INTRODUCTION: Temporo-mandibular joint ankylosis is a common cause of acquired deformity in children.
PURPOSE: Surgical correction of the ankylosis only leaves the patient with an uncorrected mandibular deformity. This study was to evaluate the use of distraction osteogenesis for simultaneous correction of the mandibular deformity.
MATERIAL: This study was done on six children with temporo-mandibular joint ankylosis and mandibular deformity. Uniaxial double pin distractors with Schanz pins were used in this study.
METHODS: The patients underwent simultaneous gap arthroplasty and mandibular

The purpose of this study is twofold: first, to measure the joint contact pressure across the calcaneocuboid joint in a planovalgus deformity and compare the results to pressures measured in a normal foot; and second, to determine the change in pressure across the calcaneocuboid joint after an Evan's-type calcaneal lengthening osteotomy. The effect of this procedure on the calcaneocuboid joint was evaluated using seven cadaver feet to measure peak pressure across the calcaneocuboid joint under a constant load. Each foot was sectioned medially to reproduce a deformity consistent with an adult,

Disorders of the distal radioulnar joint (DRUJ) are relatively common and often associated with the triangular fibrocartilage complex. Anatomically and biomechanically, the DRUJ should be considered in the broader context of the forearm joint, which has both distal and proximal articulations. Clinical examination is the best method of evaluating trauma to the DRUJ or DRUJ instability, although the clinical appearance of disorders may be subtle and imaging studies may be difficult to interpret. Arthroscopy is the best tool for evaluating the integrity of the triangular fibrocartilage complex. A

This study examined radiographically the prevalence of arthritis mutilans hand deformities in an inception cohort of 68 rheumatoid arthritis (RA) patients. Hand deformities of 103 RF-positive RA patients were evaluated after 8 years, 83 patients after 15 years and 68 patients 20 years after entry. The grade of destruction in the hand joints was assessed by the Larsen method and Larsen scores of 0-50 were determined for both PIP (+IP) and MCP joints. At the end point, 3 patients had Larsen scores > or =40 for both PIP and MCP joints. These three patients had severe resorption in most of the fin

The ankle joint unites high functionality with high load on a small surface. Congenital as well as acquired variations lead to a derangement of the complex concerted action of tendons, ligaments and joints. This causes dysfunction and the development of typical deformities. It is necessary to discriminate physiological variants, congenital, posttraumatic and tumoral variations for the differential diagnosis. In most instances this is only possible with the aid of radiological clarification. Congenital malformations in the region of the upper and lower ankle joint are generally rare. One diseas

MR imaging can be extremely helpful in evaluating congenital and acquired deformities of the foot and ankle. Knowledge of the normal developmental anatomy of the foot and ankle is important to accurately identify abnormalities versus normal variants. The foot and ankle may be abnormal because of trauma, infection, tumor, or systemic disease. MR imaging can help in the differential diagnosis of abnormalities as well as in treatment planning and follow-up.

INTRODUCTION: Distraction osteogenesis, has emerged as a revolutionary concept and an effective means to treat extensive craniomaxillofacial defects and malformations. In cases of mandibular distraction for patients with extreme acquired mandibular deformities, there is so far, no recorded literature on precise quantification and scientific estimation of the percentage of the efficacy of intraoral monoplanar distractors, for an objective evaluation of their effectiveness. The objective was to study the efficacy of intraoral mandibular monoplanar distractors in the correction of severe acquired

Although cubitus varus has been regarded as a purely cosmetic problem in the pediatric population, symptomatic elbow instability, and ulnar neuropathy from the mechanical axis malalignment have been reported in adults. This overview discusses the biomechanical axis disruption that leads to soft tissue and morphologic bony alterations in the elbow and offers a compelling argument for corrective osteotomy to treat pediatric cubitus varus.

BACKGROUND: Osteoarthritis of the hands is a prevalent musculoskeletal disease with a considerable effect on patients' lives, but knowledge and research results in the field of hand osteoarthritis are limited. Therefore, the Disease Characteristics in Hand OA (DICHOA) initiative was founded in early 2005 with the aim of addressing key issues and facilitating research into hand osteoarthritis.
OBJECTIVE: To review and discuss current knowledge on hand osteoarthritis with regard to aetiopathogenesis, diagnostic criteria, biomarkers and clinical outcome measures.
METHODS: Recommendations were m

Recurrent varus deformity and an associated lateral instability are a common and perplexing concern after high tibial osteotomy. An anatomic study using fresh cadaver specimens was done to delineate the cause of this complication. A closing wedge osteotomy was done on four cadaveric knees, which then were subjected to a small varus load to simulate the adduction moment experienced by the knee during normal gait. The osteotomy was progressively closed in 5 degree increments and the angular alignment of the knee was measured with each incremental change. Experimental results identified postosteo

In patients with rheumatoid arthritis, metacarpophalangeal joint deformities can significantly affect hand function. Flexible hinge implant arthroplasty, designed in the 1960s, remains the most accepted and widely performed technique for treatment of severely involved metacarpophalangeal joints in rheumatoid arthritis. An arc of motion of 40 degrees to 60 degrees can be expected after arthroplasty, with improvement of finger extension and ulnar deviation. Silicone implant arthroplasty, although technically challenging, is the standard surgical procedure for improving hand function in these pat

Multiple reconstruction techniques have been described for correction of boutonniere deformities including direct repair, central slip reconstruction, lateral band reconstruction, transverse retinacular ligament reconstruction, staged reconstruction, and extensor tenotomy. Each technique has been reported to have variable results with complications including capsular contracture, loss of proximal interphalangeal flexion, and residual deformity. We describe a surgical technique for central slip reconstruction using a slip of the flexor digitorum superficialis tendon through a bone tunnel.

In cases of adult acquired flatfoot associated with peritalar destabilisation, special reference is made to the plantar calcaneo-navicular (spring) ligament's degenerative disease (degenerative glenopathy) and to the presence of the accessory navicular bone as a possible pathogenic cause. Peritalar destabilization syndrome is proposed for the articular (subtalar and talo-navicular joints) or tendinosis (tibialis posterior tendon) separately or in association with degenerative glenopathy of the coxa pedis. In degenerative glenopathy surgical reconstruction of the glenoid also makes use of a pos

The results of limb lengthening, correction of limb deformities and solving of bone loss, by the use of Mitkovic external fixation system and minimally invasive technique on the series of 96 operations in 89 patients are shown. Only lower extremities were included in this series. The advantages of this external fixation system in comparison to other systems are pointed out. The main advantages are the optimal biomechanical conditions including unilateral but 3-dimensional frame, which provides conditions very similar to natural biomechanical features of the human long bones, simplicity of appl

BACKGROUND: In 1985, the senior author (C.S.R.) developed a new soft-tissue release technique to balance valgus knees to avoid unacceptably high rates of late-onset instability and the need for primary constrained implants. This report describes the soft-tissue release technique and its long-term results when performed in primary total knee arthroplasty in patients with a severe valgus knee deformity.
METHODS: Four hundred and ninety consecutive total knee arthroplasties were performed by one surgeon between January 1988 and December 1992. In this group, seventy-one patients (eighty-five knee

An approach to the valgus knee based on anatomic function of ligaments in flexion and extension consistently yields a knee that is balanced in flexion and extension when the implants have been positioned correctly. Two hundred thirty-one knees had a valgus deformity (range, 12 degrees-45 degrees) and were corrected with valgus alignment to 5 degrees by resecting the intact joint surfaces to match implant thickness. Femoral joint surfaces were aligned in 5 degrees valgus to the long axis of the femur and parallel to the epicondylar axis of the femur in flexion and extension. The tibial surfaces

Between 1970 and 1992, 23 patients with cubitus varus deformities following elbow fractures underwent 26 corrective lateral-wedge osteotomies at Texas Scottish Rite Hospital. Internal fixation was most commonly provided by either small Steinmann pins, two screws and a figure-of-eight wire, or a small two-hole plate. Sixteen patients had a good result and 10 were judged to have poor results. Two of the 10 patients with poor results had inadequate intraoperative correction. Unstable internal fixation allowed osteotomy fragments to slip into varus in the remaining eight poor results (three with S
