Enteral feeding complications and nursing management.

Enteral feeding is an essential intervention for patients unable to meet their nutritional needs orally, but it carries risks that require skilled nursing recognition and management. This article provides a practical overview of the most common complications encountered across the range of enteral tube types - including refeeding syndrome, nausea and vomiting, aspiration, diarrhoea and constipation, tube blockage, and tract infection - and outlines the nursing management strategies supported by current evidence and guidance. Prompt identification of complications, accurate documentation, early

[Visual ultrasound guided naso-jejunal tube replacement in situ].

To address the problems of traditional naso-jejunal tube replacement in post-pyloric feeding, such as high blind insertion failure rate, high radiation exposure and high risk of complications, this study proposed a naso-jejunal tube replacement technique with real-time ultrasound guidance and zebra guide wire, and applied it to the clinical practice of a 67-year-old male patient with severe traumatic brain injury. The patient needed long-term post-pyloric enteral nutrition support due to severe traumatic brain injury, but blind tube insertion was difficult (the first tube insertion was assiste

Interpreting Cortrak™ feeding tube position: Safety assessment of manufacturer training guides.

BackgroundExpert Cortrak™ guided-tube placement attains near-zero pneumothorax risk versus 0.52% in 'blind' tube placement. Conversely, in low-use Cortrak centres, insufficient training and misinterpretation results in undetected lung misplacement and pneumothorax in 0.10% versus 0.013% for blind placement.ObjectiveDetermine whether manufacturer training guides enable safe interpretation of tube position.MethodsManufacturer guidance was compared to the evidence-based lung-oesophagus, gastrointestinal flexure system (LOGIFS). Manufacturer text and figures were analysed to determine whether it i

Nasogastric tube placement perforating the nasopharynx causing mediastinal passage and feeding into the pleural space.

A mid-60s female with known primary progressive multiple sclerosis was admitted to our hospital with aspiration pneumonia. Due to reduced consciousness, a nasogastric (NG) tube was inserted for feeding. A chest x-ray (CXR) report from a thoracic radiologist confirmed an atypical tip position due to a hiatus hernia but concluded it was almost certainly within the stomach. However, after deteriorating on feed commencement, a CT scan of the chest confirmed the tube had perforated the nasopharynx and run parallel to the oesophagus through the mediastinum into the right pleural space. The patient s

Efficacy and safety of a modified method for blind bedside placement of post-pyloric feeding tube: a prospective preliminary clinical trial.

OBJECTIVE: To compare the efficacy and safety of a new modified method of bedside post-pyloric feeding tube catheterization with the Corpak protocol versus electromagnetic-guided catheterization. MATERIALS AND METHODS: We conducted a single-center, single-blinded, prospective clinical trial. Sixty-three patients were treated with a non-gravity type gastrointestinal feeding tube using different procedures: modified bedside post-pyloric feeding tube placement (M group), the conventional Corpak protocol (C group), and standard electromagnetic-guided tube placement (EM group). RESULTS: The succe

Nasogastric tube drainage and pyloric intervention after oesophageal resection: UK practice variation and effect on outcomes.

BACKGROUND: Over 1500 patients with oesophageal cancer undergo a resection in the UK each year. At surgery, patients commonly have a nasogastric tube (NGT) placed and may undergo a pyloric intervention. There is conflicting evidence on the use of both NGTs and pyloric interventions during oesophageal resections. We performed a national survey of oesophageal centres and assessed practice variation. MATERIAL AND METHODS: An electronic survey was distributed to all resection centres in England, Wales and Scotland. Variations in practice regarding NGTs and pyloric intervention were assessed, and

Comparison of electromagnetic guided imagery to standard confirmatory methods for ascertaining nasogastric tube placement in children.

PURPOSE: Evaluate the accuracy of an electromagnetic device (EMD) guided nasogastric tube (NGT) placement compared with standard confirmation methods. A secondary aim was to determine if EMD guided NGT placement would avert potential pulmonary misplacements of the tube. DESIGN AND METHODS: Pediatric Intensive Care Unit (PICU) patients were enrolled if they had an NGT order during the study period of April 2014 through December 2016. Patients were included if they were one through 18 years of age. An EMD trained nurse inserted the NGT using EMD guidance. An insertion questionnaire, confirming

Alexis St. Martin Gastropexy: A Novel Technique for Gastropexy During Percutaneous Endoscopic Gastrostomy Tube Placement.

BACKGROUND: Percutaneous endoscopic gastrostomy (PEG) is a preferred method of long-term enteral nutritional support. Despite its ease of placement, it has a 4% major complication rate, requiring surgical intervention or hospitalization. Early PEG tube dislodgment can cause peritonitis, requiring emergent laparotomy at significant morbidity and cost. T-fasteners have been used as an adjunct gastropexy, but nearly one third migrate into the abdominal wall within the first 2 weeks. We describe a low-cost, minimally invasive technique using widely available surgical instruments to appose the gast

[Displacement of endobronchial tube in a neonate in spite of using recommended methods].

Placement of nasogastric tubes are common procedures in neonatal departments, but there is a risk of severe complications although it is considered an innocuous procedure. This case report describes accidental endobronchial tube displacement in a mature neonate despite using clinically recommended methods of verification. It illustrates how symptoms of tube displacement can be disregarded, especially in critically ill neonates. Radiologic examination is the recommended method to ensure tube position but it cannot be used as daily practice. Commonly used methods of bedside verification are disc

Nasojejunal feeding tube placement in immobile patients.

A technique for placement of nasojejunal feeding tubes has been developed that makes use of a long peel-away sheath for nasoesophageal access and a torque catheter for tube placement. The technique requires little dependent patient positioning. It was successful in 33 of 34 patients, 24 of whom were relatively immobile. Procedure time and fluoroscopy time were reduced, and irritation to the oropharynx, hypopharynx, and esophagus was minimized.

Use of tubes and radiographs in the management of small bowel obstruction.

During the past 10 years 311 consecutive patients were admitted with 342 episodes of small bowel obstruction (SBO). There were 193 cases of partial small bowel obstruction (PSBO) and 149 cases of complete small bowel obstruction (CSBO) as determined by interpretation of the abdominal radiographs done on admission. The purpose of this review was to determine the reliability of the admission plain abdominal radiographs and subsequent upper gastrointestinal (UGI) contrast studies in predicting the need for operative intervention. The use of nasogastric tubes (NGT) versus nasointestinal (long) tub

Errant placement of nasoenteric tubes. A hazard in obtunded patients.

We describe a case of intubation of both main-stem bronchi with a narrow-bore nasoenteric tube on two separate occasions, with the subsequent development of pleural effusions of enteral solution, in an elderly semicomatose woman with a properly positioned cuffed endotracheal tube. Neither aspiration of fluid from the tube nor propulsion of air with auscultation of gastric borborygmi are positive proof of proper positioning. We recommend that in obtunded patients, especially if there is the possibility of impaired mucosal integrity, appropriate placement of the tube should be confirmed by chest

Comparison of gastric cancer surgery with versus without nasogastric decompression.

There is a widespread belief that nasogastric decompression in gastric cancer surgery allows better surgical field and leads to the reduction of postoperative complications. The aim of this study was to evaluate whether gastric cancer surgery can be safely performed without nasogastric decompression. From March to June 2000, 119 patients with gastric adenocarcinoma were randomized into either a tubeless group (n=56) or an intubated group (n=63). Exclusion criteria included a history of upper gastrointestinal bleeding and pyloric obstruction. No remarkable difference was found in the incidence

An improved method for oesophageal intubation.

The authors describe a method of inserting an Atkinson tube using a flexible upper gastrointestinal endoscope which does not require the use of X-ray screening. A retrospective review of 50 patients intubated by this technique has been undertaken and the results presented. Of the lesions, 40% were squamous cell carcinoma and 40% adenocarcinoma, with 84% being situated in the lower third of the oesophagus; 94% of patients only required one intubation. The two major complications of the procedure are tube displacement and oesophageal leak; a tube displacement rate of 13% was recorded, comparable
