Inhibition of active sodium absorption leads to a net liquid secretion into in vivo rabbit lung at two levels of alveolar hypoxia.

Active sodium transport across alveolar epithelium is known to contribute to the resolution of pulmonary oedema. We have attempted to assess whether sodium transport is essential to prevent liquid accumulation in healthy pulmonary alveoli exposed to mild hypoxia, and whether its contribution to liquid absorption differs between mild and moderate levels of hypoxia. In twenty-four anaesthetized adult rabbits we used direct bronchial cannulation to measure liquid movement from the liquid-filled left lung over 3.5 h. Half of the rabbits were studied at a level of mixed venous (and alveolar) oxygen

Chest wall motion during spontaneous breathing and mechanical ventilation in dogs.

We measured the volume change of the thoracic cavity (delta Vth) and the volumes displaced by the diaphragm (delta Vdi) and rib cage (delta Vrc) in six pentobarbital-anesthetized dogs lying supine. A high-speed X-ray scanner (dynamic spatial reconstructor) provided three-dimensional images of the thorax during spontaneous breathing and during mechanical ventilation with paralysis. Tidal volume (VT) was measured by integrating gas flow. Changes in thoracic liquid volume (delta Vliq, presumably caused by changes in thoracic blood volume) were calculated as delta Vth - VT. Absolute volume displac

Surfactant enhances the distribution of ventilation in partially aerated lungs of preterm rabbits.

In very preterm infants, nonuniform lung aeration occurs when regions of the immature lung remain liquid-filled after birth, which restricts gas exchange to aerated lung regions. We have examined the effect of surfactant administration on the uniformity of lung aeration and the distribution of ventilation in mechanically ventilated preterm rabbits. Preterm kittens (28-29 days gestation; term: ∼32 days) were delivered by cesarean section and intubated via tracheostomy. Before the experiment, kittens were initially ventilated (intermittent positive pressure ventilation; iPPV) to achieve whole (b

Mechanisms of the Effects of Short-Term Inhalations of Xe and O2 Gas Mixture in the Rehabilitation of Post-COVID Ventilation Failure.

The article presents a theoretical rationale and a clinical case of relief of post-COVID ventilation failure by inhalation of Xe and O2 gas mixture. Pneumonitis of coronavirus etiology transforms saturated phospholipids of surfactant into a solid-ordered phase, which disrupts surface tension, alveolar pneumatization, and alveolar-capillary gas exchange. Using molecular modeling (B3LYP/lanl2dz; GAUSSIAN09), we demonstrated that Xe atom due to the van der Waals dispersion interaction increases the distance between the phospholipid acyl chains providing a phase transition from the solid-ordered t

Transmission of pathogen-laden expiratory droplets in a coach bus.

Droplet dispersion carrying viruses/bacteria in enclosed/crowded buses may induce transmissions of respiratory infectious diseases, but the influencing mechanisms have been rarely investigated. By conducting high-resolution CFD simulations, this paper investigates the evaporation and transport of solid-liquid mixed droplets (initial diameter 10 μm and 50 μm, solid to liquid ratio is 1:9) exhaled in a coach bus with 14 thermal manikins. Five air-conditioning supply directions and ambient relative humidity (RH = 35 % and 95 %) are considered. Results show that ventilation effectiveness, RH and i

Mammalian enteral ventilation ameliorates respiratory failure.

BACKGROUND: Several aquatic organisms such as loaches have evolved unique intestinal breathing mechanisms to survive under extensive hypoxia. To date, it is highly controversial whether such capability can be adapted in mammalian species as another site for gas exchange. Here, we report the advent of the intestinal breathing phenomenon in mammalians by exploiting EVA (enteral ventilation via anus). METHODS: Two different modes of EVA were investigated in an experimental model of respiratory failure: intra-rectal oxygen O2 gas ventilation (g-EVA) or liquid ventilation (l-EVA) with oxygenated p

Hydroxyl radical formation in the perilymph of asphyxic guinea pig.

To elucidate the role of hydroxyl radical (.OH) species in the generation mechanism of the cochlear pathology induced by transient asphyxia and subsequent re-ventilation, the concentrations of 2,3-hydroxybenzoic acid (DHBA) and 2,5-DHBA, major products arising from the attack of .OH upon salicylate, were measured in the perilymph of the guinea pig by the high performance liquid chromatography-electrochemical/UV method. The mean value of 2, 3-DHBA concentration in the perilymph significantly increased from the pre-asphyxic level (6.4 microM) to 7.6 microM and 8.8 microM during asphyxia of 3 min

Respiratory support techniques for prematurely born infants: new advances and perspectives.

A variety of respiratory techniques have been introduced into the neonatal intensive care unit. High frequency positive pressure ventilation (HFPPV) compared to slow rate ventilation significantly reduced the incidence of airleaks, but this has yet to be confirmed in infants routinely exposed to antenatal steroids and postnatal surfactant. Continuous positive airways pressure (CPAP) is useful for infants with obstructive or mixed apnoea and reduces the need for extra respiratory support following extubation, whether it reduces chronic lung disease (CLD) remains to be appropriately tested. Achalasia Revealed by Respiratory Failure and Hemodynamic Instability.

Achalasia is a rare condition that most often presents with progressive dysphagia to solids and liquids. We report a case of achalasia presenting with acute respiratory failure and hemodynamic instability requiring both ventilator and vasopressor support because of extrinsic compression of the airway and left atrium by a dilated and fluid-filled esophagus. This is the first case reported of achalasia, causing both left atrial compression and airway compression.

Preterm ovine respiratory epithelial cell responses to mechanical ventilation, lipopolysaccharide, and interleukin-13.

Mechanical ventilation causes airway injury, respiratory epithelial cell proliferation, and lung inflammation in preterm sheep. Whether preterm epithelial cells respond similarly to adult epithelial cells or are altered by mechanical ventilation is unknown. We test the hypothesis that mechanical ventilation alters the responses of preterm airway epithelium to stimulation in culture. Respiratory epithelial cells from the trachea, left mainstem bronchi (LMSB), and distal bronchioles were harvested from unventilated preterm lambs, ventilated preterm lambs, and adult ewes. Epithelial cells were gr

Respiratory failure. Conventional and high-tech support.

Although significant progress has been made in the treatment of patients with acute lung failure in the critical care setting, the mortality rate from acute lung injury and ARDS is unacceptably high, given the numbers of patients treated for these syndromes each year. The improved understanding of the pathophysiology of respiratory failure from basic science and clinical research is reflected in improved survival rates over the years. Advances in the mechanical ventilator (through microprocessor technology); biosurface technology; liquid ventilation; and, in some cases, returning to so-called

[Eustachian tube drug delivery to the tympanum in patients with otitis media with effusion].

There are no reliable data on the volume of the drug reaching the affected tympanic cavity during the catheterization of Eustachian tube (ET). Objective: to conduct an experiment and to study the effectiveness of administering a drug into the tympanic cavity of the middle ear using an original ear catheter, which allows catheterization of ET in the direction of its lumen. The study included patients with unilateral OME (n=21, the duration of the disease from 2 to 3 months), who were recommended for the ventilation tube insertion. The main inclusion criterion was 3 degree of patency of the ET.

Near fatal intoxication by nicotine and propylene glycol injection: a case report of an e-liquid poisoning.

BACKGROUND: Concern about intoxication by e-liquid is growing as calls to poison control centers have increased since their introduction. Only three cases of intoxication by injection have been reported worldwide. Our case is unique because of the precise follow-up of a patient who survived a lethal dose of self-injected e-liquid, without other co-intoxication. CASE PRESENTATION: A 51-year-old male presented to the Emergency Department after injecting himself intravenously (IV) in the forearm with 10 mL of e-liquid (1000 mg of nicotine diluted in propylene glycol). An agitation phase was foll

Generation of purified nitric oxide from liquid N2O4 for the treatment of pulmonary hypertension in hypoxemic swine.

Inhaled nitric oxide (NO) selectively dilates pulmonary blood vessels, reduces pulmonary vascular resistance (PVR), and enhances ventilation-perfusion matching. However, existing modes of delivery for the treatment of chronic pulmonary hypertension are limited due to the bulk and heft of large tanks of compressed gas. We present a novel system for the generation of inhaled NO that is based on the initial heat-induced evaporation of liquid N2O4 into gas phase NO2 followed by the room temperature reduction to NO by an antioxidant, ascorbic acid cartridge just prior to inhalation. The biologic ef

Influence of kindergarten dormitory bed layout on the proximity propagation characteristics of exhaled pollutants.

Kindergarten dormitories are indoor napping areas where preschool children spend extended periods nearby, making them high-risk environments for the transmission of respiratory diseases. To understand the transmission characteristics of respiratory pollutants, particularly CO2 and simulated cough aerosols between adjacent beds, two common bed layouts in kindergartens were investigated: three beds of staggered height (TBSH) and three beds of uniform height (TBUH). The experiments measured CO2 and PM2.5 concentrations (using liquid aerosols generated by an ultrasonic nebulizer as surrogates for

Current and emerging treatment for congenital diaphragmatic hernia.

Management of congenital diaphragmatic hernia has changed dramatically over the past couple of decades. Until the early 1980s, it was felt that the abdominal contents should be returned to the abdomen as soon as possible to allow the lungs to expand. It is now known that it is not the defect that causes respiratory distress, but the infant's hypoplastic lungs and accompanying pulmonary hypertension. Advances in treatment and technology have contributed to changes in management. Ultrasonography now allows for early prenatal detection. Prenatal treatment modalities include in utero tracheal liga

Proposal of selective wedge instillation of pulmonary surfactant for COVID-19 pneumonia based on computational fluid dynamics simulation.

BACKGROUND: The most important target cell of SARS-CoV-2 is Type II pneumocyte which produces and secretes pulmonary surfactant (PS) that prevents alveolar collapse. PS instillation therapy is dramatically effective for infant respiratory distress syndrome but has been clinically ineffective for ARDS. Nowadays, ARDS is regarded as non-cardiogenic pulmonary edema with vascular hyper-permeability regardless of direct relation to PS dysfunction. However, there is a possibility that this ineffectiveness of PS instillation for ARDS is caused by insufficient delivery. Then, we performed PS instillat

Liquid Lung Rest During Extracorporeal Life Support in a Neonatal Porcine Model of Acute Lung Injury.

In patients with severe lung injury, extracorporeal life support (ECLS) enables lung rest, but the optimal strategy is unknown. This study compared liquid lung rest (LLR) versus standard gas rest ventilation in a porcine model of acute lung injury supported by ECLS. Twenty neonatal pigs received oleic acid-induced lung injury and were placed on ECLS. Pigs were ventilated with standard rest settings (peak inspiratory pressure 20 cm H2O; end expiratory pressure 10 cm H2O; rate 10 breaths/minute) and randomized to gas ventilation alone or 5 ml/kg perfluorooctylbromide instilled endotracheally (LL
