The mis-wired language network in children with developmental language disorder: insights from DTI tractography.

This study aims to detect the neural substrate underlying the language impairment in children with developmental language disorder (DLD) using diffusion tensor imaging (DTI) tractography. Deterministic DTI tractography was performed in a group of right-handed children with DLD (N = 17; mean age 10;07 ± 2;01 years) and a typically developing control group matched for age, gender and handedness (N = 22; mean age 11;00 ± 1;11 years) to bilaterally identify the superior longitudinal fascicle, arcuate fascicle, anterior lateral segment and posterior lateral segment (also called dorsal language netw

Stigmatizing Language in Large Language Models for Alcohol and Substance Use Disorders: A Multimodel Evaluation and Prompt Engineering Approach.

OBJECTIVES: Large language models (LLMs) are increasingly used in health care communication but can inadvertently perpetuate stigmatizing language toward individuals with alcohol and substance use disorders. Despite growing interest in LLM performance, a focused evaluation of their propensity for SL and strategies to mitigate it remains lacking. METHODS: We generated 60 clinically relevant questions ["prompts"; 20 each for alcohol use disorder (AUD), alcohol-associated liver disease (ALD), and substance use disorder (SUD)] and tested 14 LLMs. Two physicians independently assessed all response

Comparing English-Language and Chinese-Language Assessment of DSM-5 Personality Disorders and Interpersonal Problems in Chinese Bilingual Speakers.

A review of the literature on Chinese translations of Western self-report personality disorder assessment measures indicates the need to empirically evaluate the validity of assessing Western personality disorder constructs in Chinese language and culture. The current study presents a novel approach to examining this critical question in cross-cultural clinical assessment science and practice. One hundred and ninety-nine (199) Mandarin Chinese and English bilingual participants (92 males and 107 females) provided both English and Chinese self-report ratings on the Inventory of Interpersonal Pr

Language and reading abilities of children with autism spectrum disorders and specific language impairment and their first-degree relatives.

Autism spectrum disorder (ASD) and specific language impairment (SLI) are developmental disorders exhibiting language deficits, but it is unclear whether they arise from similar etiologies. Language impairments have been described in family members of children with ASD and SLI, but few studies have quantified them. In this study, we examined IQ, language, and reading abilities of ASD and SLI children and their first-degree relatives to address whether the language difficulties observed in some children with ASD are familial and to better understand the degree of overlap between these disorders

Examining Risk of Speech-Language Disorders in Children With Cleft Lip.

Although it is well-established that children with cleft palate are at high risk for communication disorders, little is known about the speech-language profiles of children with cleft lip (with or without cleft alveolus), who do not have an overt cleft palate. The purpose of this study was to determine the prevalence of speech-language disorders in a consecutively evaluated group of children with cleft lip, with or without cleft alveolus (CL ± A) from a single cleft lip and palate center in the United States. We also explored the types of speech and language disorders in this cohort and examin

Reading and language disorders: the importance of both quantity and quality.

Reading and language disorders are common childhood conditions that often co-occur with each other and with other neurodevelopmental impairments. There is strong evidence that disorders, such as dyslexia and Specific Language Impairment (SLI), have a genetic basis, but we expect the contributing genetic factors to be complex in nature. To date, only a few genes have been implicated in these traits. Their functional characterization has provided novel insight into the biology of neurodevelopmental disorders. However, the lack of biological markers and clear diagnostic criteria have prevented th

Hearing, language, speech, vestibular, and dentofacial disorders in fetal alcohol syndrome.

Fetal alcohol syndrome (FAS) is characterized by congenital anomalies traditionally associated with hearing disorders. The present study sought to (a) evaluate possible central hearing loss; (b) verify and extend previous observations on sensorineural and conductive hearing losses; (c) evaluate possible vestibular disorders; (d) examine the relationships between hearing, speech, language, vestibular, and dentofacial disorders in FAS patients; and (e) evaluate the influence of patient age, race, and gender on the expression of these morbidities. A biracial group of 22 FAS patients (aged 3 to 26

An examination of language learning disabilities in youth with psychiatric disorders.

This study examined the relationship between psychiatric and language disorders in a sample of youth who were hospitalized at a private psychiatric hospital. Ninety six participants were given a test battery that consisted of tests measuring phonology, syntax, and semantics. The resulting sample scored in the average range on IQ measures and in the low-average to below-average range on written language abilities tests. Over 33% of the participants qualified for the diagnosis of a language learning disability (LLD); however, only 44% of those participants who qualified as LLD were receiving spe

Mismatch Negativity and Auditory Brain Stem Response in Children with Autism Spectrum Disorders and Language Disorders.

OBJECTIVE: Language disorders (LD) in autism spectrum disorders (ASD) are highly variable and has a severe impact on the level of functioning in autistic children. Early diagnosis of these language disorders is essential for early interventions for children at risk. The electrophysiological measurements are considered valuable tools for determining language disabilities in children with ASD. This study aimed to study and compare ABR and MMN in autistic children with language disorders. METHODS: This study included a group of typically developing children and a group of children diagnosed with

Speech and Language Markers of Bipolar Disorder: Challenges and Opportunities.

BACKGROUND: Clinicians aspire to predict the emergence of Bipolar Disorder (BD) in a timely manner. To accomplish this, markers reflecting mental states that can be gathered non-invasively and at large scale are needed. Here, we systematically evaluate evidence relating speech-based markers to mood states in BD. METHODS: We searched Medline and Google Scholar for all published studies in English up to February 2026 on the use of speech markers in BD. We undertook thematic analysis on abstracts using topic modeling and a qualitative gap analysis to identify potential opportunities for future r

Progressive Supranuclear Palsy with Predominant Speech/Language Disorder.

We herein report a rare case of progressive supranuclear palsy (PSP) that initially presented as progressive fluent aphasia and echolalia. A 75-year-old woman exhibited significant language impairments for 2 years before developing motor symptoms, including vertical gaze palsy and frequent falls. Neuroimaging revealed asymmetric frontal and midbrain atrophy, with reduced perfusion in the left frontal and parietal lobes. This case highlights the importance of recognizing atypical PSP presentations, particularly language-dominant variants that may mimic other neurodegenerative disorders. Early i

Bridging the Divide: Brain and Behavior in Developmental Language Disorder.

Developmental language disorder (DLD) is a heterogenous neurodevelopmental disorder that affects a child's ability to comprehend and/or produce spoken and/or written language, yet it cannot be attributed to hearing loss or overt neurological damage. It is widely believed that some combination of genetic, biological, and environmental factors influences brain and language development in this population, but it has been difficult to bridge theoretical accounts of DLD with neuroimaging findings, due to heterogeneity in language impairment profiles across individuals and inconsistent neuroimaging

Speech and language deficits are central to SETBP1 haploinsufficiency disorder.

Expressive communication impairment is associated with haploinsufficiency of SETBP1, as reported in small case series. Heterozygous pathogenic loss-of-function (LoF) variants in SETBP1 have also been identified in independent cohorts ascertained for childhood apraxia of speech (CAS), warranting further investigation of the roles of this gene in speech development. Thirty-one participants (12 males, aged 0; 8-23; 2 years, 28 with pathogenic SETBP1 LoF variants, 3 with 18q12.3 deletions) were assessed for speech, language and literacy abilities. Broader development was examined with standardised

How We Fail Children With Developmental Language Disorder.

Purpose For over two decades, we have known that children with developmental language disorder (DLD) are underserved. We have also known that DLD does not attract the research attention that it merits given its prevalence and impact. The purposes of this clinical focus article are to present evidence that these failures continue, explore the reasons behind these failures, and propose solutions. Method I reviewed the literature and applied bibliometric analysis procedures from Bishop (2010) to quantify research efforts aimed at DLD compared to other neurodevelopmental disorders. Results The per

Dorsal language stream anomalies in an inherited speech disorder.

Speech disorders are highly prevalent in the preschool years, but frequently resolve. The neurobiological basis of the most persistent and severe form, apraxia of speech, remains elusive. Current neuroanatomical models of speech processing in adults propose two parallel streams. The dorsal stream is involved in sound to motor speech transformations, while the ventral stream supports sound/letter to meaning. Data-driven theories on the role of these streams during atypical speech and language development are lacking. Here we provide comprehensive behavioural and neuroimaging data on a large nov

Grey matter volume in developmental speech and language disorder.

Developmental language disorder (DLD) and developmental speech disorder (DSD) are common, yet their etiologies are not well understood. Atypical volume of the inferior and posterior language regions and striatum have been reported in DLD; however, variability in both methodology and study findings limits interpretations. Imaging research within DSD, on the other hand, is scarce. The present study compared grey matter volume in children with DLD, DSD, and typically developing speech and language. Compared to typically developing controls, children with DLD had larger volume in the right cerebel
