Gelastic seizures due to right temporal cortical dysplasia.

Gelastic seizures are an uncommon seizure type. They are most frequently observed in patients with hypothalamic hamartoma. Their association with other types of cerebral lesions is rare. Depending on the location of the lesion, gelastic seizures may or may not be accompanied by a subjective feeling of mirth. The pathophysiological mechanisms of this type of seizure are still undefined, and little is known about which pathways promote laughter and its emotional content, mirth. We present a young man with drug-resistant, gelastic seizures due to focal cortical dysplasia of the right inferior tem

[Precocious puberty and laugh attacks].

A 7 year old girl presented with precocious puberty and ictal laughter. Brief, repetitive, stereotyped attacks of laughter were the first manifestation of the epileptic syndrome. Stages of puberty were noted as B III-IV and PH II according to Tanner. X-rays showed a bone age of 11 years and the weight increased from the 50th beyond 97th percentile. Plasma concentrations of LH, FSH, testosterone and Ostradiol were elevated. The CT scan was normal and a magnetic resonance imaging showed an hypothalamic hamartoma. The control of the seizures control and social adjustment were poor. MR scanning 3

The Psychology of Patients With Oral-Facial Clefts: A Review of Victor Hugo's The Man Who Laughs ( L'homme qui rit ).

In Victor Hugo's The Man Who Laughs , the protagonist has a facial cleft. The authors aimed to comprehend the psychological experience of a patient with a facial cleft by analyzing this novel. The novel concerns the life of a young nobleman, Gwynplaine, disfigured as a child, who travels with his protector and companion, the vagabond philosopher Ursus, and Dea, a baby girl he rescued. During the oppressive rule of the king, one of his adversaries was Lord Linnaeus Clancharlie, who had taken refuge in Switzerland. The tyrannical monarch ordered the execution of Gwynplaine's father and commissio

[Hypothalamic hamartoma dissection using focused ultrasound under MRI control. The first successful experience in Russia].

Treatment of motor disorders by MRI-guided focused ultrasound is an alternative to neuro- and radiosurgery such as stereotactic radiofrequency ablation and thalamotomy with a gamma knife. However, safety, efficacy and feasibility of this technology for intracranial neoplasms are still unclear. The authors report successful hypothalamic hamartoma dissection by MRI-guided focused ultrasound in a 32-year-old woman with drug-resistant gelastic epilepsy and violent laughter and crying attacks. Magnetic resonance imaging revealed type II hypothalamic hamartoma. The last one was detached from surroun

The distinguishing motor features of cataplexy: a study from video-recorded attacks.

STUDY OBJECTIVES: To describe the motor pattern of cataplexy and to determine its phenomenological differences from pseudocataplexy in the differential diagnosis of episodic falls. METHODS: We selected 30 video-recorded cataplexy and 21 pseudocataplexy attacks in 17 and 10 patients evaluated for suspected narcolepsy and with final diagnosis of narcolepsy type 1 and conversion disorder, respectively, together with self-reported attacks features, and asked expert neurologists to blindly evaluate the motor features of the attacks. Video documented and self-reported attack features of cataplexy a

Cataplexy leading to the diagnosis of Niemann-Pick disease type C.

Cataplexy in childhood is a rare and often misdiagnosed symptom. It is described as a brief episode of bilateral loss of muscle tone with intact consciousness, triggered by a variety of strong emotions and in particular with unexpected laughter. This report presents a 9-year old male with progressive cerebellar and pyramidal symptoms and a cognitive decline since the age of 4. His recently developed "drop attacks" on laughter were recognized as cataplexy and led to the diagnosis of Niemann-Pick type C disease. With biochemical studies this diagnosis, a lysosomal storage disease, was confirmed.

Atomoxetine (Strattera)-Induced Pathologic Laughing in a Patient With Pontine Hemorrhage: A Case Report.

OBJECTIVES: Pathologic laughing, characterized by episodes of abrupt and inappropriate laughter occurring irrespective of a person's emotional feelings, has been reported in patients with neurologic deficits. Some pharmacotherapies, including selective serotonin reuptake inhibitors, are effective in alleviating pathologic laughing. However, contrary to previous reports, we report a case of pathologic laughing that developed after a patient with pontine hemorrhage was administered atomoxetine (Strattera). CASE PRESENTATION: A 55-year-old man was diagnosed with acute intracerebral hemorrhage in

Fluctuation of primary motor cortex excitability during cataplexy in narcolepsy.

OBJECTIVE: Cataplexy is a complicated and dynamic process in narcolepsy type 1 (NT1) patients. This study aimed to clarify the distinct stages during a cataplectic attack and identify the changes of the primary motor cortex (PMC) excitability during these stages. METHODS: Thirty-five patients with NT1 and 29 healthy controls were recruited to this study. Cataplectic stages were distinguished from a cataplectic attack by video-polysomnogram monitoring. Transcranial magnetic stimulation motor-evoked potential (TMS-MEP) was performed to measure the excitability of PMC during quiet wakefulness, l

Humour in clinical-educational interactions between graduate student clinicians and people with aphasia.

BACKGROUND: During clinical interactions, clinicians and people with aphasia (PWA) use humour and laughter for a range of purposes, most of which contribute to friendly interactions in which the participants appear to develop a positive regard for one another. Moreover, humour is a vital component of facework, or the processes interactants engage in to protect their own and one another's well-respected, public personas. AIMS: To examine the ways in which speech-language pathology graduate student clinicians enlist humour during one-on-one therapy sessions for PWA. METHODS & PROCEDURES: Three

Psychiatric features in NMDAR and LGI1 antibody-associated autoimmune encephalitis.

Patients with autoimmune encephalitis (AE) often developed psychiatric features during the disease course. Many studies focused on the psychiatric characteristic in anti-NMDAR encephalitis (NMDAR-E), but anti-LGI1 encephalitis (LGI1-E) had received less attention regarding the analysis of psychiatric features, and no study compared psychiatric characteristic between these two groups. The clinical data of AE patients (62 NMDAR-E and 20 LGI1-E) who developed psychiatric symptoms were analyzed in this study. In NMDAR-E, the most common higher-level feature was "behavior changes" (60/62, 96.8%) an

Motor and emotional facial features in parietal lobe seizures: an ictal functional connectivity study using SEEG.

OBJECTIVES: Parietal lobe epilepsy is one of the rarest types and patients with this form of epilepsy report multiple subjective symptoms during ictal manifestation. Specific facial coupling of emotion and motor symptoms may take various forms, such as pouting and disgust or smiling. We aimed to highlight brain structures and the network involved during ictal grimacing in parietal lobe seizures. METHODS: In this study, we report two patients with drug-resistant epilepsy, with seizure onset located in the inferior parietal lobule and a semiology characterized by ictal grimacing. Patients were

Neural correlates and network dynamics of uncommon semiologies in the cingulo-insulo-opercular network explored with Stereo-EEG.

OBJECTIVE: We investigated five uncommon semiologies associated with suspected cingulo-insulo-opercular network involvement using stereo-electroencephalography (SEEG): smiling, laughter, neck choreoathetosis, face wiping, and arm-over-head pose. METHODS: Among 75 SEEG cases reviewed between February 2023 and August 2024, 11 patients exhibited at least one semiology of interest. Video-annotated peri-onset epochs were analyzed using an in-house MATLAB-based pipeline with band-specific filtering and event-related desynchronization/synchronization (ERDS) analysis across six frequency bands. Emotional maltreatment is associated with atypical responding to stimulation of endogenous oxytocin release through mechanically-delivered massage in males.

The neuropeptide oxytocin plays an important role in social behavior, parenting, and affectionate touch and there is some evidence that oxytocin release can be stimulated by massage or affectionate touch. We examined the effects of massage applied by a massage seat cover on salivary oxytocin levels in two exploratory studies using within-subject designs. In Study 1 massage effects on oxytocin levels were examined in a sample of N=20 healthy female participants. Effects of a 15-min massage session were compared to a control condition during which participants sat on a comfortable chair without

Three physiological responses in fathers and non-fathers' to vocalizations of typically developing infants and infants with Autism Spectrum Disorder.

Children with ASD, even before receiving a formal diagnosis, express atypical patterns of distress vocalizations (namely, episodes of crying). Their cries have higher fundamental frequencies, shorter inter-bout pauses, and fewer utterances. Cries of children with ASD are also perceived differently from other cries, and these perceptual differences may alter parent-infant interaction. This study assessed multiple physiological responses in fathers and non-fathers to atypical distress vocalizations (cries of children with ASD), acoustically matched typical distress vocalizations (cries of typica

Unilateral cataplexy associated with systemic lupus erythematosus.

A patient with systemic lupus erythematosus (SLE) developed attacks of unilateral cataplexy precipitated by laughter. Unilateral cataplexy has not been described previously in detail and its association with SLE is unique. The clinical details, investigations, and diagnostic criteria are discussed and a causal relationship between cataplexy and SLE is suggested.

Refractory Temporal Gelastic Seizure: A Case Report.

Gelastic seizures are rare epileptic events characterized by sudden, unprovoked bursts of laughter that are typically associated with hypothalamic hamartomas but may also arise from cortical epileptogenic foci. We report the case of an eight-year-old boy born prematurely at 28 weeks' gestation who presented with a two-year history of recurrent hypermotor seizures, loss of consciousness, behavioral arrest, falls, and frequent episodes of inappropriate laughter. Video electroencephalography demonstrated epileptiform activity consistent with left temporal lobe epilepsy with mild diffuse encephalo
