INTRODUCTION: Bromocriptin has been associated with stroke and myocardial infarction in the postpartum period. We report on the case of a patient who developed digital ischemia while receiving this drug.
EXEGESIS: Mrs D, 28 years old presented with digital ischemia occurring five days after the introduction of bromocriptin. Magnetic resonance imaging also displayed stroke in the area of the right posterior cerebellar artery. The course was favourable after discontinuation of the drug.

Acute mesenteric ischemia (AMI) is an abdominal emergency with a high mortality. Prompt revascularization can prevent intestinal infarction and reduce mortality. We report three cases of acute occlusive mesenteric ischemia without signs of intestinal necrosis, which were successfully managed with endovascular interventions. Mechanical thrombus fragmentation was performed and underlying chronic stenoses were treated with stent implantation. Prognosis of acute mesenteric ischemia depends on the quality of the initial management of the treating physician. To reduce the still appallingly high mortality rate of this disease a rapid, invasive and aggressive diagnosis and treatment is necessary. However, early signs and symptoms of mesenteric ischemia as well as laboratory studies are nonspecific. Therefore, definitive diagnosis requires invasive testing using mesenteric angiography. Surgery should not be delayed in patients with peritoneal signs and suspected of having intestinal perforation or gangrene.

This report describes an unusual etiology of coronary artery disease. A 60-year-old male presented with angina. He was treated with methysergide for migraine. It was determined that the patient possessed an extremely thick-walled ascending aorta that caused the coronary ostial narrowing. He underwent replacement of the ascending aorta and proximal aortic arch. Double vessel coronary artery bypass grafting was performed using saphenous vein. Microscopic examination indicated the pathology to be sclerosing mediastinis.

A 33-year-old woman who had undergone a Cabrol-type aortic root replacement for acute aortic dissection during labor 27 months ago was admitted with chest pain. Electron-beam tomography and coronary angiography showed stenosis at the level of the anastomosis. Urgent coronary revascularization was performed using bilateral internal mammary artery grafts. Although graft occlusion after the Cabrol procedure is an infrequent complication, it should be considered during follow-up.

OBJECTIVE: To examine cerebral arterial blood flow in 2 patients exhibiting signs of vertebrobasilar arterial ischemia (VBI) before and after spinal manipulative therapy.
CLINICAL FEATURES: Two patients had a repetitive/resting tremor, one from a spastic torticollis with the onset immediately after self-manipulation by the patient 6 months earlier, and the second one with a generalized resting tremor, hip clonus, dizziness, and presyncope. BACKGROUND: Brain tissue acidosis can result from ischemia when cerebral blood flow reduction reaches a critical value. The portable, battery-operated Khuri pH monitor has been used previously in a large number of patients undergoing cardiopulmonary bypass surgery to monitor the intramyocardial pH during aortic clamping.
