PURPOSE: The literature on postlaryngectomy voice and speech rehabilitation is long-standing. Although multiple rehabilitation options have existed over the years, the acquisition and use of esophageal speech (ES) has decreased significantly over the past 40 years. This reduction coincides with the increased application of tracheoesophageal puncture (TEP) voice restoration. The literature suggests that voice acquisition failures observed secondary to TEP may represent a similar phenomenon that led to ES acquisition failures.

In 1980, Singer and Blom published the results of their study on use of the tracheoesophageal puncture prosthesis for restoration of voice after total laryngectomy. Since then, the placement of tracheoesophageal puncture prostheses has been an integral part of rehabilitation after laryngectomy. Complications of this procedure have been recognized and are usually minimal. Inadvertent aspiration of the prosthesis is rare. Usually, patients seek help immediately after the incident. We report a case of unnoticed aspiration of a Blom-Singer prosthesis in a patient with a laryngectomy.

OBJECTIVES: Tracheoesophageal puncture (TEP) is considered the gold standard for voice rehabilitation after total laryngectomy. One of the main causes of treatment failure, and a potentially serious complication, is the TEP enlargement and/or leakage around the voice prosthesis. Speech restoration after total laryngectomy may be achieved in different ways, the gold standard being tracheoesophageal puncture (TPE) with the positioning of a speech prosthesis. TPE is not immune to complications, the most common of which being leakage through or around the prosthesis. When dealing with an enlarged tracheoesophageal fistula, the management can be either conservative or surgical. In the following case report, we present a particularly challenging case, in which every conservative strategies failed and a major surgery was required to close the fistula.
