Comparative effectiveness of lactulose and sennosides for the prevention of peritoneal dialysis-related peritonitis: an open-label, randomized, active-controlled trial.

BACKGROUND: To the best of our knowledge, the effectiveness and safety of lactulose in comparison to sennosides, for the prevention of peritoneal dialysis (PD)-related peritonitis, has never been tested in a randomized study. METHODS: We conducted an open-label, randomized, active-controlled trial in a PD-center in Northern Thailand. Adult patients on PD were enrolled and randomly assigned in a 1:1 ratio into two groups; one group received lactulose 15 mL once daily (n = 50) and the other group received sennosides two tablets daily (n = 50). The primary outcome was time-to-first bacterial per

Non-absorbable disaccharides versus placebo/no intervention and lactulose versus lactitol for the prevention and treatment of hepatic encephalopathy in people with cirrhosis.

BACKGROUND: Non-absorbable disaccharides (lactulose and lactitol) are recommended as first-line treatment for hepatic encephalopathy. The previous (second) version of this review included 10 randomised clinical trials (RCTs) evaluating non-absorbable disaccharides versus placebo/no intervention and eight RCTs evaluating lactulose versus lactitol for people with cirrhosis and hepatic encephalopathy. The review found no evidence to either support or refute the use of the non-absorbable disaccharides and no differences between lactulose versus lactitol. OBJECTIVES: To assess the beneficial and h

Alleviative Effects of Low-Dose Lactulose Combined with Bacillus pumilus NJAUYBN1-7 on Slow Transit Constipation in Loperamide-Induced Mice.

AIMS: Slow transit constipation (STC) is a common functional gastrointestinal disorder characterized by intestinal motility dysfunction, barrier damage, hormonal imbalance and gut microbiota dysbiosis. High-concentration (50%) lactulose is routinely used for STC treatment, while Bacillus pumilus acts as a probiotic to regulate intestinal flora. However, the synergistic efficacy and mechanism of low-dose lactulose combined with Bacillus pumilus NJAUYBN1-7 for STC remain unclear. This study aimed to clarify the therapeutic effects and underlying mechanisms of this combined therapy and compare it

Lactulose Ingestion Induces a Rapid Increase in Genus Bifidobacterium in Healthy Japanese: A Randomised, Double-Blind, Placebo-Controlled Crossover Trial.

Oral administration of a low dose of lactulose increases the abundance of genus Bifidobacterium in the large intestine; however, the details of the daily variation in Bifidobacterium have not been researched. To observe how the intestinal microbiota, including Bifidobacterium, change, especially immediately after the initiation of ingestion, we conducted a randomised, placebo-controlled, double-blind crossover study of ingestion of 4 g lactulose/day for 2 weeks in 36 healthy Japanese (including males and females). The primary outcome was the percentage of Bifidobacterium in the faecal bacteria

Effect of lactulose on growth performance and intestinal morphology of pre-ruminant calves using a milk replacer containing Enterococcus faecium.

The synthetic disaccharide lactulose is known to improve the intestinal microflora by stimulating the growth of selected probiotic bacteria in the gut. In our experiment the effects of lactulose in combination with the probiotic bacteria Enterococcus faecium on growth performance and morphology of the bovine intestine were examined. Calves aged 39 ± 2 days were randomised to three feeding groups (no. = 14 each group): control (L0), fed milk replacer (MR) containing E. faecium; a lactulose group (L1) containing additional 1% lactulose and a second lactulose group (L3) containing 3% lactulose dr

Detection of bacterial overgrowth in IBS using the lactulose H2 breath test: comparison with 14C-D-xylose and healthy controls.

UNLABELLED: OBJECTIVES Recent reports suggest bacterial overgrowth is commonly associated with irritable bowel syndrome (IBS) when diagnosed using the lactulose hydrogen breath test (LHBT). We employed this test to examine whether similar findings exist in a geographically distinct population of Rome II positive IBS patients and compared it to the 14C-D-xylose breath test, a test with acknowledged greater specificity for bacterial overgrowth. METHODS: In the first series, Rome II IBS patients underwent a 10 g lactulose breath test and a standardized 1 g 14C-D-xylose breath test and answered I

Breath testing to evaluate lactose intolerance in irritable bowel syndrome correlates with lactulose testing and may not reflect true lactose malabsorption.

OBJECTIVES: An increased prevalence of lactose intolerance is seen in irritable bowel syndrome (IBS). Recently, we demonstrated a high prevalence of abnormal lactulose breath test results in IBS suggesting bacterial overgrowth. Because symptoms of lactose intolerance result from bacterial fermentation, the purpose of this study was to determine whether an abnormal lactose breath test is reflective of malabsorption or early presentation to bacteria. METHODS: Subjects with diarrhea-predominant IBS were enrolled. On day 1, subjects underwent a lactulose breath test after an overnight fast. Withi

Delayed mouth-caecum transit of a lactulose labelled liquid test meal in patients with steatorrhoea caused by partially treated coeliac disease.

Mouth-caecum transit time (M-CTT) of a lactulose labelled liquid test meal has been measured in 27 coeliac patients and 10 healthy controls using the breath hydrogen technique. Although all patients were urged to maintain a gluten free diet, not all did, and there was, therefore, a wide range in the severity of fat malabsorption within the patient group. Gastric emptying of a 113Indium DTPA-labelled liquid test meal was also assessed in separate studies on six healthy controls and 11 of the coeliac patients. Fasting breath hydrogen concentrations and the response to lactulose, as assessed both

Use Of Quantitative Modelling To Elucidate The Roles Of The Liver, Gut, Kidney, And Muscle In Ammonia Homeostasis And How Lactulose And Rifaximin Alter This Homeostasis.

Humans must eliminate approximately 1M of ammonia per day while maintaining the blood concentration of this potent neurotoxin at a concentration of only about 30 µM. The mechanisms producing such effective ammonia homeostasis are poorly understood by clinicians due to the multiple organs (liver, gut, kidney and muscle) involved in ammonia homeostasis. Based on literature values we present a novel, simplified description of normal and disordered ammonia and the potential mechanisms whereby the drugs used to treat hepatic encephalopathy, lactulose and rifaximin, lower the blood ammonia concentra

Prevalence and Lack of Cross-Sectional Association Between Positive Lactulose Breath Test Findings and Fecal Calprotectin Positivity in Patients with Functional Diarrhea.

Background/Objectives: Functional diarrhea (FDr) is a common gastrointestinal disorder, and alterations in the gut microbiota have been proposed as one of its potential pathophysiological mechanisms. Hydrogen/methane breath tests are commonly used to identify findings suggestive of small intestinal bacterial overgrowth (SIBO) and intestinal methanogen overgrowth (IMO), whereas fecal calprotectin (FC) serves as a marker of intestinal inflammation. However, the prevalence of these findings and their relationship in patients with FDr remain unclear. This study aimed to determine the prevalence of
