Tibial plateau fractures are severe injuries of the knee joint and oftentimes lead to the development of posttraumatic osteoarthritis. A proportion of patients therefore requires secondary arthroplasty. Total knee arthroplasty after tibial plateau fracture, however, carries a markedly higher risk for complications when compared to primary arthroplasty, especially in geriatric patients. Bone defects, malalignment, instability, and a higher infection rate are major concerns in this subset of patients. Retained hardware and compromised soft tissues may further complicate surgical treatment. Caref

OBJECTIVES: Knee osteoarthritis (KO) is a leading contributor to disability years, with a prevalence ranging from 22% to 39%. Tibia stress fractures (TSFs) are well-described for end-stage arthritis patients undergoing TKA. Literature is ambiguous, with a wide range of management options. This study primarily aims to compare and determine the clinical, functional, and radiological outcomes of TSFs in end-stage KO treated with TKA by propensity score-based matching.
METHODS: It is a retrospective, single-center, comparative study conducted at University Teaching Hospital. The institutional med

Rheumatoid arthritis (RA) is a chronic systemic disease that causes progressive joint damage, bony defects, and ligament imbalance. These sequelae of RA present major difficulties to surgeons during hip or knee arthroplasty. The presence of coexistent periarticular fractures adds to these difficulties and represents a surgical dilemma. No guidance exists within the literature for the medical and surgical management of complicated cases of RA with coexistent fractures. So far, the evidence has focused on fixation techniques, arthroplasty, and conservative management for periarticular fractures

BACKGROUND: Post-traumatic stiffness after open reduction and internal fixation of fractures about the knee can have dramatic effects on function. Traditionally, open quadricepsplasty has been the treatment of choice, but is associated with significant morbidity.
PURPOSE: The purpose of this study is to examine the immediate and sustainable range of motion (ROM) changes after surgical arthroscopic lysis of knee adhesions (SALKA) for post-traumatic knee stiffness after open reduction internal fixation (ORIF).
METHODS: We retrospectively reviewed a consecutive series of patients at a single in

Subchondral Insufficiency Fracture of the Knee (SIFK) is a disease with unclear pathogenesis and rapid progression, it is prone to developing end-stage osteoarthritis. Due to the lack of a definitive treatment strategy for SIFK in the clinic, knee replacement is often the preferred measure. However, in recent years, as the disease has become better understood, more and more effective treatments have been proposed, but there is a lack of summarization. Therefore, this article summarizes its pathogenesis and therapeutic advances.

AIMS: Periprosthetic fractures (PPFs) around the knee are challenging injuries. This study aims to describe the characteristics of knee PPFs and the impact of patient demographics, fracture types, and management modalities on in-hospital mortality.
METHODS: Using a multicentre study design, independent of registry data, we included adult patients sustaining a PPF around a knee arthroplasty between 1 January 2010 and 31 December 2019. Univariate, then multivariable, logistic regression analyses were performed to study the impact of patient, fracture, and treatment on mortality.

105 patients with intra- and periarticular fractures of the knee joint were operated on with the use of nail and spin-nail apparatus constructed by A.I. Gorodnichenko. The possibility of atraumatic closed bone fragments fixation allowed quick patients' activization, and, as follows, improved their life quality, decreased the hospital stay and minimized complication rate. Fracture consolidation was achieved in all cases, the only complication registered was skin inflammation around the nail in 5 patients.

Sixty-one supracondylar fractures above prosthetic knee replacements in 58 patients were reviewed with a mean follow-up time of 3.7 years (range, 9 months to ten years). The mean interim between arthroplasty and fracture was 2.9 years (range, intraoperative to ten years). Twenty-seven cases demonstrated notching of the anterior femoral cortex. Seventeen patients suffered from a severe neurologic disorder. Group A consisted of 30 patients with 31 fractures treated by open reduction and internal fixation or revision arthroplasty. Follow-up study revealed 25 unions, three malunions, one nonunion,

We reviewed the data on thirty-six supracondylar fractures of the femur (in thirty-four patients) that occurred after total knee arthroplasties that were done between April 1974 and December 1981. Patients who had osteoporosis, rheumatoid arthritis, one or more previous arthroplasties of the knee, or inadvertent breeching of the anterior aspect of the femoral cortex at operation appeared to be particularly at risk for a supracondylar femoral fracture. Malalignment of the component could not be implicated as a cause. Twenty-six fractures (in twenty-five patients) were treated by non-operative m

Cortical bone pegs have been used in osteochondritis dissecans to fix the loosened or loose fragment. In osteochondral fractures of the knee the fragment has either been removed or reattached usually with headless pins or ASIF screws and only occasionally with cortical bone pegs. We report on four patients with osteochondral fractures of the knee, where cortical bone pegs were used as the means of fixation, with good results.

The management of complicated distal femur fractures (DFF) of the elderly continues to pose a challenge. Knee mega-prosthesis are mostly used for Total knee arthroplasty revision and tumor resection surgery but they can be used for the treatment of complex knee fractures. The purpose of the present study is to examine the short- to mid- term outcomes of their use for complex DFF of the elderly. We retrospectively identified 4 patients with DFF AO33C3 on osteoporotic bone treated by total knee arthroplasty from September 2015 to October 2019. The average age at the time of the surgery was 79,5

Wear debris of polyethylene, polymethylmethacrylate, and metal have been recognized to be associated with foreign body reactions, osteolysis, and aseptic prosthetic loosening after joint replacement arthroplasty. Further, foreign body reaction due to the presence of extensive wear debris can cause aggressive granulomatous lesions and pathologic fracture. To our knowledge, there has been no previous report of pathologic fracture of the femur due to an agressive pseudotumor. This report describes a case of pathologic supracondylar fracture of the femur 6 years and 5 months after cementless total

A 57-year-old black man with sickle cell disease was admitted to the hospital because of a painful crisis. After a fall with a fracture into the right knee joint, he developed an acutely painful, swollen knee. Synovial fluid from the right knee showed leukocyte counts of up to 154,000/mm3 and was negative for urate and calcium pyrophosphate dihydrate crystals. Gram stains and cultures were negative. Some sickled red cells were seen by light microscopy; electron microscopy revealed crystal-like arrays of sickled hemoglobin tactoids in erythrocytes which were enfolded and phagocytized by the cel

BACKGROUND: Knee fractures may lead to post-traumatic knee osteoarthritis and subsequent TKA in some patients. However, absolute risk estimates and risk factors for TKA in patients with knee fractures compared with those of the general population remain largely unknown. Such knowledge would help establish the treatment burden and direct patient counseling after a knee fracture is sustained.
QUESTIONS/PURPOSES: (1) What is the short-term risk of TKA after knee fracture? (2) What is the long-term risk of TKA after knee fracture? (3) What are the risk factors for TKA in patients with knee fractu

An isolated periprosthetic compression fracture following total knee arthroplasty has not been described in periprosthetic fracture classifications. Thus, the purpose is to describe this unique type of fracture based on clinical and radiographic analysis and identify the incidence and potential risk factors of this fracture. A retrospective chart review was performed from a database of 5864 primary total knee. A total of 56 (0.9%) periprosthetic fractures were identified with 15 (26.8%) of them demonstrating an isolated lateral compression fracture. Patients exhibiting this fracture pattern ha

Functional treatment of fractures is a nonoperative technique used in intraarticular knee fractures and multifragment and multilevel fractures of the shaft of femurs since 1974. The basic principle of this technique is placement of the fractured extremity in the specially designed rehabilitation splint, with skeletal traction fixed above the ankle joint through both tibia and fibula. Traction reduces the fracture while stretching of muscles provides conditions favorable for callus formation. Exercises are first isometric and then isotonic. During exercises, muscle tone and moments of force in

A series of 110 patients with 114 fractures of the femur were reviewed an average of 3.9 years after injury. Demonstrable knee ligament laxity was present in 31 (27 per cent) of these patients, while 13 (11 per cent) complained of instability. Thirty-three patients with 34 ipsilateral femoral and tibial shaft fractures were examined an average of 3.7 years after injury. Demonstrable knee ligament laxity was present in 18 (53 per cent) of these patients, while 6 (18 per cent) complained of instability. Most of the patients with instability had a rupture of the anterior cruciate ligament with or
