Feeding problems, sleep disturbances, and negative behaviors in a toddler.

Tiffany, a 3-year-old girl, was referred to the developmental and behavioral pediatrics service for evaluation of significant and persistent negative behaviors associated with refusal to eat at meal time and constant snacking during the past 3 months. She lost 2 pounds, but her weight for her height was at the 50th percentile. Her mother indicated that Tiffany had frequent night awakenings (>10) and late sleep onset (between 12:00 and 1:00 a.m.). Her mother described her as being "easily frustrated," getting upset and angry very quickly. Tiffany was identified at an early intervention program

An ethnography of chronic pain management in primary care: The social organization of physicians' work in the midst of the opioid crisis.

BACKGROUND: This study reports on physicians' experiences with chronic pain management. For over a decade prescription opioids have been a primary treatment for chronic pain in North America. However, the current opioid epidemic has complicated long-standing practices for chronic pain management which historically involved prescribing pain medication. Caring for patients with chronic pain occurs within a context in which a growing proportion of patients suffer from chronic rather than acute conditions alongside rising social inequities. METHODS: Our team undertook an ethnographic approach kno

Relating personnel costs in special care units and in traditional care units to resident characteristics.

BACKGROUND: There are over 16 000 nursing homes in the United States (US), among which approximately 70% of residents are cognitively impaired. Reflecting this, approximately 20% of US nursing homes maintain Special Dementia Care Units (SCUs). SCUs supposedly provide more staff time and more specialized staff assignments to residents than do traditional care units. AIMS OF THE STUDY: This paper addresses the issues of staff time and assignment: do the costs of personal care inputs differ according to whether they are provided by SCUs or in traditional care settings? Related to this, are differ

[Palliative care in the light of legal and regulatory requirements in Germany].

In Germany, palliative care has developed rapidly since the establishment of the first palliative care unit in 1983. More improvements in patient-centered care are only possible if legal requirements as well as education of physicians, nurses, and other professionals involved in palliative care are adapted to current needs. This paper provides an overview of legal and regulatory requirements including a critical appreciation of their influence on palliative care in Germany. Only recently has medical education in palliative medicine been implemented as an integral part of medical studies at Ger

Comparison of an immunochromatographic method for rapid identification of group A streptococcal antigen with culture method.

OBJECTIVES: To compare the sensitivity and specificity of Concise Strep A (Hybritech, San Diego, Calif), an immunochromatographic group A streptococcal rapid antigen detection system, with a two-plate culture method for the diagnosis of streptococcal pharyngitis, and to evaluate the need for routine back-up culture when this rapid test is used. DESIGN: Throat cultures were obtained from 351 children with acute pharyngitis by duplicate rayon-tipped swabs held in parallel and vigorously rubbed against both tonsils and the posterior pharyngeal wall. One swab was tested for group A streptococcal

Clinician Perceptions of Therapeutic Inertia in Type 2 Diabetes Mellitus: A Qualitative Study.

BACKGROUND: Therapeutic inertia (TI) in type 2 diabetes mellitus (T2DM) is defined as the lack of timely adjustment to therapy when a patient's treatment goals are not met. Failure to initiate or intensify evidence-based therapies for T2DM increases the risk of micro and macrovascular disease complications. There are limited data regarding clinicians' (nurse practitioners and pharmacists) perceptions of TI and patient-perceived barriers to therapeutic progress in T2DM. A better understanding of perceptions of TI might assist in identifying targeted interventions to limit its occurrence. The ob

Case reports of acupuncturists and massage therapists at Mayo Clinic: New allies in expediting patient diagnoses.

BACKGROUND: Massage therapists (MTs) and licensed acupuncturists (LAcs) are increasingly embedded within interdisciplinary care teams in both inpatient and outpatient settings. While not traditionally viewed as diagnostic providers, their extended, hands-on interactions with patients enable them to observe subtle physical and behavioral changes that may support early clinical recognition of underlying health issues. OBJECTIVE: To present a case series in which MTs and LAcs identified abnormal clinical findings during routine care, prompting timely referrals that led to new or expedited diagno

Injected with controversy: sales and administration of injectable contraceptives in drug shops in Uganda.

CONTEXT: Informal drug shops are the first line of health care in many poor countries. In Uganda, these facilities commonly sell and administer the injectable contraceptive depot medroxyprogesterone acetate (DMPA), even though they are prohibited by law from selling any injectable drugs. It is important to understand drug shop operators' current practices and their potential to provide DMPA to hard-to-reach populations. METHODS: Between November 2007 and January 2008, 157 drug shops were identified in three rural districts of Uganda, and the operators of the 124 facilities that sold DMPA were

Micronutrient use in critical care: Survey of clinical practice.

BACKGROUND & AIMS: Micronutrients, principally vitamins and minerals, play an important role both in health and in disease. Parenteral micronutrient products are commonly prescribed for critically ill patients both in line with the terms of the product's license, and for other indications where there is an underpinning physiological rationale, or precedent, for their use but little evidence. This survey sought to understand United Kingdom (UK) prescribing practice in this area. METHODS: A 12-question survey was circulated to healthcare professionals working in UK critical care units. Key Outcomes for Evaluating Hand and Wrist Scars: A Nationwide Survey of Clinicians in Saudi Arabia.

Background and Objectives: Hand and wrist scars alter physical appearance and can result in functional impairments and psychosocial difficulties. Although these effects are clinically important, rehabilitation services in Saudi Arabia lack consistent and standardised scar assessment protocols. The limited use of validated outcome measures hinders both clinical practice and research. Standardised scar assessment is essential for evidence synthesis, developing new scar care interventions and promoting best outcomes. We aim to investigate healthcare professionals' perspectives on key scar outcome

A Brief History of Human Challenge Studies (1900-2021) Emphasising the Virology, Regulatory and Ethical Requirements, Raison D'etre, Ethnography, Selection of Volunteers and Unit Design.

Venetian quarantine 400 years ago was an important public health measure. Since 1900 this has been refined to include "challenge" or deliberate infection with pathogens be they viruses, bacteria, or parasites. Our focus is virology and ranges from the early experiments in Cuba with Yellow Fever Virus to the most widespread pathogen of our current times, COVID-19. The latter has so far caused over four million deaths worldwide and 190 million cases of the disease. Quarantine and challenge were also used to investigate the Spanish Influenza of 1918 which caused over 100 million deaths. We consid

Exploring clinical reasoning in physical and occupational therapists treating frail older adults in skilled nursing facilities: a Delphi study.

Frailty is highly prevalent in skilled nursing facilities (SNFs), yet little is known about how physical and occupational therapists (PTs/OTs) apply clinical reasoning (CR) when designing and progressing EPs for frail older adults. This study used a three-round Delphi method (DM) to generate expert consensus on the reasoning strategies therapists employ in SNF practice. Licensed PTs and OTs from the USA and Canada were recruited through professional directories and completed iterative surveys focused on assessment, goal-setting and intervention adaptation. Round 1 consisted of open-ended quest

Primary care provider attitudes regarding physical therapists addressing opioid use and opioid misuse: a qualitative study.

IMPORTANCE: Primary care providers (PCPs) manage musculoskeletal pain and may prescribe opioids for their patients, presenting risk for opioid misuse. Physical therapists are well-positioned to collaborate with PCPs in identifying and mitigating opioid risk and misuse for patients that PCPs and physical therapists co-manage. How PCPs view such collaboration is unclear. OBJECTIVE: The objective of this study was to explore PCPs' attitudes regarding physical therapists' role in identifying and mitigating opioid risk and opioid misuse. DESIGN: This was a qualitative study using rapid content an

Stigma and barriers to seeking mental health care among emergency department providers - A mixed methods study.

High levels of psychological distress and depression are common among clinicians working in US emergency departments [ED], yet treatment-seeking remains rare, contributing to burnout and negative well-being. Reluctance to seek care often reflects concerns about negative consequences (e.g., stigma, negative professional impact). We sought to characterize the availability of mental health resource availability, beliefs about seeking care, and barriers faced by emergency department healthcare providers. A sequential mixed-method design was employed. Phase 1 included a survey of ED physicians, adv

Expectations and experiences of yoga in women with breast cancer receiving endocrine therapy: a qualitative study.

BACKGROUND: Online, group-based yoga is increasingly explored as a supportive intervention for women with breast cancer, yet such programs are rarely delivered by yoga instructors who are also licensed nurses and have seldom focused on the early phase of endocrine therapy-a period often characterized by a high symptom burden and considerable bodily uncertainty. Little is known about how women in Türkiye experience participation in such programs during this stage of treatment. METHODS: A qualitative design informed by Interpretative phenomenological analysis (IPA) was employed. Semi-structured
