Sarcoidosis among US Hispanics in a Nationwide Registry.

BACKGROUND: Sarcoidosis has been well studied in multiple races and ethnic groups. However, there is a paucity of data that describes sarcoidosis in Hispanics. We aimed to determine the prevalence of Hispanic ethnicity, clinical characteristics and impact of sarcoidosis among Hispanics from a US based national registry. METHODS: We conducted a national registry-based study investigating 3835 respondents to the Sarcoidosis Advanced Registry for Cures questionnaire. This registry is a web-based, self-reported questionnaire that provides data related to demographics, diagnostics, organ involveme

Exploring Caregiver Perceptions of Post-High School Employment Experiences Among Young Adults With Down Syndrome.

This qualitative study explored employment experiences and perceived satisfaction of young adults with Down syndrome (DS) who recently exited high school, as reported by their caregivers (n = 101). We analyzed caregivers' open-ended responses about their young adults' type of employment (n = 52 were employed) and identified themes associated with reported satisfaction (for both employed and unemployed). Natural supports were key to caregiver satisfaction; few opportunities for paid, community-based employment and long waiting lists for formal services were related to caregiver dissatisfaction.

[Work performance as a risk factor for carpal tunnel syndrome].

Certification of carpal tunnel syndrome (CTS) as an occupational disease should be based on evidence that the job performance is a dominant factor responsible for its development. Features of the way the job is performed, which can increase the constriction in carpal tunnel and their quantitative analysis are presented. They include a specific position of the hand during the job performance (dorsal bending, alternate bending and extension), e.g., overcoming resistance with fingers, pincher grip, object catching and holding, exerting pressure on the hand, repeated movements or work with vibrato

Burnout: The Resident Evil - Perspectives from the Horses' Mouth!

Burn-out is a syndrome conceptualized as resulting from chronic workplace stress that has not been successfully managed. It is characterized by three dimensions- (1) feelings of energy depletion or exhaustion, (2) increased mental distance from one's job, or feelings of negativism or cynicism related to one's job and (3) reduced professional efficacy. Any profession where a certain amount of emotional work and empathy are expected renders a person vulnerable to being burned out. Resident doctors, who play the role of a primary care-giver and spend a great deal of time with patients, are most p

Yoga for stress reduction and injury prevention at work.

At work employees face numerous psychological stressors that can undermine their work performance. These stressors, stemming from a variety of possible causes, have enormous health and financial impacts on employees as well as employers. Stress has been shown to be one of the factors leading to musculo-skeletal disorders (MSDs) such as: include back pain, carpal tunnel syndrome, shoulder or neck tension, eye strain, or headaches. Yoga is an ancient form of exercise that can reduce stress and relieve muscular tension or pain. Practicing yoga at the workplace teaches employees to use relaxation

Obesity as a risk factor for slowing of sensory conduction of the median nerve in industry. A cross-sectional and longitudinal study involving 429 workers.

As part of a longitudinal study of the etiology of carpal tunnel syndrome in industry, we evaluated weight and body mass index (BMI) as risk factors for slowing of sensory conduction of the median nerve. Using nerve conduction studies, we evaluated 858 hands of 429 industrial workers in 1984 and reevaluated 630 hands of 316 of these same workers in 1989. Weight and BMI were strongly and positively correlated with the maximum latency difference (MLD). In 1984 and 1989 the risk for abnormal nerve conduction averaged 3.5-fold and 4.1-fold greater, respectively, in the obese workers than in the sl

[Combined immunodeficiency due to DOCK8 deficiency. State of the art].

Combinedimmunodeficiency (CID) due to DOCK8 deficiency is an inborn error of immunity (IBD) characterized by dysfunctional T and B lymphocytes; The spectrum of manifestations includes allergy, autoimmunity, inflammation, predisposition to cancer, and recurrent infections. DOCK8 deficiency can be distinguished from other CIDs or within the spectrum of hyper-IgE syndromes by exhibiting profound susceptibility to viral skin infections, associated skin cancers, and severe food allergies. The 9p24.3 subtelomeric locus where DOCK8 is located includes numerous repetitive sequence elements that predis

[The cervicobrachial syndrome in women in the metal-working industry].

All metal workers from a metal-working industry (eight women) who operated pressing, cutting and boring machines were found to have cervicobrachial syndrome. Their job involved forced-sitting body posture and repetitive movements. These are particularly important risk factors at the workplace for the occurrence and development of cervicobrachial syndrome. The workers demanded medical assistance at 12 years of work and an average age of 32.1 years. The working chair of poor ergonomic design was established as a major cause of the early occurrence of cervicobrachial syndrome.

Rubinstein-Taybi Syndrome With Severe Eczema, Recurrent Infections, and Hyper IgE Profile Responsive to Dupilumab Treatment.

Rubinstein-Taybi syndrome (RTS) is a rare autosomal dominant disorder characterized by post-natal growth retardation, facial dysmorphism, large thumbs and halluces, and intellectual deficits. RTS has a broad phenotypic spectrum including cardiac, genitourinary, digestive, ear, nose and throat, and skin manifestations. Patients also have an increased risk of severe infections, developing benign tumors, and immunological abnormalities suggesting primary immunodeficiencies. We report a case of RTS with severe recalcitrant eczema, recurrent skin and chest infections, food allergies associated with

Traumatic brain injury and occupational risk of low-level blast exposure on adverse career outcomes: an examination of administrative and medical separations from Service (2005-2015).

INTRODUCTION: Although traumatic brain injury (TBI) has been linked with adverse long-term health, less research has examined whether TBI is linked with non-clinical outcomes including involuntary job loss. Symptoms associated with TBI may influence one's ability to maintain gainful employment including employment in the U.S. military. That influence may impact military service members with exposure to repetitive low-level blast (LLB). Understanding the association between TBI and involuntary job loss outcomes among military populations is particularly important as it may be associated with di

Validation of the Revised Strain Index for Predicting Risk of Incident Carpal Tunnel Syndrome in a Prospective Cohort.

The Revised Strain Index (RSI), a model that quantifies physical exposure from individual hand/wrist exertions, tasks, and multi-task jobs, was used to quantify exposure for 1372 incident-eligible manufacturing, service and healthcare workers. Workers were followed for an average of 2.5 years (maximum 6 years) and had an average carpal tunnel syndrome (CTS) incidence rate of 4.6 per 100 person-years. Exceeding the a-priori RSI limit of 10.0 showed increased risk of CTS (Hazard Ratio (HR) = 1.45, 95% CI: 1.11-1.91, p = 0.01). There also was a dose-response relationship using proposed low (RSI ≤

[What is burnout syndrome?].

Now, more than ever, everyone is talking about burnout syndrome. As was previously the case with neurasthenia, the aim is to characterize a job-related stress and non-stigmatized clinical picture. However, burnout is neither a psychiatric diagnosis nor a clearly delineated scientific concept. The primary symptoms constitute emotional exhaustion, changes in behavior at the workplace and impaired performance. Some of the causes may be attributable to chronic stress at work, but also to biographically related vulnerability and general meaninglessness in our society. Prevention and treatment focus

Interventional pulmonary procedures and their outcomes in patients with STAT3 hyper IgE syndrome.

BACKGROUND: STAT3 hyperimmunoglobulin E syndrome (STAT3-HIES) also referred to as autosomal dominant HIES (AD-HIES) is an inborn error of immunity characterized by the classic triad of eczema, frequent opportunistic infections, and elevated serum IgE levels. As a consequence of lung sequels due to repeated infections and impaired tissue healing, patients may require interventional pulmonary procedures. METHOD: Four patients with dominant-negative STAT3 mutations who had received interventional pulmonary procedures were enrolled. The demographic, clinical, and molecular characteristics were ga

A challenging differential diagnosis in a patient with autosomal dominant STAT3 deficiency.

Autosomal dominant hyper IgE syndrome (AD-HIES) is a primary immunodeficiency caused by loss-of-function (LOF) mutations in the Signal Transducer and Activator of Transcription 3 (STAT3) gene. In these patients, performing a correct differential diagnosis of pulmonary infections is difficult and challenging, as they usually have atypical presentations. However, establishing a correct diagnostic and therapeutic approach is essential, as pulmonary complications are responsible for high morbidity and mortality rates in these patients. We report the case of a teenage girl with AD-HIES and respirat

Very high immunoglobulin E levels in children: when from pediatrician to immunologist?

BACKGROUND: Total serum immunoglobulin E (IgE) is increased in many situations such as allergic diseases, primary immunodeficiencies (PID), parasitosis, infections and malignancies. When IgE levels are >1000 kU/L PIDs are suspected by pediatricians. We tried to define some clinical and laboratory parameters to distinguish PID from the others. METHODS: We evaluated 158 children between 1.7-17 years (mean: 6.6±3.4) for allergic diseases, PID, parasitosis and others. Total IgE, specific IgE, immunoglobulin levels and skin prick tests were performed to all patients. Parasite investigations, viral

[Hyper-immunoglobulin E syndrome: report of one case].

The Hyperimmunoglobulin E syndrome (HIES) is a rare sporadic or autosomal dominant immune and connective tissue disorder characterized by chronic eczema, cutaneous abscesses, pneumonias, invasive infections, high levels of Immunoglobulin E, primary teeth retention and bone abnormalities. We report a 24-year-old male with a history of cutaneous abscesses and esophageal candidiasis. He was admitted due to a left gluteal cellulitis. During the fifth day of hospitalization he presented a distal necrosis of the fourth finger of the right hand. Laboratory results showed high levels of IgE and positi

Candida endocarditis in a child with hyperimmunoglobulinemia E syndrome.

Hyperimmunoglobulin E syndrome (HIE) is a disorder characterized by extremely elevated serum levels of IgE and recurrent infections. Patients are particularly predisposed to have staphylococcal abscesses, usually involving skin, lungs, and joints; but they are also at risk for infections with other bacteria and fungi. We report the case of a 46-month-old boy with HIE who had Candida endocarditis and sepsis with a large fungal mass extending through the tricuspid valve and into the surrounding heart tissue, requiring surgical excision and replacement with a prosthetic valve. He had an indwellin
