Endothelial system dysregulation underlies keloid development and therapeutic targeting.

Cutaneous keloids are pathological scars resulting from aberrant wound healing, characterized by infiltrative growth beyond the original wound margins and a failure to regress spontaneously. Although traditionally attributed to excessive extracellular matrix deposition and fibroblast proliferation, emerging evidence underscores the pivotal role of vascular dysregulation and endothelial dysfunction in keloid pathogenesis. This review systematically examines the endothelial cell system-comprising vascular endothelial cells (VECs) and lymphatic endothelial cells (LECs)-within the keloid microenvi

A rare case of keloid combined with bullous pemphigoid.

Keloids and bullous pemphigoid (BP) are two clinically and pathophysiologically distinct dermatologic disorders - the former is characterized by abnormal scar formation due to excessive collagen deposition, whereas the latter is an autoimmune blistering disease. We report a case of a 67-year-old man with a 19-year history of abdominal keloids and the subsequent appearance of vesicular skin lesions. Histopathologic examination revealed both keloidal fibrosis and subepidermal blister formation. Serum testing demonstrated elevated anti-BP180 antibody levels. Initial treatment with topical and sys

Keloid masking a parotid tumor: A rare case presentation.

A 37-year-old Nigerian woman presented with an itchy and occasionally painful scar that appeared after the healing of an acne lesion over the left parotid region, 13 years ago. She noticed a progressive increase; however, there was no history of facial weakness. Examination revealed a keloid lesion overlying a firm nontender mass. Ultrasound scan and fine-needle aspiration cytology indicated a benign tumor of the left parotid. Superficial parotidectomy was performed, and the histology revealed a keloid overlying a pleomorphic adenoma. This is an unusual presentation of keloid which developed o

Extensive keloid scarring in a patient with pemphigus vulgaris.

Since pemphigus blisters are intraepidermal, scarring should induce at most a post-inflammatory hyperpigmentation. We describe a very atypical and unusual course of pemphigus vulgaris with extensive keloid formation despite high systemic steroids. This could be promoted by the severe flare of the disease, the delay of scarring, and the superinfection.

Targeting of keloid with TRAIL and TRAIL-R2/DR5.

Keloid is a common and frequently-occurring disease in plastic surgery, and its ugly appearance and itching symptoms bring mental and life pain to patients. However, the clinical treatment of keloid, such as drug injection treatment, surgical resection, cryotherapy, laser treatment and other therapeutic effects are poor. Since the discovery of tumor necrosis factor related apoptosis inducing ligand (TRAIL) in 1995, its selective apoptosis on tumor cells makes it have a great prospect in the targeted treatment of tumor. In recent years, it has been found that the formation of keloid is related

Characterization of CD45RO+ memory T lymphocytes in keloid disease.

BACKGROUND: Memory T cells, a highly effective subset of T lymphocytes, have been reported to be involved in many inflammatory skin disorders. However, the potential role of memory T cells in keloid disease (KD) remains unclear. OBJECTIVES: Due to their important role in regulating inflammation, we investigated the characteristics of CD45RO+ memory T cells in KD. METHODS: Primary cutaneous cells were isolated from keloid scars and normal skin by enzymic digestion. Peripheral blood mononuclear cells were isolated from a related blood sample, and flow cytometry was applied to identify the phen

Helical rim advancement - A technique to avoid keloid recurrence.

INTRODUCTION: Helical rim keloids occur commonly following ear piercings, trauma and previous surgeries and can be disfiguring. Many techniques have been developed to treated these disfiguring lesions with varying successes, however, individuals prone to developing keloids inadvertently recur despite best efforts. OBJECTIVE: To determine whether helical rim advancement flap reconstruction following helical rim keloid excision can reduce recurrences. DESIGN: Case series followed up to 2 years. SETTING: Single Centre Tertiary Hospital Facial Plastics Service. PARTICIPANTS: All patients who c

Embedded tooth fragment masquerading as keloid for 11 months.

A foreign body retained in the skin can originate from numerous sources and most of the time these foreign bodies are removed without any sequelae. The present article reports a case of persistent swelling in the lower lip misdiagnosed as keloid, which later turned out to be an embedded tooth fragment identified post spontaneous extrusion after 11 months.

Treatment of keloid formation following syndactyly division: surgical technique.

A strong association has been reported between keloid formation after syndactyly reconstruction and primary digital enlargement, and methotrexate (MTX) treatment has been shown to produce promising results in a few reported cases. However, detailed surgical technique for revision of keloid formation after syndactyly division has not been well described, and there is still no standard treatment protocol regarding use of MTX in such cases. In this paper, we describe the technical details of keloid excision and full thickness skin grafting followed by MTX medication for the treatment of massive k

Comparative proteomic analysis between normal skin and keloid scar.

BACKGROUND: Keloids are pathological scars and, despite numerous available treatment modalities, continue to plague physicians and patients. OBJECTIVES: Identification of molecular mediators that contribute to this fibrotic phenotype. METHODS: Two-dimensional gel electrophoresis, MALDI-TOF, Mascot online database searching algorithm and Melanie 5 gel analysis software were employed for comparative proteomic analysis between normal skin (NS) and keloid scar (KS) tissue extracts. RESULTS: Seventy-nine protein spots corresponding to 23 and 32 differentially expressed proteins were identified i

Intralesional 5-fluorouracil in the treatment of keloid scars.

Two patients with keloid scars are described. The first patient presented with extensive keloid scarring on both cheeks secondary to acne. The second patient developed a keloid scar on her chest following excision of a mole. Both patients' scars were diagnosed clinically and treated with fortnightly injections of a mixture of 5-fluorouracil and betamethasone acetate and betamethasone sodium phosphate. At each injection session up to 1.6 mL of 5-fluorouracil at a concentration of 500 mg/10 mL and 0.4 mL of betamethasone acetate and betamethasone sodium phosphate (as betamethasone acetate 3 mg i

Aggressive keloid scarring of the Caucasian wrist and palm.

Keloid scarring of the distal upper extremity is very rare. We report a Caucasian woman who presented with aggressive keloids of the hand and wrist causing De Quervain's syndrome, superficial radial-nerve entrapment and ulnar-nerve compression at the wrist. Multiple operations were required to alleviate her symptoms. A number of management conundrums arose, requiring defensive planning to pre-empt the possible complications of recurrent keloid scarring as a result of the surgical procedures.

The molecular basis of keloid and hypertrophic scar formation.

Excess scar formation secondary to traumatic or surgical injuries can have devastating consequences, ranging from body disfigurement to organ dysfunction. Hypertrophic scars and keloids are skin fibrotic conditions that can be caused by minor insults to skin, such as acne or ear piercing, or by severe injuries such as burns. Differences between keloids, hypertrophic scars and normal scars include distinct scar appearance, histologic morphology and cellular function in response to growth factors. Recent advances in our understanding of the wound healing process reveal possible causes for hypert

Morphological and immunochemical differences between keloid and hypertrophic scar.

There are two types of excessive scarring, keloid and hypertrophic scar. Contrary to hypertrophic scars, keloids do not regress with time, are difficult to revise surgically, and do not provoke scar contractures. These two lesions require different therapeutic approaches but are often confused because of an apparent lack of morphological differences. We have investigated the collagen organization and the possible presence of alpha-smooth muscle (SM) actin-expressing myofibroblasts in these conditions. Keloids contain large, thick collagen fibers composed of numerous fibrils closely packed toge
