Gini index, lagged markups, trade openness, GDP per capita, tax revenue and R&D expenditure. RESULTS: Higher lagged markups

A warmer policy for a colder climate: Can China both reduce poverty and cap carbon emissions?

Reducing global carbon dioxide (CO2) emissions is often thought to be at odds with economic growth and poverty reduction. Using an integrated assessment modeling approach, we find that China can cap CO2 emissions at 2015 level while sustaining economic growth and reducing the urban-rural income gap by a third by 2030. As a result, the Chinese economy becomes less dependent on exports and investments, as household consumption emerges as a driver behind economic growth, in line with current policy priorities. The resulting accumulated greenhouse gas emissions reduction 2016-2030 is about 60billi

Association of social determinants of health with self-rated health among Australian gay and bisexual men living with HIV.

Despite a vast improvement in the survival of people living with HIV (PLHIV) since the introduction of combination antiretroviral treatment (cART), little change in the self-rated health of PLHIV has been observed since the introduction of cART in Australia. Difficulties with attaining employment or achieving financial security have been noted as some of the key challenges still facing PLHIV in the post-cART era. As a result, we investigated the independent association of a number of key social determinants of health with self-rated health among HIV-positive gay and bisexual men in Australia.

Income after cancer across gender and age among Canadian adolescents and young adults.

BACKGROUND: Cancer in adolescents and young adults emerges during critical transitional phases, resulting in lasting effects on financial well-being. It remains uncertain whether cancer in adolescents and young adults exhibits differences in financial impact on income based on gender and diagnosis age over time. METHODS: We linked Canada's national cancer registry to personal tax records to identify adolescents and young adults (aged 15-39 years) diagnosed between 1994 and 2013. In the year before diagnosis, survivors were variable-ratio matched to 10 cancer-free individuals on several sociod
