Psychotherapy for depression in claimants receiving wage replacement benefits: review of the evidence.

PURPOSE: To review the evidence on the provision of psychotherapy for claimants who are suffering from depression and receiving wage replacement benefits. METHODS: A literature review was performed using PubMed and EMBASE. RESULTS: Results from three studies are discussed. The first is a systematic review and individual patient data meta-analysis of randomized controlled trials to assess the relative effectiveness of cognitive behavioral therapy (CBT) for depression in patients receiving disability benefits. A non-significant trend showed that the effect of CBT was greater in patients receiv

Modeling the contribution of speeding and impaired driving to insurance claim counts and costs when contributing factors are unknown.

PROBLEM: There are no specific indicators for distinguishing insurance claims related to speeding and impaired driving in the information warehouse at the Insurance Corporation of British Columbia. Contributing factors are only recorded for that part of the claim data that is also reported by the police. Most published statistics on crashes that are related to alcohol or speeding are based on police-reported data, but this represents only a fraction of all incidents. METHOD: This paper proposes surrogate models to estimate the counts and the average costs associated with speeding and impaired

[The economic evaluation of group examination for gastrointestinal disease in an electronics corporation--the cost-benefit analysis of follow-up study for peptic ulcer].

To investigate the usefulness of gastrointestinal fiberscope examination (GIF) for peptic ulcer in the health care station (station), a cost-benefit analysis was performed from the standpoint of the company, the health insurance society (insurance society), and workers. Before the analysis, the clinical course of the peptic ulcer, direct cost of the examination, and indirect cost due to time taken for the examination at the station were compared with those in a private hospital (hospital). In 1994, 202 cases of gastric ulcer (GU) (194 male cases, 8 female cases) and 199 cases of duodenal ulcer

What happens when you wait? Effects of Social Security Disability Insurance wait time on health and financial well-being.

While waiting for a decision, individuals who apply for Social Security Disability Insurance face strong incentives to remain out of the labor force and receive no support from the program. An extended period with low income, high uncertainty, and lowered access to health insurance has the potential to harm applicants' well-being along many dimensions. In this study, I estimate the effect of initial wait time on health, healthcare access, and financial well-being, using the prevailing wait at the time an applicant applied as an instrument for their own initial wait. I find that a longer wait d

["With all suitable means". Off-label-use and public statutory employers' liability insurance].

In Germany, responsibility for treatment of occupational diseases lies with the public statutory employers' liability insurances (SELI). According to their legal obligation SELI have to ascertain cure--wherever possible--"with all suitable means". Thus, dermatologists treating patients with occupational dermatoses are required to deliver the best possible therapy, which, according to the current scientific knowledge, may in some cases be off-label. For example, in occupational contact dermatitis a number of scientifically promising topical and systemic drugs are not yet licensed for this indic

Brazilian physicians hold national strike against medical insurance companies.

The Brazilian Health System has two different forms of access, public and private. The purpose of the public health system is to provide universal, complete access, free of charge, for the entire population. The private sector is composed by people who have private insurance. Nowadays, about 43 million Brazilians, or 26.3% of the population, have private health insurance. The main motivations of the physicians for the strike were the low payment for medical services and the constant interference of the private health insurance companies in medical autonomy. For this reason, Brazilian physician

Disease subject to presumptive service connection (radiation risk activity)--VA. Final rule.

This document amends Department of Veterans Affairs (VA) adjudication regulations concerning diseases presumed to be the result of exposure to ionizing radiation. This amendment is necessary to implement Public Law 103-446, the Veterans' Benefits Improvements Act, which provides that the term "radiation risk activity" includes the onsite participation in a test involving the atmospheric detonation of a nuclear device by the United States and by other governments. The intended effect of this amendment is to extend the presumption of service connection for radiogenic disabilities to those vetera

Smoking cessation: the value of a comprehensive carved-in benefit.

This qualitative narrative review examines the potential returns from providing smoking cessation treatments (SCTs) through an insurance plan's standard benefit package versus through an optional supplementary wellness ('rider') program. Research indicates most employers offer SCTs as part of a rider available for purchase. Studies demonstrate that the higher the cost of SCTs, the lower the SCT participation rates; when employees receive SCTs, smoking cessation rates increase, effecting lower employee healthcare costs and improved productivity. Employers may receive a considerable return on th

Future of supplementary private hospitalization insurance in Switzerland: a policy brief.

BACKGROUND: Voluntary hospitalization insurance (VHI) in Switzerland has lost part of its traditional role as mandatory health insurance has expanded, hospital financing reforms have increased public co-financing of inpatient care, and patterns of hospitalization have changed. Nevertheless, VHI remains strategically important for hospitals, insurers, and some providers. ANALYSIS: This policy brief draws on a structured review of academic and grey literature, regulatory and policy documents, market information, and ten semi-structured interviews with key stakeholders. The analysis identifies f

Remodelling reparation: changes in the compensation of victims of natural catastrophes in Belgium and the Netherlands.

The paper presents a comparative analysis of the development and present state of compensation for victims of catastrophes in Belgium and the Netherlands. These two neighbouring countries have both seen legislative changes in this field in recent years, albeit with different outcomes. The paper thus analyses to what extent the two compensation scheme structures allow for conclusions as to the comparative benefits of a comprehensive insurance scheme for natural disasters. From the perspective of law and economics, the evolution of private insurance and public intervention through compensation f

After CLASS--Is a voluntary public insurance program a realistic way to meet the long-term support and service needs of adults with disabilities?

BACKGROUND: The CLASS Act, which was part of the Affordable Care Act of 2010, established a voluntary personal assistance services (PAS) insurance program. However, concerns about enrollment and adverse selection led to repeal of the CLASS Act in 2013. OBJECTIVES: To estimate the number of middle-aged adults interested in purchasing PAS insurance, the sociodemographic, socioeconomic and disability attributes of this population, and the maximum monthly premium they would be willing to pay for such coverage. METHODS: A total of 13,384 adults aged 40-65 answered questions about their interest i

Mental illness as a risk factor for uninsurance among mothers of infants.

The objective of this study was to assess the extent to which maternal prenatal mental illness is associated with mothers' health insurance status 12-18 months after giving birth. The sample consisted of 2,956 urban, mostly unwed, mothers who gave birth in 20 large U.S. cities between 1998 and 2000 and participated in the Fragile Families and Child Wellbeing birth cohort study. Multinomial logistic regression models were used to assess associations between maternal prenatal mental illness and whether the mother had private, public, or no insurance one year after the birth. Covariates included

Health for all? A cost-utility evaluation of Colombia's policy to enroll Venezuelan migrants (2021-2023).

INTRODUCTION: From 2021 to 2023, Colombia implemented a national policy to expand health insurance coverage for over 1.4 million Venezuelan migrants through its subsidized health insurance system, which provides universal health coverage to low-income Colombian populations. This study evaluates the cost-utility of the intervention, focusing on its health and economic returns. METHODS: We conducted a cost-utility analysis by translating government expenditure per enrolled migrant into health outcomes (QALYs/YLLs) using Colombia-specific cost-effectiveness thresholds. Health benefits were measu

Coverage for Gender-Affirming Care: Making Health Insurance Work for Transgender Americans.

Many transgender Americans continue to remain uninsured or are underinsured because of payers' refusal to cover medically necessary, gender-affirming healthcare services-such as hormone therapy, mental health counseling, and reconstructive surgeries. Coverage refusal results in higher costs and poor health outcomes among transgender people who cannot access gender-affirming care. Research into the value of health insurance coverage for gender-affirming care for transgender individuals shows that the health benefits far outweigh the costs of insuring transition procedures. Although the Affordab

Benefit Type and Care Source in Relation to Mammography Screening and Breast Cancer Stage at Diagnosis Among DoD Beneficiaries.

BACKGROUND: Type of insurance and out-of-pocket costs influence the use of medical care. Specifically, type of insurance can impact an individual's likelihood of receiving a screening mammogram. Additionally, variation in tumor stage at diagnosis exists for patients with different types of insurance. It is not clear whether different benefit types and care sources differ in breast cancer care and outcomes among Department of Defense (DoD) beneficiaries. METHODS: The objective of this study was to examine differences in screening mammography and tumor stage at diagnosis between different benef

Primary care physician supply, insurance type, and late-stage cancer diagnosis.

BACKGROUND: Understanding the joint effects of insurance type and primary care physician density on stage at diagnosis is essential to elucidating the healthcare access and late-stage cancer relationship. PURPOSE: To determine if the relationship between primary care physician density and odds of late-stage cancer are modified by insurance type at diagnosis. METHODS: Case patients were Ohio adults diagnosed between 1996 and 2008 with cancer of one of the following sites: female breast, cervix, colon/rectum, lung/bronchus, melanoma of the skin, oral cavity and pharynx, or prostate (N=376,425)

Trends in employer-sponsored health insurance coverage for tobacco-dependence treatments.

BACKGROUND: Nearly 1.8 million smokers in California receive their health insurance benefits through their employer. The extent to which these workers have coverage for tobacco-dependence treatments (TDTs) through their employer-sponsored health care is unknown. METHODS: This research used the 2000 and 2005 data from the California Employer Health Benefits Surveys to determine coverage for TDTs by private firms. The overall response rates of firms to the survey were 41% and 36%, respectively. The samples used in this analysis are limited to private firms in California that offered employee he

The workers' compensation system: worker friend or foe?

BACKGROUND: The workers' compensation system was designed to help injured workers who have substantial medical expenses and perhaps have lost a great deal of income. This study determines both similarities and differences in how workers experience their interactions with the workers' compensation systems in Florida and Wisconsin. METHODS: Ethnographic open-ended interviews with 204 workers from Florida and 198 workers in Wisconsin were conducted. All the workers had back injuries in 1990 and were either paid workers' compensation temporary disability benefits for at least 4 weeks or received

Use and Outcomes of Cost-Effectiveness Analyses and Additional Considerations in Pharmaceutical Reimbursement in the Netherlands.

BACKGROUND AND OBJECTIVE: In the Netherlands, the National Health Care Institute (ZIN) assesses pharmaceuticals and advises the Minister of Health, Welfare, and Sport (MoH) on their reimbursement. Since 2015, two policy reforms-the introduction of willingness-to-pay reference values and the lock procedure, which temporarily withholds high-budget hospital pharmaceuticals from reimbursement pending assessment and price negotiations-have changed the decision-making process and expanded the role of cost-effectiveness analysis (CEA). This study provides a comprehensive overview of the use and outco
