Primary tuberculosis of the larynx.

INTRODUCTION: Tuberculosis is a chronic bacterial infection caused by Mycobacterium tuberculosis, usually affecting the lung, but very rare cases of isolated laryngeal tuberculosis have been reported. PURPOSE: To demonstrate the importance of considering the diagnosis of this disease, which presents with very variable and sometimes misleading clinical and endoscopic features, but for which medical treatment is very effective. MATERIAL AND METHODS: This retrospective study was based on ten cases of laryngeal tuberculosis managed between January 2004 and December 2009. The objective of this st

[Reinnervation and neurostimulation of the larynx].

Dynamic rehabilitation of vocal fold paralysis (VFP) should receive more emphasis in the future. In unilateral immobility with signs of atrophy and wide glottal gap, non-selective reinnervation with ansa cervicalis may become an alternative to augmentation and thyroplasty. For bilateral VFP progress has been made in the concepts of selective reinnervation and neurostimulation (pacing). These new therapies have the potential to restore near normal respiration-without compromising the voice quality-and may contribute to the development of larynx transplantation surgery.

[Larynx: implants and stents].

There is a wide variety of devices and materials to be implanted into the human larynx. Some are intended to remain only for a period of time, like laryngeal stents. If removal is not intended the device meets the definition for a medical implant. The majority of implants is used for the treatment of unilateral vocal fold immobility. There a 2 types of implants serving this purpose: Implants in a stricter sense are devices of solid material, which are brought into the paraglottic space through a window in the laryngeal framework (medialization thyroplasty). Several different products are prese

A complete transection of larynx.

Acute laryngeal injury is a rare, potentially lethal injury. It often poses difficulty in airway management. Commonly described are external laryngeal or blunt injuries. We report a case of penetrating neck injury resulting in complete transection of cricoid cartilage. A lady presented with a large, deep slash wound in her neck from a knife injury. There was complete laryngotracheal separation. The airway was secured before she was taken to the operating theatre. A tracheostomy was performed. Exploration revealed the cricoid ring was cut horizontally into two. Anastomosis of the cartilaginous

Congenital anomalies of the larynx.

Congenital laryngeal anomalies are relatively rare. However, they may present with life-threatening respiratory problems in the newborn period. Associated problems with phonation and swallowing may prevent a baby from thriving. Stridor is the most common presenting symptom of congenital laryngeal abnormalities. Often, it is associated with dysphagia, aspiration, and failure to thrive. Endoscopy is essential for evaluation and diagnosis in most cases. The differential diagnosis includes laryngeal cysts, atresia and stenosis, vocal fold immobility, and subglottic hemangiomas. In this article, th

Extramedullary plasmacytoma of the larynx.

Extramedullary plasmacytoma (EMP) is a rare plasma cell neoplasm that occurs in the soft tissues, especially in the upper airways. We report two cases of laryngeal EMP, both of which were treated by surgical resection, one with and one without combination radiotherapy. A 39-year-old male was diagnosed with a laryngeal tumor localized in the left arytenoid region in October 1999. Biopsy showed the tumor to be a plasmacytoma. He underwent a direct laryngoscopy and resection of the tumor with a KTP laser. Consequently, he received radiotherapy to a total dose of 60 Gy. He has remained clear, with

Cartilaginous tumours of the larynx.

Cartilaginous tumours of the larynx are rare. Chondrosarcomas represent the most common sarcoma involving this site. A series of seven patients with the diagnosis of chondrosarcoma of the larynx from the cancer registry at the University of Michigan Medical Center from 1975 to 1997 and one recent case from the Ann Arbor Department of Veterans Affairs Hospital were identified. One patient with a laryngeal chondroma was also included in the series. Pertinent clinical, pathologic, and therapeutic data were reviewed. In three of the nine patients, tumours involved the thyroid cartilage; the remain

The larynx and neonatal apneas.

OBJECTIVE: We sought to review the current literature on the role of the larynx in neonatal apneas. STUDY DESIGN: We conducted a literature search and analysis of the published and other available information on the topic. RESULTS: Publications on this topic in otolaryngology journals are rare, although more basic sciences-oriented journals have published interesting results from animal studies during the past 20 years. The scarce data on human neonates are also presented. CONCLUSIONS: The authors follow the phylogenetic and oncogenetic pathways to explain the active role of the larynx in n

Human papillomavirus in larynx.

OBJECTIVES: The core of the present clinical and basic research knowledge of laryngeal human papillomavirus (HPV) infection is described. STUDY DESIGN: Review. METHODS: A computer-aided search of MEDLINE database supplemented by hand searches of key journals was conducted. RESULTS: One of the tumor-promoting factors in the larynx is the HPV found both in normal laryngeal epithelium and in laryngeal tumors. The most important manifestation of laryngeal HPV infection is laryngeal papillomatosis, a rare disease caused by HPV types 6 and 11. In laryngeal carcinogenesis, the role of HPV remains

Rupture of the larynx in a newborn.

BACKGROUND: Lesions of the laryngotrachea due to repeated intubation trauma after birth are reported. CASE: A macrosomic neonate was born at term after uneventful pregnancy. On the basis of diminished uterine contractions and arrest of descent, the head was delivered by using obstetric forceps. No intubation was attempted after birth. Ten hours later, the infant developed acute life-threatening respiratory distress and subcutaneous emphysema. Intubation was impossible. Open surgical exploration of the neck revealed a rupture of the cricothyroid ligament within the larynx. The defect was close

Lymphoepithelial carcinoma of the larynx.

Lymphoepithelial carcinoma is a very rare tumour of the larynx, with an exhaustive review of the literature having disclosed only 33 documented cases. The relationship between lymphoepithelial carcinoma of the larynx and Epstein-Barr virus is still controversial. We describe one new case of this tumour involving the supraglottis. The patient was treated with supraglottic laryngectomy and left modified neck dissection. Three years and 4 months later, the right side of the neck was found positive for metastatic disease and a right modified neck dissection was performed. No evidence of disease wa
