Dissecting aneurysms of the intracranial vertebral artery.

Among 86 patients with aneurysms arising from the vertebral artery or its branches, 24 had dissecting aneurysms. The patients with dissecting aneurysms were characteristically relatively young males. Twenty-one patients presented with subarachnoid hemorrhage (SAH) and three with ischemia. Severe headache or neck pain occurred in all three patients with ischemia. Five of the 21 patients with SAH and all three patients with ischemia experienced recurrent episodes. Angiography typically showed fusiform dilatation and proximal and/or distal narrowing of the affected artery. The difficulty of diagn

[Natural history of giant intracranial aneurysms].

The outcome of a consecutive series of 28 patients with giant aneurysm who had been followed without surgery from one month to 12 years after the diagnosis was made, are presented with reviewing their radiological and clinical features. Symptoms and signs were directly or indirectly attributable to the intracranial mass effect and nine patients (32%) presented subarachnoid hemorrhage. Subarachnoid hemorrhage was frequently associated with intraventricular or intracerebral hemorrhage, a poor clinical grading at admission and a high mortality. Subarachnoid hemorrhage was most often recorded from

Surgical treatment of incidental intracranial aneurysms.

It is clear that more incidental aneurysms will be encountered in the future. Approximately 5% or more of the population harbors these lesions, and advancing technology can be expected to demonstrate them with increasing regularity. Multiple aneurysms will also be found in at least 18% of patients with subarachnoid hemorrhage due to aneurysms. The best estimates suggest a rate of hemorrhage approximating 1%/year for incidental aneurysms and a 0.4 to 0.65% annual mortality rate for these lesions. It has also been shown that even small aneurysms may enlarge and bleed unpredictably with the passa

[Ischemic complications of ruptured intracranial aneurysms].

Eighty patients are reported with ischaemic complications developing during subarachnoideal haemorrhage due to aneurysm rupture. Ischaemic complications developed in 12.82% of cases in the whole group of subarachnoideal haemorrhages caused by aneurysm rupture. The causes of ischaemia were: closure or stenosis of the afferent artery during the operation, arterial spasm, thrombosis at the site of the aneurysm and brain oedema. Ischaemia during intracranial aneurysm rupture caused in most cases pale infarcts. Ischaemic complications of aneurysm rupture had a serious prognosis, nearly half the pat

Cerebral emboli from intracranial aneurysms.

Cerebral vasospasm has been implicated as the major pathophysiologic mechanism responsible for the cerebral ischemia and infarction that sometimes follows the rupture of intracranial aneurysms. Two cases are reported in which aneurysms of the internal carotid arteries were the source of emboli resulting in infarctions. Embolic phenomena should be considered as a possible cause of the morbidity associated with intracranial aneurysms.

New approach to management of intracranial aneurysms.

In six cases an attempt was made to relieve the tension on intracranial aneurysms by temporarily clamping the internal carotid artery in the neck, so as to increase the expansibility of the artery. This approach was based on the concept (or "A principle") that haemorrhage is caused by the aneurysm having to bear the full force of systolic pulse pressure when atherosclerosis prevents this pressure being taken up by the normally expansile arterial wall. Follow-up has been fairly short, but the preliminary findings in four of the six patients are encouraging. More attention must be paid in the fu

Traumatic Falcine Artery Aneurysm Resulting in Intracranial Hemorrhage.

A 50-year-old man presented with mild unconsciousness after a fall-induced head injury. Initial imaging revealed a left-sided acute subdural hematoma. After transportation to our hospital, his condition deteriorated, leading to the discovery of a new hemorrhage and an anterior falcine artery aneurysm upon further examination. The patient underwent successful decompressive craniectomy and endovascular occlusion. This case, the first reported of a traumatic anterior falcine artery aneurysm, suggests the initial injury caused both the hematoma and aneurysm. The aneurysm's specific location near t

Primary central nervous system vasculitis with intracranial aneurysm.

BACKGROUND: Unruptured intracranial aneurysms (UIAs) are rarely reported in primary central nervous system vasculitis (PCNSV). In this study we described the clinical findings, response to therapy, and outcomes of UIA in a large cohort of PCNSV patients. METHODS: We retrospectively studied 216 consecutive patients with PCNSV, selected by predetermined diagnostic criteria, who were seen during a 40-year period. UIAs were identified on cerebral angiography. The clinical, laboratory, radiologic and pathologic findings, management, and outcomes of patients with UIA were described and compared wit

Intracranial aneurysm distribution and characteristics according to gender.

OBJECTIVE: Female sex is associated with a greater prevalence of cerebral aneurysms. However, it also might be linked to the location and size of the aneurysm. The aim of this study was to find the link between female sex and aneurysm characteristics. METHODS: Our study group consisted of 357 patients admitted to the hospital with both unruptured and ruptured aneurysms confirmed by Digital Subtraction Angiography or Computed Tomography Angiography and aneurysmal subarachnoid haemorrhage confirmed by head CT. Patients' medical records were analyzed for necessary information including, but not

Iatrogenic traumatic intracranial aneurysm after endoscopic sinus surgery.

Iatrogenic traumatic intracranial aneurysms are rare, but their clinical impact is significant secondary to their risk of intracranial hemorrhage and in their frequent complexity in management. We report an adult patient with a history of chronic sinusitis who, while undergoing elective endoscopic polypectomy, suffered an iatrogenic injury to an A2 segment branch of the left anterior cerebral artery, resulting in a pseudoaneurysm. Management included endovascular coiling and a bicoronal craniotomy approach, using a split-thickness cranial graft and abdominal fat graft to repair the associated

Visual field findings after a ruptured intracranial aneurysm.

BACKGROUND: Visual field defects (VFDs) negatively affect activities of daily living and rehabilitation following aneurysmal subarachnoid haemorrhage (aSAH). The aim here was to assess VFDs in patients with aSAH and their associations with age, gender, aSAH severity, and clinical outcome. METHODS: Patients admitted to Helsinki University Central Hospital and treated during 2011 were participants in this prospective study. Findings obtained with the Octopus 900 perimeter (Haag-Streit Inc, Koenic, Switzerland), the Goldmann perimeter (Haag-Streit Inc, Bern, Switzerland), or the confrontation vi

Death associated with an unruptured giant intracranial aneurysm.

Deaths due to or associated with unruptured giant intracranial aneurysms are extremely rare. We describe a case of a sudden death associated with an unruptured giant vertebrobasilar aneurysm in a symptomatic 18-year-old male. Post-mortem examination revealed three giant fusiform and thrombosed aneurysms (vertebrobasilar junction, internal carotid arteries), an old infarct with some sub-acute changes in the brain stem and medulla, an incipient pneumonia, and a high blood alcohol concentration (2.4 o/oo). The association of these findings suggest the role of a multi-factorial respiratory dysfunc

Risk of aneurysm rupture at intracranial arterial bifurcations.

BACKGROUND: Aneurysms on the posterior circulation, most commonly located at the basilar top, have a higher risk of rupture than aneurysms on the anterior circulation. If hemodynamic shear stress, which has its maximum impact at the distal carina of bifurcations, explains the higher rupture rate of basilar top aneurysms, aneurysms at the top of the carotid artery should have similar rupture rates given their geometrical similarities. AIM: Our purpose was to compare rupture risks of carotid and basilar artery bifurcation aneurysms. METHODS: We included studies from Medline and Embase searches

Spontaneous thrombosis of a recurrent clipped intracranial aneurysm.

Spontaneous thrombosis of an intracranial aneurysm is a rare event. It is predominantly observed with aneurysms that are large and have relatively narrow necks. We report here a case of a 48-year-old woman presenting with subarachnoid hemorrhage (SAH) due to rupture of a 2-mm aneurysm of the anterior cerebral artery treated by microsurgical clipping. Six months after treatment of the aneurysm, the patient presented with severe headache. SAH was excluded, but computed tomographic angiography (CTA) revealed the recurrence of a large aneurysm (7 mm) that was confirmed by cerebral angiography (DSA

Parry-Romberg syndrome with migraine and intracranial aneurysm.

Parry-Romberg syndrome or progressive facial hemiatrophy (PFH) is a rare disease of unknown etiology characterized by atrophy of the skin and subcutaneous tissue on one side of the face. The authors present the case of a 32-year-old woman with PFH who had migraine and an intracranial aneurysm. The findings support the hypothesis that the disease could be related to a neural crest migration disorder, from which both fronto-nasal mass and cranial vessels take origin.

Intracranial mycotic aneurysm caused by Aspergillus--case report.

A 75-year-old female with chronic renal failure diagnosed as Wegener's granulomatosis was receiving steroids and immunosuppressive agents when subarachnoid hemorrhage developed. Cerebral angiography showed a fusiform aneurysm arising from an angular branch of the left middle cerebral artery. Hemorrhage occurred and the aneurysm was excised by emergency surgery. Microscopic examination of the aneurysm revealed dense infiltration of hyphae identified as Aspergillus. She died of subsequent hemorrhage. Autopsy showed numerous Aspergillus hyphae in the lung. Fungal mycotic aneurysm should be consid

Infected intracranial aneurysm in an infant: case report.

The case of a ruptured, infected intracranial aneurysm occurring in a 34-day-old child is reported. The child was brought for examination after a sudden onset of screaming and pallor, followed by focal seizures. Blood cultures grew Staphylococcus aureus, and a computed tomographic scan revealed a large hematoma in the region of the left sylvian fissure. An angiogram demonstrated a 17-mm aneurysm arising from a small branch of the middle cerebral artery. At craniotomy, the hematoma was evacuated and an infected aneurysm was removed. The etiology, pathogenesis, and management of infected intracr

[Aneurysm surgery in relation to secondary intracranial findings].

In 116 patients operated on because of aneurysms we found 125 aneurysms, one convexity meningioma, and one A. V. supratentorial angioma. In 9 patients (8%) more than one aneurysm were found. In 8 patients there were double aneurysms and on one case 4 aneurysms, 18 of these 20 aneurysms were clipped. If these were situated on the same side they were treated simultaneously. If they were situated on two sides, the aneurysms were clipped in two surgical interventions. In 2 patients one of the two aneurysms remained untreated. Two interventions were also carried out for the combination aneurysm/tum
