Scaling Up HIV Self-Testing in Africa: Insights From National Programmatic Data in Eight Countries.

INTRODUCTION: Evidence from routine, national programme data on HIV self-testing (HIVST) scale-up is limited. This study examines HIVST scale-up in eight African countries, describing how HIVST has been integrated into testing strategies and how testing coverage, test positivity, and linkage to antiretroviral therapy (ART) have evolved. METHODS: We conducted a retrospective descriptive analysis of national programme data from January 2019 to December 2023 across Kenya, Lesotho, Malawi, Mozambique, South Africa, Tanzania, Uganda and Zimbabwe. Data were disaggregated by quarter and subnational

HIV-related discrimination among grade six students in nine Southern African countries.

BACKGROUND: HIV-related stigmatisation and discrimination by young children towards their peers have important consequences at the individual level and for our response to the epidemic, yet research on this area is limited. METHODS: We used nationally representative data to examine discrimination of HIV-positive children by grade six students (n = 39,664) across nine countries in Southern Africa: Botswana, Lesotho, Malawi, Mozambique, Namibia, South Africa, Swaziland, Zambia and Zimbabwe. Descriptive statistics are used to compare discrimination by country, gender, geographic location and soc

Concurrent coverage and determinants of vitamin A supplementation and deworming among children aged 12-59 months in 15 Sub-Saharan African countries.

BACKGROUND: Vitamin A supplementation (VAS) and deworming (DW) are proven, cost-effective interventions that protect children against preventable morbidity, mortality, and the burden of micronutrient deficiencies and parasitic infections. Nevertheless, many children fail to receive both interventions simultaneously, limiting the potential health gains. Assessing co-coverage and its determinants is crucial for guiding integrated child health strategies and closing persistent gaps across Sub-Saharan Africa. METHODS: We analyzed DHS data from 15 Sub-Saharan African countries, including 107,725 c

Hormonal contraception and HIV prevalence in four African countries.

BACKGROUND: The HIV seroprevalence among women aged 15-24 years was compared according to their pattern of contraceptive use in four African countries: Kenya, Lesotho, Malawi and Zimbabwe. STUDY DESIGN: Data were derived from Demographic and Health Surveys (DHS) conducted between 2003 and 2006 on representative samples, totaling 4549 women. RESULTS: It is indicated that users of depo-medroxyprogesterone acetate (DMPA) have a significantly higher seroprevalence than nonusers [odds ratio (OR)=1.82, 95% CI=1.63-2.03] and higher than users of oral contraceptives and users of traditional methods.

Parametric Bayesian modelling of tuberculosis mortality determinants and facility level heterogeneity effect using Gamma and Gaussian shared frailty techniques.

BACKGROUND: In a normal regression analysis for determinants of tuberculosis (TB) outcomes, assumptions that the sample is homogenous are made. However, the model does not account for the overall effect of unobserved or unmeasured covariates. Frailty may exacerbate the effects of TB on patients and raise their risk of death in Lesotho. This study aims to quantify the amount of heterogeneity that exists at the facility level, and to ascertain the determinants of TB mortality across all the catchment areas in Lesotho. METHODS: This was a retrospective cohort of patients on TB treatment register

Analysis of tuberculosis prevalence surveys in Africa: evidence to change the status quo in case finding.

INTRODUCTION: Lesotho has among the world's highest TB prevalences and the national program finding only 50% of incident cases. Lesotho underwent its first TB prevalence study in 2019 in reviewing this study, the authors recognized that the standard measures for screening and diagnosis TB often failed to detect the presence of culture positive pulmonary TB. This review seeks to determine if the systematic under detection of culture positive TB by standard methods is true among prevalence surveys in Africa. METHODS: We conducted a review to identify TB prevalence surveys conducted in Africa fr

SESOTHO trial ("Switch Either near Suppression Or THOusand") - switch to second-line versus WHO-guided standard of care for unsuppressed patients on first-line ART with viremia below 1000 copies/mL: protocol of a multicenter, parallel-group, open-label, randomized clinical trial in Lesotho, Southern Africa.

BACKGROUND: The World Health Organization (WHO) recommends viral load (VL) measurement as the preferred monitoring strategy for HIV-infected individuals on antiretroviral therapy (ART) in resource-limited settings. The new WHO guidelines 2016 continue to define virologic failure as two consecutive VL ≥1000 copies/mL (at least 3 months apart) despite good adherence, triggering switch to second-line therapy. However, the threshold of 1000 copies/mL for defining virologic failure is based on low-quality evidence. Observational studies have shown that individuals with low-level viremia (measurable

In vitro Antigonorrhea Activity of the Aerial Part of Asparagus suaveolens n-Hexane Fraction and Palmitone as a Bioactive Compound.

Asparagus suaveolens is a medicinal plant used in Lesotho and South Africa to treat epilepsy and gonorrhea. The current investigation identifies the compound responsible for the antiepileptic and antimicrobial properties as palmitone which showed antigonorrhea activities against WHO 2008 Neisseria gonorrhea F and 0 strains more than the standard used, gentamicin. These results support the traditional use of A. suaveolens for treatment of gonorrhea and epilepsy since palmitone is known as an anticonvulsant agent. This is the first study indicating the presence of palmitone in the Asparagus genu

Ticks and tick-borne pathogens of domestic ruminants in southern Africa: A systematic review of diversity, distribution, and epidemiology.

The resurgence of tick-borne diseases remains a global concern, hindering livestock production across southern Africa and resulting in significant economic losses and threatening the livelihoods of small-scale farmers through reduced yields, higher treatment costs, and increased livestock mortality. This review synthesizes evidence on the genetic characterization, distribution, and epidemiology of ticks and tick-borne pathogens affecting domestic ruminants (cattle, goats, and sheep) in southern Africa. A search for peer-reviewed articles published between 2016 and 2025 was conducted across thr

Associations Between Mobility, Food Insecurity, and Transactional Sex Among Women in Cohabitating Partnerships: An Analysis From 6 African Countries 2016-2017.

BACKGROUND: Mobile women are at risk of HIV infection in sub-Saharan Africa, although we lack evidence for HIV risk among women in mobile partnerships, especially in the context of household food insecurity, a growing concern in the region. SETTING: Women aged 15-59 years with a cohabitating male partner who participated in population-based HIV impact assessment surveys in Eswatini, Lesotho, Namibia, Tanzania, Uganda, and Zambia. METHODS: We evaluated the association between women's and their partner's mobility (being away from home for more than 1 month or staying elsewhere) and transaction

Cross-country inequalities and frontier analysis in global burden of cervical cancer: cross-sectional studies.

OBJECTIVE: This research evaluated global health disparities of cervical cancer, and forecasted trends from 2022 to 2040. METHODS: Cross-country inequalities [Slope Index of Inequality (SII) and the Concentration Index] are used to measure inequality. Frontier analysis evaluates the medical quality by calculating the difference between actual values and frontier values. Average annual percentage changes (AAPCs) were computed to discern trends from 2022 to 2040. RESULTS: The global SII for the age-standardized death rate (ASDR) of cervical cancer was -9.14 per 100,000 in 1990 and -9.62 in 202

HIV incidence among pregnant and postpartum women in a high prevalence setting.

In sub-Saharan Africa, most women who test HIV negative at the first antenatal care encounter are rarely tested again during pregnancy and postpartum, yet data suggests that pregnancy is associated with increased risk of HIV acquisition compared to non-pregnant women. We describe HIV incidence during pregnancy and postpartum in Lesotho, a high prevalence setting, and factors associated with HIV seroconversion. We enrolled a cohort of HIV negative women presenting at health facilities for antenatal care and followed them through delivery up to 24 months postpartum. Women were repeatedly tested

[Eye trauma in southern Africa].

The population of southern Africa living in South Africa, Swaziland, Lesotho and Mozambique is 50 million. Because of intertribal hostility, the particular family structure, and the widespread use of alcohol and cannabinoid drugs, violence is very common. Trauma results mainly from "cold" weapons, which are an essential part of the traditional accoutrements of the African male. 202 patients (31.86% of those who underwent eye surgery in 2 years) needed surgery for severe ocular trauma in Mbabane, Swaziland. In a similar period, 100 patients (19.49%) required similar surgery in Boksburg, South A

[Amebic liver abscesses developing during R-CHOP chemotherapy in a patient with mantle cell lymphoma].

A 51-year-old man was diagnosed with stage IV mantle cell lymphoma based on terminal ileum biopsy and treated with the R-CHOP regimen. Abdominal CT to assess continuous fever after three courses of R-CHOP revealed three low-density areas in the liver. PCR of the fluid obtained by percutaneous drainage revealed Entamoeba histolytica positivity, although the cultures were negative. Metronidazole treatment achieved cure. The patient was not a homosexual but had an 8-month stay in Lesotho 21 years ago, leading to the possibility that E. histolytica infection at the time continued as an asymptomati
