We present an interesting but intriguing case of an indolent palatal swelling. The lesion was asymptomatic causing little discomfort to the patient and thus was an incidental clinical finding. Provisional diagnosis was a benign, minor salivary gland tumor. Clinical differential diagnoses included benign lymphoepithelial lesion or mucus extravasation phenomenon. Ehrlich's reagent (p-dimethylaminobenzaldehyde [DMAB, 1] in 95% EtOH with HCl as catalyst) was employed in spot tests of indoles, providing a diagnosis of, for example, liver diseases, hemolytic processes, occlusion of the common bile duct, and carcinoid syndrome. Although the reagent has been widely used for more than a century, it is not clear how many indole molecules react with a DMAB molecule and whether the reaction takes place at the α- or β-position of the indole molecule. Research here shows that the reaction of DMAB (1) with indole (2) in a 1:2 ratio gives β-bis(indolyl)methane (3).

Systemic mastocytosis is a clonal disorder of the mast cell and its progenitor cell. It is a rare disorder with unknown incidence in Greece, with an estimate of 2 cases per year in Great Britain. We present a case of an asymptomatic, 72-year-old man who was found to have ileocolitis on endoscopy. Histology revealed mast cells in lamina propria >15 HPF and biochemistry showed high levels of serum total tryptase. Molecular testing was positive for the mutation Asp816Val in exon 17 of c-kit gene. The patient met one major and two minor criteria for the diagnosis of systemic indolent mastocytosis

Recessive mutations in the TK2 gene typically cause fatal infantile mitochondrial DNA (mtDNA) depletion syndromes (MDS).(1-3) However, the progression of weakness may vary,(4) as shown by recently described adult patients with late-onset myopathy.(5,6) To date, only 5 adult patients with TK2-related MDS have been reported. Herein, we describe a man who had several unusual features. Clinically, he was weak as a child but sought medical attention as an adult. At the molecular level, multiple mtDNA deletions in muscle were more prominent than mtDNA depletion.

PURPOSE: We present a case series of four patients with unilateral, nonprogressive, yellow or white choroidal lesions of unknown etiology.
METHODS: Four healthy patients were referred to an academic medical retina practice for unusual fundus findings in one eye only. Both eyes of all four patients underwent clinical examination and retinal imaging, including fluorescein angiography, indocyanine green imaging, and optical coherence tomography. The outcome of this series was based on the clinical course of these patients and the features of the retinal images.

The lymphomas of small B lymphocytes are a biologically diverse group of B cell derived neoplasms that includes B cell small lymphocytic lymphoma/chronic lymphocytic leukemia; mantle cell lymphoma; follicular lymphoma; nodal, splenic and extranodal marginal zone lymphomas; and lymphoplasmacytic lymphoma. They are distinguished from one another on clinical, morphological, phenotypic and genetic grounds. This article reviews the essential diagnostic and biologic features of these clinically indolent B cell malignancies.

Before the advent of rationally designed targeted antineoplastic therapies, cladribine was identified as a lymphocyte-specific cytotoxic agent. Intravascular lymphomatosis (IVL) is a rare malignancy characterized by neoplastic proliferation of lymphoid cells within the lumens of arteries, small veins and capillaries. Carcinoid tumors of the colon are a rare cause of colonic malignant disease. A case of carcinoid tumor of the sigmoid colon is presented that illustrates the indolent course of this type of tumor. The case presented highlights the clinicopathologic features of carcinoid tumors of hindgut origin, including advanced local and widely metastatic disease at the time of diagnosis in the absence of symptoms of the carcinoid syndrome. The diagnosis and management of gastrointestinal carcinoid tumors are reviewed.
