[Good and cost-effective palliative care in Westphalia-Lippe - but why? Results of a mixed-methods study].

BACKGROUND: The organization of palliative care in Germany varies from region to region. Previous studies show that palliative care in Westphalia-Lippe (WL) - measured by quality and cost indicators - performs better than in other regions. The Westphalian model for outpatient care emphasizes a close cooperation between general practitioners (GPs) and palliative care consultation services (PCS). The basis is a contractual regulation of general and specialized care within a joint outpatient palliative care contract according to Sect. 140a SGB V. The VESPAL study (on the Quality of care in outpat

Clinical Efficacy of Telemedicine for Musculoskeletal Conditions in a Medicare Advantage Population.

INTRODUCTION: Musculoskeletal (MSK) conditions are highly prevalent among the Medicare population, and telerehabilitation has been shown to be equivalent to traditional in-person physical therapy for many patients. METHOD: We studied the clinical outcomes of 100 consecutive Medicare Advantage patients from one insurer treated with a virtual physician-led MSK care team model. Patients had to have completed at least three clinical video visits to be eligible for participation. We also recorded rates of image ordering and referrals for in-person services. RESULTS: The average age of our patient

Views and perspectives toward implementing the Global Spine Care Initiative (GSCI) model of care, and related spine care program by the people in Cross Lake, Northern Manitoba, Canada: a qualitative study using the Theoretical Domain Framework (TDF).

BACKGROUND: Back pain is very common and a leading cause of disability worldwide. Due to health care system inequalities, Indigenous communities have a disproportionately higher prevalence of injury and acute and chronic diseases compared to the general Canadian population. Indigenous communities, particularly in northern Canada, have limited access to evidence-based spine care. Strategies established in collaboration with Indigenous peoples are needed to address unmet healthcare needs, including spine care (chiropractic and movement program) services. This study aimed to understand the views

How Can Framed Mammography Screening Messages Be Optimally Persuasive for Foreign-Born Chinese American Women?

BACKGROUND: Foreign-born Chinese American women (FBCAW) have the lowest mammography rates compared with other racial groups despite the overwhelming evidence of the benefits of screening. Message framing based on the prospect theory has shown significant but inconsistent effects on mammogram screening among ethnic minority groups. OBJECTIVE: Using data from a randomized controlled trial, this secondary analysis aims to identify factors that interact with message framing to improve mammography screening in FBCAW. METHODS: In the parent study, participants were randomized to receive either a g

Is It Fair That Patient-Reported Outcome Measures Completion Is Tied to Reimbursement? Patient Demographics Are Associated With Rates of PROM Completion and Potential Health Disparities.

BACKGROUND: Patient-reported outcome measures (PROM) allow clinicians to measure functional outcomes following surgery. The Centers for Medicare & Medicaid Services (CMS) has made reporting of PROMs mandatory for inpatient total joint arthroplasty (TJA) reimbursement. Despite these mandates, there is lack of research identifying patient factors associated with PROM noncompliance. The goal of this study was to identify significant predictors of which patients are less likely to complete PROMs. METHODS: This retrospective study included patients who underwent primary TJA between January 1, 2022

The Shifting Policy Landscape in Reproductive Health.

Since the U.S. Supreme Court's decision in Dobbs v. Jackson Women's Health Organization and accelerating over the past year, reproductive health policy in the United States has become increasingly fragmented and unstable. Recent federal actions have introduced new barriers across the continuum of reproductive care. These actions include changes to Title X family planning funding, substantial reductions to Medicaid, rescission of federal Emergency Medical Treatment and Labor Act (EMTALA) guidance related to pregnancy emergencies, and renewed regulatory scrutiny of mifepristone. Together, these

Slovakia: Health System Review.

This analysis of the Slovak health system reviews developments in governance, organization, financing and delivery of care, health reforms and health system performance. Slovakia, a central European country with a population of 5.4 million, continues to face significant health and health care system challenges. Slovakia's health system is founded on universal coverage with compulsory health insurance, a broad benefits package and a competitive insurance model. Although life expectancy improved between 2000 and 2019, the COVID-19 pandemic reversed gains, and in 2023 Slovak life expectancy remai

Acute neurological care in north-east Germany with telemedicine support (ANNOTeM): protocol of a multi-center, controlled, open-label, two-arm intervention study.

BACKGROUND: Both diagnosis and treatment of neurological emergencies require neurological expertise and are time-sensitive. The lack of fast neurological expertise in regions with underserved infrastructure poses a major barrier for state-of-the-art care of patients with acute neurological diseases and leads to disparity in provision of health care. The main purpose of ANNOTeM (acute neurological care in North East Germany with telemedicine support) is to establish effective and sustainable support structures for evidence based treatments for stroke and other neurological emergencies and to im
