Examining the Role of Complement in Predicting, Preventing, and Treating Hemolytic Transfusion Reactions.

Red blood cell (RBC) transfusion is a critical component of optimal management for a broad range of conditions. Regardless of the indication, pretransfusion testing is required to appropriately match RBC donors and recipients to provide immunologically compatible blood. Although this approach is effective in the vast majority of situations, occasionally, patients will inadvertently receive an incompatible RBC transfusion, which can result in a hemolytic transfusion reaction (HTR). In addition, patients with life-threatening anemia and a complex alloantibody profile, which precludes rapid procu

Markers of susceptibility to acute rheumatic fever: the B-cell antigen D8/17 is not robust as a marker in South Africa.

BACKGROUND: Acute rheumatic fever and its chronic sequelae, rheumatic cardiac disease, and neuropsychiatric movement disorders, remain major public health problems in South Africa. Early identification and treatment of streptococcal pharyngitis in susceptible individuals would prevent rheumatic cardiac disease. The B-cell antigen D8/17 is a marker of susceptibility to rheumatic fever in some populations. METHODS AND RESULTS: We assessed the significance of the D8/17 marker in a group of South Africans. Blood was collected from 107 individuals; 40 patients had previous confirmed rheumatic feve

Differential immunogenicity of HLA class I alloantigens for the humoral versus the cellular immune response: "towards tailor-made HLA mismatching".

The immunogenicity of an individual human leukocyte antigen (HLA) class I mismatch is different for the cellular and the humoral alloimmune responses. The consequence is that the same antigen can induce a strong antibody response and no cytotoxic T lymphocyte reactivity, but the reverse can occur also. Exact knowledge of the immunogenicity of an HLA mismatch for an individual patient can lead to a strategy of tailor-made HLA mismatching if no HLA identical donor is available. Depending on the clinical situation, one should select a donor with HLA mismatches according to the humoral or cellular

Bystander immune cytolysis.

In addition to alloimmune and autoimmune cell lysis, a third category of immune destruction of blood cells should be recognized. This additional immunologic response occurs when cells or tissues are injured by immunologic reactions in which the cells act as "innocent bystanders." One mechanism by which an immune response to an exogenous antigen leads to the destruction of autologous blood cells is the temporary development of autoantibodies. This is actually an alloimmune reaction which results in a temporary state of "pseudo"-autoimmunity. Although originally described as a type of hemolysis

[Subcellular localization of blood group substances ABH in human gastrointestinal tracts].

Authers investigated subcellular localization of ABH substances in normal human stomach, duodenum and transverse colon by post-embedding immunogold method using monoclonal antibodies specific for A, B and H antigens. The immunoreactivity was usually restricted to the following organelles: mucigen granules of epithelical cells of gastric mucosa, zymogenic granules of chief cells of the gastric gland, both mucigen granules and Golgi cisternae of neck mucous cells of the gastric gland and of epithelial cells of the duodenum mucosa as well as the Brunner's gland. ABH antigens were also visualized

Histochemical analysis of blood group antigens in human sublingual glands and pancreas. An application of high-performance liquid chromatography to estimate the quantity of galactose liberated from tissue sections by alpha-galactosidase digestion.

Using immunostaining with monoclonal antibodies (mAbs) from three different sources as well as anti-B lectin, GSAI-B4 staining and alpha-galactosidase digestion, blood group B antigens were localized and analysed in tissue sections of sublingual glands from blood group B and AB individuals. Quantitative analysis of galactose was simultaneously carried out on the supernatant enzyme solution used for treating tissue sections by high-performance liquid chromatography (HPLC). In addition, galactose liberated from the pancreas tissues of blood group B and AB individuals was also estimated by HPLC a

Potential HLA-matched platelet donor availability for alloimmunized patients.

An analysis of the computer searches for HLA-matched donors was done for 100 alloimmunized patients requiring HLA-matched platelet transfusion support. With a pool of 2470 donors, an average of only 1.3 (range 0-14) perfectly matched donors were potentially available per patient. Patients had an average of 9 donors with no mismatched antigens. Although the number of potential donors increased when cross reactive or single antigen mismatches were considered, our estimate of donor availability of this type is lower than reported previously. More donors were identified for the 39 patients with re

[A study on HNA-1~-5 genotypes and frequencies in the blood donor population of jiangsu province].

Objective To investigate the distribution characteristics of the genotypes and allele frequencies of HNA-1~-5 alleles in a blood donor population from Jiangsu Province. Methods Genomic DNA was extracted from 300 peripheral blood samples collected from healthy donors. All samples were genotyped using an optimized polymerase chain reaction-sequence specific primer (PCR-SSP) protocol. Randomly selected results were verified by direct sequencing, and genotype distributions were tested for conformity to Hardy-Weinberg equilibrium. Samples identified as potential HNA-1 null were confirmed by polymer

Generation and Use of an Alloantigen-Specific Preclinical Humanized Mouse Model of Fetal and Neonatal Alloimmune Thrombocytopenia.

Fetal/neonatal alloimmune thrombocytopenia (FNAIT) is a life-threatening bleeding disorder caused by maternal antibodies targeting a paternally inherited alloantigen present on the surface of fetal platelets. The human platelet alloantigen HPA-1a/HPA-1b, also known as the PlA1/PlA2 alloantigen, is the most frequently implicated HPA for causing FNAIT in Caucasians. A single Leu33Pro amino acid polymorphism residing within the plexin/semaphorin/integrin (PSI) domain of the integrin β3 subunit (platelet membrane glycoprotein (GP), GPIIIa) is responsible for generating the HPA-1a and HPA-1b epitop

Human Neutrophil Alloantigen Genotype Frequencies Among Syrian Population.

BACKGROUND: Human neutrophil antigens (HNA) are able to provoke allo- and autoimmune antibodies which lead to reactions like autoimmune and neonatal neutropenia. However, until now no data about HNA genotype distribution in Syrian population exists. The aim of this study was to determine the HNA allele frequencies in the largest group asking for asylum in Germany since 2015. Allele frequencies were compared to data from German blood donors. Therefore, we calculated the risk of alloimmunization and associated transfusion reactions, as well as the risk of developing neonatal neutropenia for newb

Advances in Pretransplant Donor-Specific Antibody Testing in Solid Organ Transplantation: From Bench to Bedside.

Immunological risk stratification has a central role in determining both the feasibility of solid organ transplantation and the type (and amount) of induction and maintenance immunosuppressive therapy. Currently there is poor consensus on how to exactly estimate the global immunological risk, and most transplant centers adopt complicated internal guidelines for risk stratification. Here we systematically review published evidences that should drive appropriateness in risk stratification, focusing on donor-specific antibodies against HLA and other antigens.

Geno- and phenotyping of human neutrophil antigens.

For typing of human neutrophil antigens (HNA) usually genotyping techniques are used, except for HNA-2, which-due to a gene expression defect-requires phenotyping. For genotyping, several techniques have been described. Most reference laboratories use variations of the polymerase chain reaction (PCR) for antigen typing which showed good results in international quality assessment exercises. The granulocyte immunofluorescence test has been the gold standard technique for phenotyping for all HNA antigens except for HNA-3a and -3b phenotyping. The expression of the latter antigens on neutrophils

D8/17 and CD19 expression on lymphocytes of patients with acute rheumatic fever and Tourette's disorder.

D8/17, an alloantigen found on B lymphocytes, has been reported to be elevated in patients susceptible to rheumatic fever and may be associated with autoimmune types of neuropsychiatric disorders. The pediatric-autoimmune-neuropsychiatric-disorders-associated-with-streptococci model is a putative model of pathogenesis for a group of children whose symptoms of obsessive-compulsive disorder and Tourette's disorder (TD) are abrupt and may be triggered by an infection with group A streptococci. As a test of this model, we have examined D8/17 levels on the B cells of patients with TD and acute rheu

[D8/17 marker: implication to neuropsychiatry].

Recent studies suggest that there is a relationship between rheumatic fever (RF) and some neuropsychiatric disorders. Thus, it has been thought that autoimmune mechanisms might be related to the etiology of these neuropsychiatric disorders. It has also been demonstrated that a B cell alloantigen associated to RF is also abnormally overexpressed in patients with such neuropsychiatric disorders. This alloantigen is recognised by a monoclonal antibody known as D8/17. The aim of this article is to introduce the recent work done about D8/17 and its possible implications to the study of neuropsychia

The CCR5 receptor acts as an alloantigen in CCR5Delta32 homozygous individuals: identification of chemokineand HIV-1-blocking human antibodies.

The chemokine receptor CCR5 is the major coreceptor for infection by macrophage-tropic R5 HIV-1. A 32-bp deletion in the gene coding for CCR5 (CCR5Delta32) occurs with a frequency of 10% in the Caucasian population and results in a receptor protein that is truncated and not expressed at the cell surface. CCR5Delta32 homozygous individuals are apparently normal but resistant to infection with R5 HIV-1. In two individuals homozygous for CCR5Delta32, who had been repeatedly exposed to CCR5-expressing blood cells through sexual activity, we have identified antibodies to CCR5 that bound specificall

Specific elimination of alloreactive T cells by an anti-interleukin-2 receptor B chain-specific immunotoxin.

Graft-versus-host disease and graft rejection remain the two principal causes of morbidity and mortality after major-histocompatibility-complex-mismatched bone marrow transplantation. Human and animal models suggest that both CD4+ and CD8+ T cell subsets present in the donor inoculum are responsible for their initiation. Since the human mixed lymphocyte culture (MLC) and the HLA-restricted cytotoxicity may reflect cellular interactions occurring during GVHD and graft rejection, inhibitions of these responses may represent useful approaches for screening functional T cell depletion in experimen

[A new immunogenetic marker of rheumatism].

Use of monoclonal D8/17 antiserum was studied for identification of B-cell antigen in 44 rheumatic fever patients. The control group comprised healthy subjects (30) and patients suffering from rheumatoid arthritis (30), ischemic heart disease (IO); chronic tonsillitis (12) and different neoplasma of the brain (10). The preliminary genetic analysis of these patients has shown that monoclonal antiserum D8/17 can identify B-cell marker in 88-100 per cent of rheumatic fever cases.
