[What's new … Patients are listening-communication under general anesthesia].

There is ample evidence of awareness of at least some unconscious patients. A recent multicenter study found significant reductions after therapeutic communication during general anesthesia in postoperative pain and analgesic consumption, as well as in postoperative nausea and vomiting (PONV) and antiemetic requirements in high-risk patients. Thus, an intraoperatively presentet text represents a simple non-pharmacological method to reduce side effects of surgery and anesthesia. This also offers treatment in other unconscious patients. However, another finding seems worth noting: the results of

Pressure alopecia following robotic gynaecological surgery: Lessons from a case series on a preventable complication.

Pressure alopecia is an under-recognised but preventable post-operative complication associated with surgeries. With the increasing use of robotic gynaecological procedures, patients are often subjected to steep Trendelenburg position predisposing them to scalp pressure and scalp oedema which leads to local ischaemia. We present a case series of patients who developed post-operative pressure alopecia following robotic gynaecological surgery. All cases were characterised by delayed onset, non-scarring alopecia over pressure-bearing areas of the scalp, with spontaneous regrowth over time. Althou

Decoding follicular cholangitis: A benign entity with a malignant façade.

Follicular cholangitis (FC) is a rare, benign inflammatory biliary disorder that can closely mimic malignancy. We present the case of a 34-year-old lady with an incidentally detected distal common bile duct (CBD) lesion. Preoperative imaging was suggestive of periampullary malignancy. However, serology and biopsy findings were inconclusive. Intraoperative frozen section analysis revealed cholangitis with prominent lymphoid aggregates, ruling out malignancy. These critical intraoperative histopathological findings guided toward a conservative surgical approach in the form of antegrade CBD excis

Allergy to Ringer's Lactate - Uncommon Cause of Anaphylaxis during Intraoperative Period: A Case Report.

Perioperative anaphylaxis is a rare but potentially fatal event, with an incidence ranging from 1:300 to 1:20,000 surgeries and a mortality rate of 3-9%. Its intraoperative occurrence is diagnostically challenging due to patient sedation and overlapping drug effects. Crystalloids like Ringer's Lactate are rarely implicated. A 27-year-old American Society of Anesthesiologists grade I female undergoing mastoidectomy developed intraoperative hypotension, tachycardia, and generalized erythema after induction with standard anesthetics and Ringer's Lactate infusion. Anaphylaxis was suspected, and Ri

Intraoperative management of suspected ocular venous air embolism (OVAE) during vitrectomy for retinal detachment.

OBJECTIVE: report a rare case of ocular venous air embolism (OVAE) occurring during pars plana vitrectomy for retinal detachment and to highlight key intraoperative management steps that led to a successful outcome. OBSERVATIONS: A 57-year-old patient undergoing 23-gauge vitrectomy developed abrupt hemodynamic instability during air-fluid exchange. Intraoperative signs-including sudden drops in end-tidal CO2 and oxygen saturation-prompted suspicion of air embolism, likely due to suprachoroidal air infusion via a dislodged trocar. Immediate cessation of air infusion, internal tamponade, cardio

Transient cortical blindness following occipital lobe retraction in a pineal region meningioma resection.

A woman in her 50s experiences complete cortical blindness following a successful resection of a pineal region meningioma, most likely secondary to intraoperative bilateral occipital lobe retraction. The post-operative MRI did not demonstrate any ischaemia. The index presentation was worsening recurrent falls and cognitive decline from a large mass in the pineal recess resulting in triventriculomegaly, managed successfully with an endoscopic third ventriculostomy 7 months prior. The enlarging mass was resected via a parieto-occipital interhemispheric approach with a 7 hour operative time and n

Atypical primary hyperparathyroidism due to parathyroid lipoadenoma: a case report.

Parathyroid lipoadenoma is a very rare cause of primary hyperparathyroidism. Preoperative imaging techniques often fail to detect such lesions, and even during surgery they can be misinterpreted just as fat tissue. A 62-year-old woman clinically monitored for primary hyperparathyroidism, with hypertension and a left nephrectomy for hydrouretheronephrosis caused by recurrent kidney stones. A neck ultrasound showed a nodule consistent with left parathyroid of 9 × 5 mm, which was not confirmed on single-photon-emission computed tomography/computed tomography (CT) scan. On surgery, a voluminous le

Nerve injuries in cervical spine surgery via anterior approach: a comprehensive review.

Anterior cervical spine surgery is widely performed to manage degenerative, traumatic, and compressive spinal conditions. Despite its effectiveness, iatrogenic nerve injuries remain a significant concern due to the complex anatomy of the cervical region. This review offers a focused narrative on nerve injuries associated with anterior cervical approaches, including recurrent and superior laryngeal nerve palsy, C5 nerve root palsy, and hypoglossal nerve injury. Less common complications such as Parsonage-Turner syndrome (PTS), C8-T1 radiculopathy, and Horner's syndrome are also addressed. Each

Confirmation of Parathyroid Tissue: Are Surgeons Aware of New and Novel Techniques?

BACKGROUND: Ex vivo aspiration of parathyroid glands for the measurement of intraoperative parathyroid hormone (IOPTH) levels is a rapid point-of-care method to confirm parathyroid tissue during parathyroidectomy and an alternative to frozen section (FS). This study sought to determine the awareness and utilization of this technique among endocrine surgeons. MATERIALS AND METHODS: A de-identified 12-question survey regarding techniques for intraoperative identification/confirmation of parathyroid tissue and the use of IOPTH monitoring was distributed to all 608 members of the American Associa

Vulvar Angioleiomyoma.

Vulvar angioleiomyoma is a very rare neoplasm. We describe the case of a 49-year-old woman who presented with a small, firm, tender labial mass. Intraoperatively, the lesion appeared hypervascular and was excised using sharp dissection. Histological and immunohistochemical analyses were necessary to make the diagnosis. The report of this extremely uncommon gynecologic lesion is important to make clinicians aware of the possibility of vulvar angioleiomyoma.

Suspicion of intraoperative left main coronary artery extrinsic compression with Perceval® sutureless bioprosthesis.

We present an uncommon case of intraoperative transient myocardial ischemia related to suspicion of left main stem artery extrinsic compression caused by the radial force of the stent of Perceval® bioprosthesis. No other cases have been described, but some similarities can be found with extrinsic coronary compression in percutaneous pulmonary valves. Surgeons should be aware of this complication in patients with coronary anomalies or with bicuspid anatomy.

Congenital Ventricular Diverticulum or Aneurysm: A Difficult Diagnosis to Make.

A pouch protruding from the free wall of the left ventricle may be either a congenital ventricular diverticulum (CVD) or aneurysm (CVA). Being aware of these rare congenital anomalies is critical in making the diagnosis. Differentiating the two is important for treatment decisions. We describe a patient with dextrocardia, Tetralogy of Fallot, and a congenital left ventricular apical diverticulum diagnosed following the induction of anesthesia. CVD and CVA may present in the antenatal period through late adulthood with differing morphology, location, and symptoms. Echocardiography is paramount

Primary presentation of Jeune's syndrome as gastric motility disorder in an infant: A case report.

We report a case of a 4-week-old female neonate with Jeune's asphyxiating thoracic dystrophy (JATD) and coexistent situs anomaly, primarily presenting as gastric motility disorder. The child presented with abdominal distension and nonbilious vomiting since birth with failure to thrive. However, skeletal survey revealed JATD. Upper gastrointestinal contrast study showed situs inversus with delayed gastric emptying. Pyloric biopsy and intraoperative antro-duodenal manometry confirmed association of gastric motility disorder. Awareness of the unusual possibility of primary presentation of Jeune s

Pediatric tanycytic ependymoma of the cauda equina: case report and review of the literature.

Tanycytic tumors of the cauda equina region are being increasingly reported. Such tumors can occur among pediatric patients and adults. An unusual case of tanycytic ependymoma located at the region of the cauda equina is reported in a girl. The clinical presentation, radiological features, intraoperative findings, and histopathologic features are described. The prognostic significance of differentiating such a neoplasm from other intramedullary tumors is noted. A heightened awareness of this pathologic entity among neurosurgeons and pathologists will help in diagnosing this neoplasm and in ena

Postoperative accumulation of bone in the middle ear: effect on hearing and surgical implications.

Two patients with conductive hearing loss caused by the intraoperative accumulation of bone particles in the middle ear are presented. This complication of otologic surgery may be prevented by an awareness of its possibility and by the temporary blockage of the aditus ad antrum in those operations involving mastoidectomy, even utilizing suction irrigation of the surgical field during the procedure. Once this condition is established, tympanotomy may confirm the presence of bone debris in the middle ear and the material may then be removed, with elimination of the conductive hearing loss.

Major intraoperative complications during video-assisted thoracoscopic anatomical lung resections: an intention-to-treat analysis.

OBJECTIVES: A multicentre evaluation of the frequency and nature of major intraoperative complications during video-assisted thoracoscopic (VATS) anatomical resections. METHODS: Six European centres submitted their series of consecutive anatomical lung resections with the intention to treat by VATS. Conversions to thoracotomy, vascular injuries and major intraoperative complications were studied in relation to surgeons' experience. Major complications included immediate life-threatening complications (i.e. blood loss of more than 2 l), injury to proximal airway or other organs or those leadin

Perforated jejunal duplication cyst in an adult: diagnosis and management.

Enteric duplication cysts are an uncommon congenital anomaly that can occur anywhere along the gastrointestinal tract. These rare abnormalities are predominantly diagnosed in children and seldom reported in adults. We report a case of a male in his 50s who presented with an acute abdomen due to perforation peritonitis. Emergency exploratory laparotomy revealed two communicating jejunal duplication cysts, one of which was perforated. The involved jejunal segment was resected, and a primary end-to-end anastomosis was performed. Ultrasonography, CT and MRI can assist in making a preoperative diag

Agenesis of gallbladder-an unexpected absence in laparoscopy.

Gallbladder agenesis is a very rare condition, majority of which are normally diagnosed intraoperatively because of low index of suspicion and high rate of false positive results for cholecystitis on ultrasound imaging. We present a case of a 25-year-old man who presented with a right upper quadrant pain, whose ultrasound suggests a contracted gallbladder with gallstones. He was treated as an acute on chronic cholecystitis and booked for laparoscopic cholecystectomy. Intraoperatively, there was no gallbladder to be seen and decision was made to abandon procedure and avoid further exploration.
