Bone bruise patterns in knee injuries: where are they found?

PURPOSE: Bone bruise represents an entity of occult bone lesions that can occur in the knee, causing knee pain and tenderness clinically. The aim of this study was to investigate the incidence and pattern of bone bruising seen in the anterior cruciate ligament (ACL) injury cohort, the non-ACL injury cohort, and between both cohorts. METHODS: We reviewed 710 knee magnetic resonance imagings performed over a 6-month period. Eighty-eight patients with prior history of a knee injury were identified. The mechanism of injury and other clinical findings was noted. RESULTS: Among these 88 patients,

The epidemiology of knee and ankle injuries on Macquarie Island.

Macquarie Island is a small, rugged sub-Antarctic island with a scientific research station and a considerable reputation for knee and ankle injuries amongst the Australian Antarctic Division expeditioner population. In order to examine the accuracy of this reputation, a 10-year retrospective analysis of all knee and ankle injuries recorded by the Macquarie Island Medical Officer in the medical logs was undertaken. Knee and ankle injuries comprised 13% of the 2,678 recorded medical consultations. The majority of initial injuries occurred in the field and almost a third occurred during work rel

Contributing factors influencing the functional outcome of floating knee injuries.

The purpose of the present study was to retrospectively review the floating knee injuries treated at our institute and to determine various factors, such as severity of soft-tissue or skeletal injuries, site of fractures, and treatment methods that may significantly influence the final functional result in these injuries. Between 1986 and 1996, 65 patients with 66 floating knee injuries were treated in our institution. Among 66 fractures of the femur, 19 (29%) were open. There were 43 open tibial fractures. Fifty cases were Fraser type I floating knee fractures, 7 were type IIa, 2 were type II

Emergency department evaluation and treatment of knee and leg injuries.

The knee is one of the most commonly injured joints in the human body, and, largely because of athletic injuries, they are increasing in frequency in the United States. This article provides a brief overview of knee anatomy, examines radiographic imaging techniques and arthrocentesis of the knee, and discusses injuries specific to the knee. An overview of leg anatomy is also presented, along with discussions of specific fractures common to the leg.

[Results of diagnostic sonography in injuries of the knee joint].

In an experimental study we examined three knee specimens with regard to the value of ultrasound for imaging soft tissue structures. Afterwards we documented artificial lesions of the menisci and ligaments. With this experience we started a prospective study of 101 patients suffering from acute knee injuries or chronic knee complaints. We compared the results of preoperative ultrasound investigation done by two different examiners with the final diagnosis after arthroscopy. For ultrasound evaluation we used a 5 MHz linear and a 7.5 MHz sector scanner. In both the experimental and the clinical

The diagnosis of knee injuries in casualty--a prospective study.

A prospective study of all knee injuries seen at a Casualty Department was organized. A clinical diagnosis was made in each case, based on standardized criteria, and the subsequent progress followed. Meniscal tears tended to be over-diagnosed clinically. Many injuries thus classified settled in three to four weeks without further sequelae. Conversely, complete ligament tears were under-diagnosed and in particular these tended to be missed in the presence of a haemarthrosis. Examination under anaesthesia is recommended if there is any doubt concerning ligamentous stability and all unexplained h

[Vascular complications in injuries of the knee joint (author's transl)].

Over a total of 1,377 injuries of the knee joint, vascular complications were seen in 28 cases. Lesions of the popliteal artery or its branches are serious; over the total amount already mentioned, it was necessary to amputate in 12 cases; 12 patients presented the Volkmann syndrome. In cases of fractures of the lower end of femur, upper end of tibia and knee dislocations, the importance of early diagnosis of the acute ischemic syndrome is emphasized. Essential of emergency treatment are recalled.

The Cambridge Knee Injury Tool (CamKIT): a clinical prediction tool for acute soft tissue knee injuries.

BACKGROUND/AIM: This study focuses on the development of the Cambridge Knee Injury Tool (CamKIT), a clinical prediction tool developed as a 12-point scoring tool based on a modified e-Delphi study. METHODS: A retrospective cohort evaluation was conducted involving 229 patients presenting to a Major Trauma Centre with acute knee pain over 3 months. The evaluation extracted data on the 12 scoring tool variables as well as diagnostic and management pathway outcomes. CamKIT scores for the injured and non-injured cohorts were then calculated and evaluated. RESULTS: The CamKIT yielded a median sco

Injury patterns to the posteromedial corner of the knee in high-grade multiligament knee injuries: a MRI study.

Injury patterns to the posteromedial corner of the knee have not been previously studied in the context of multiligament knee injuries. We performed a retrospective magnetic resonance imaging and clinical review of a consecutive series of 27 dislocatable knees presenting to a single level-one trauma center from 2005 to 2008. Post-injury magnetic resonance imaging studies were reviewed by two fellowship-trained musculoskeletal radiologists to assess injury patterns to the posteromedial corner. In our series, injury to at least one structure within the posteromedial corner was observed in 81% (2

Prevention and management of knee osteoarthritis and knee cartilage injury in sports.

Articular cartilage defects in the knee of young or active individuals remain a problem in orthopaedic practice. These defects have limited ability to heal and may progress to osteoarthritis. The prevalence of knee osteoarthritis among athletes is higher than in the non-athletic population. The clinical symptoms of osteoarthritis are joint pain, limitation of range of motion and joint stiffness. The diagnosis of osteoarthritis is confirmed by the symptoms and the radiological findings (narrowing joint space, osteophyte formation and subchondral sclerosis). There is no strong correlation betwee

Bilateral Floating Knee Injury-Management of a Complex Injury.

Floating knee is a flail knee joint resulting from fractures of the shafts or adjacent metaphyses of the femur and ipsilateral tibia. Floating knee injuries may include a combination of diaphyseal, metaphyseal, and intra-articular fractures. Floating knee injuries are a group of complex injuries that require a careful assessment. This injury is generally caused by high-energy trauma with often extensive trauma to the soft tissues. There may also be life-threatening injuries to the head, chest, or abdomen and a high incidence of fat embolism. This complex injury has increased in proportion to p
