Bone marrow-versus adipose tissue-derived mesenchymal stem cells for corneal failure in an experimental model of limbal stem cell deficiency.

Ocular limbal stem cell deficiency (LSCD) occurs because of corneal epithelial stem cell destruction or dysfunction at the limbal niche. LSCD can cause corneal blindness, and the current therapy based on limbal stem cell transplantation is continuously improving. The aim of this work was to compare the safety and efficacy of human mesenchymal stem cells (hMSCs) derived from bone marrow (hBM-MSCs) and adipose tissue (hAT-MSCs) when transplanted to a rabbit model of LSCD. Both hMSC types expressed the corneal and limbal epithelial cell markers CK3, CK12, ZO-1, and ABCG2 under standard culture co

PAX6 Deficiency Compromises the Ability of Limbal Epithelial Stem Cells to Properly Differentiate Into Mature Corneal Epithelial Cells.

PURPOSE: Aniridia, driven by PAX6 mutations, causes aniridia-associated keratopathy (AAK), a progressive condition linked to limbal stem cell deficiency. A major hurdle to developing targeted therapies for AAK is the incomplete understanding of the molecular abnormalities in affected corneas. To address this, we leveraged Pax6± (Pax6 het) mice, a model of AAK, and applied single-cell RNA sequencing (scRNA-seq) to profile the transcriptomic changes at a single-cell resolution. METHODS: ScRNA-seq of corneal/limbal tissues of wild type (WT) and Pax6 het mice were conducted. Immunostaining was pe

Simple oral mucosal epithelial transplantation (SOMET) for bilateral limbal stem cell deficiency in a patient of failed simple limbal epithelial transplantation (SLET): a case report.

INTRODUCTION: We report the successful outcome of cataract surgery following simple oral mucosal epithelial transplantation (SOMET) in a patient with drug-induced cicatrizing conjunctivitis (DICC) and failed simple limbal epithelial transplantation (SLET). While SOMET has previously been described in Stevens-Johnson syndrome, its utility in cases of failed SLET or DICC has not been reported. Additionally, the long-term outcomes of SOMET and subsequent cataract surgery post SOMET have also not been described previously. CASE PRESENTATION: A male in his thirties presented with bilateral limbal

Limbal stem cell deficiency (LSCD) in rats and mice following whole body exposure to sulfur mustard (SM) vapor.

Ocular injuries following sulfur mustard (SM) exposure are characterized by an acute phase expressed by corneal erosions and inflammation of the anterior segment that after a clinically silent period may be followed by irreversible corneal injuries. The latter includes epithelial defects, chronic inflammation and neovascularization (NV), and were defined in rabbits and in humans as Limbal Stem Cell Deficiency (LSCD), that derived from a delayed loss of corneal epithelial stem cells (ESC), due to secondary processes most likely in the epithelial stem cell (SC) niche. The effect of in vivo grown corneal epithelium transplantation on persistent epithelial defects with limbal stem cell deficiency.

We report our experience with corneal epithelium, grown in vivo, transplantation in three patients with persistent epithelial defect (PED). The three patients had ocular surface disease unresponsive to standard treatments and were therefore chosen for transplantation. They underwent transplantation of epithelial sheets, grown in vivo, to the most affected eye. In vivo cultivation was carried out in the cornea of a living related donor. After epithelialization was completed, the epithelium grown on an amniotic membrane was harvested gently; it was then transplanted into the patient's eye after

Comparison of Two Rabbit Models with Deficiency of Corneal Epithelium and Limbal Stem Cells Established by Different Methods.

Limbal stem cell defect model is an important animal model that provides a basis for the study of ocular surface diseases. The rabbit cornea is of moderate size and is widely used in such studies as an experimental animal model. At present, the main modeling methods are alkali burns, and corneal limbus girdling and corneal epithelium doctoring. Each method has its own characteristics. In this study, we observed rabbit models with severe ocular surface defect established by the two methods and changes after amniotic membrane transplantation. In the first, second, third, and fourth week after op

Why Are Corneal Specialists Resistant to Treating Patients Who Have Severe Ocular Surface Disease With Limbal Stem Cell Deficiency?

PURPOSE: To highlight the paucity of surgeons performing ocular surface stem cell transplantation with systemic immunosuppression (OSSTx with SI) for limbal stem cell deficiency (LSCD) patients, suboptimal treatments for LSCD, and obstacles to adoption. METHODS: A review of the Eye Bank Association of America annual reports and the authors' case volume for OSSTx with SI was performed. Examination of the published literature on corneal surgeries, especially for LSCD, was completed. These findings were combined with our clinical observations to develop this editorial. RESULTS: Despite techniqu

Plasticity of ocular surface epithelia: Using a murine model of limbal stem cell deficiency to delineate metaplasia and transdifferentiation.

Maintaining corneal health and transparency are necessary pre-requisites for exquisite vision, a function ascribed to stem cells (SCs) nestled within the limbus. Perturbations to this site or depletion of its SCs results in limbal SC deficiency. While characterizing a murine model of this disease, we discovered unusual transformation phenomena on the corneal surface including goblet cell metaplasia (GCM), conjunctival transdifferentiation, and squamous metaplasia (SQM). GCM arose from K8+ differentiated conjunctival epithelial cells when the limbus was breached and was exacerbated by neovascul

Transplantation of autologous cultivated oral mucosal epithelial sheets for limbal stem cell deficiency at Siriraj Hospital: a case series.

BACKGROUND: The loss of limbal stem cells owing to either corneal burn or inflammation leads to the repopulation of opaque skin over the raw surface of the cornea. It has been proposed that reconstitution of oral mucosal stem cells over this raw surface will mimic the limbal stem cells and restore vision. The efficacy and safety of applying a sheet of cultivated oral mucosal cells as an autologous graft for corneal replacement were evaluated. CASE PRESENTATION: The study was conducted during 2014-2015 and involved a total of six patients, of whom three had suffered a chemical burn and three h

Cultivated Oral Mucosal Epithelial Transplantation for Limbal Stem Cell Deficiency: A Scoping Review of Indications, Platforms, Outcomes and Safety.

Background: Cultivated oral mucosal epithelial transplantation (COMET/CAOMECS) is an autologous, immunosuppression-sparing option for ocular surface reconstruction in limbal stem cell deficiency (LSCD). After two decades, indications, platforms and outcome definitions vary, and COMET's position relative to limbal-derived epithelium remains uncertain. Methods: We conducted a PRISMA-ScR scoping review of human clinical studies (PubMed, 2000-30 December 2025) with hand-searching and regulatory sources. Eligible reports included COMET/CAOMECS series and comparative cohorts (CLET/ACLET, SLET, KLAL/

A novel rat model of neurotrophic keratopathy complicated with limbal stem cell deficiency induced by trigeminal ophthalmic branch injury.

BACKGROUND: Animal models that simultaneously reproduce neurotrophic keratopathy (NK) and limbal stem cell deficiency (LSCD) are scarce, despite the frequent clinical coexistence of these two conditions. We aimed to establish a reproducible rat model showing a combined NK/LSCD phenotype by selectively injuring the ophthalmic branch of the trigeminal nerve (V1) within the orbit. METHODS: Adult male Sprague-Dawley rats underwent intraorbital V1 transection or V1 exposure without transection. Corneal sensitivity was evaluated qualitatively using a cotton-swab touch response and quantitatively us

Development of a microscopy-based diagnostic test for alkali injury-induced limbal stem cell deficiency through Autofluorescence Multispectral Imaging.

Diagnosing limbal stem cell deficiency (LSCD) remains challenging, and this study evaluates a newly developed Autofluorescence Multispectral Imaging (AFMI) technology for its diagnostic potential. Unstained fixed corneal cross-sections from 36 mice were used in this study. The right eyes of these mice had alkali-induced LSCD, while the left eyes served as healthy controls. Autofluorescence multispectral images of the corneal epithelium were acquired across multiple spectral channels to track changes in endogenous fluorophore spectra. Big data analytics and machine learning classifiers were app

Limbal Stem Cell Deficiency Induced by Prolonged Use of Topical 0.03% Tacrolimus Ointment in a Patient With Atopic Keratoconjunctivitis.

A 20-year-old female patient with a long-standing history of severe atopic disease was referred to our department because of bilateral gradual blurry vision. She had undergone treatment with daily application of topical tacrolimus ointment 0.03% (Protopic, Fujisawa, Munich, Germany) for a period of 5 years. Slit-lamp examination revealed inferior corneal thinning with conjunctivalization and superficial neovascularization respecting both eyes. A diagnosis of bilateral inferior limbal stem cell deficiency (LSCD) was made. After the tacrolimus ointment discontinuation, symptoms, visual acuity, a
