Advanced metastatic pancreatic neuroendocrine tumor treated successfully with peptide receptor radionuclide therapy: a case report.

Neuroendocrine tumor (NET) is a rare tumor that has been observed in different sites such as lungs and throughout the gastrointestinal tract. Clinical features are usually non-specific and vary considerably depending upon the location of the tumor. Symptoms are similar to those of common conditions such as peptic ulcer disease, gastritis, irritable bowel syndrome, asthma, etc. Thus, an initial diagnosis of a NET usually occurs at an advanced stage. This report describes a case of pancreatic NET (PNET, grade 2) with liver metastasis in a 37-year-old male which was found to be inoperable due to

Prostate cancer.

Prostate cancer poses a substantial clinical challenge and accounts for a large proportion of cancer-related deaths worldwide. The therapeutic landscape has undergone a large transformation in the past 5 years, resulting in improved patient outcomes. In this Seminar, we review the pathology, diagnostic strategies, and treatments for prostate cancer. Active surveillance is the preferred treatment option for patients with indolent prostate cancer. For those requiring treatment, local therapies provide effective cancer control. Systemic treatment is essential for advanced and metastatic cases, an

Pheochromocytoma in MEN2.

Pheochromocytomas (PCs) are rare neuroendocrine tumors found in 20-50% of MEN2 patients. MEN2-related PCs are more often bilateral, identified at a younger age and have a low metastatic potential. They secrete epinephrine as the predominant catecholamine, along with its metabolite metanephrine, and lesser amounts of norepinephrine and normetanephrine. The advent of molecular diagnostic tools has enhanced the identification and stratification of these tumors, revealing a strong genotype-phenotype correlation which is crucial for screening and managing patients. Evaluation involves a combination

Synthesis and comparative evaluation of 177Lu-labeled PEG and non-PEG variant peptides as HER2-targeting probes.

Highest global cancer incidence of female breast cancer is a matter of great concern. HER2-positive breast cancers have high mortality rate hence detection at an early stage is vital for successful treatment, improved cancer care and survival rate. Radiolabeled peptides have emerged as new alternatives to radiolabeled antibodies to overcome the limitations of slow clearance and uptake in non-target tissues. Herein, DOTA-A9 peptide and its pegylated variant were constructed on solid phase and radiolabeled with [177Lu]LuCl3. [177Lu]DOTA-A9 and [177Lu]DOTA-PEG4-A9 displayed high binding affinity

Illuminating radionuclide therapy response: Monitoring cancer cell and CAF survival in a direct 2D co-culture model.

BACKGROUND: Targeted radionuclide therapy (TRT) directed at fibroblast activation protein (FAP), highly expressed on cancer-associated fibroblasts (CAFs), is a promising approach for treating stroma-rich tumors such as pancreatic ductal adenocarcinoma (PDAC) and breast cancer (BC). To better assess the efficacy of FAP-TRT and to get more insight into the potential of crossfire effects from target expressing CAFs to neighboring cancer cells, more representative preclinical models are needed. Therefore, we established a direct 2D co-culture model consisting of both CAFs and cancer cells. MATERI

A novel Lu(III)-based rare-earth metal-organic frameworks for rapid detection of phosphate with triple-signal output.

Accurate detection of phosphate is vital for food safety, health, and environmental monitoring. This work presents a rectangular plate-like lutetium(III)-based metal-organic frameworks (Lu-MOFs) utilizing Lu(III) ions as metal centers and TCPP as the ligand. Phosphate selectively disrupts the ligand-to-metal charge transfer within Lu-MOFs via host-guest interactions, inducing dual optical responses of enhanced magenta fluorescence and brown chromogenic transition. A portable sensing platform was developed using Lu-MOFs nanoprobe for on-site detection of phosphate, which integrates triple-signa

Success and Limitations of PSMA RLT : The Nuclear Medicine View.

Lutetium-177 PSMA radioligand therapy (RLT) represents a paradigm shift in advanced prostate cancer care, establishing a precision oncology standard through its inherent theranostic capability. From a nuclear medicine perspective, its defining success lies in the ability to precisely select patients and offer targeted cytotoxic delivery of radiation, resulting in high objective response rates and quality-of-life improvements, particularly rapid pain palliation. A collaborative view with medical oncology highlights critical limitations. Despite frequent favourable initial responses, disease rec

New Paradigms in Theranostics of Prostate Cancer: The Rise of α-Emitters and Next-generation Targets.

Radioligand therapy (RLT) with [177Lu]Lutetium-PSMA (prostate specific membrane antigen) has become a pillar of prostate cancer treatment; however, resistance and PSMA-negative disease remain as unmet needs. The α-emitting radionuclides represent a paradigm shift, with physical properties suited to overcome β-emitter resistance. [225Ac]Actinium-PSMA (prostate specific membrane antigen) has shown promising retrospective results, and ongoing trials are refining its sequencing and safety. Astatine-211 and lead-212 may further improve dosimetry and supply scalability. Beyond PSMA, emerging targets

Frailty and [177Lu]Lu-PSMA-617 therapy: tools and strategies for older men with metastatic castration-resistant prostate cancer.

The prognosis of metastatic castration-resistant prostate cancer is dismal. [177Lu]Lutetium-prostate-specific membrane antigen (PSMA)-617 ([177Lu]Lu-PSMA-617) is the new standard of care for PSMA-positive mCRPC previously treated with ARPI and taxane-based chemotherapy. The efficacy and safety of [177Lu]Lu-PSMA-617 have been confirmed by phase 2 (TheraP [NCT03392428]) and phase 3 (VISION study [NCT02511664] and PSMAfore trial [NCT04689828]) clinical trials. A substantial proportion of patients with mCRPC are older adults; however, chronological age alone should not be considered a contraindica

A PEI-Based Surface Strategy for Robust 177Lu Incorporation into Biodegradable Chitosan Microspheres: A Potential Platform for Hepatocellular Carcinoma Radioembolization.

While lutetium-177 (177Lu) holds immense promise as a theranostic radionuclide for image-guided radioembolization of hepatocellular carcinoma, its translation into effective biodegradable microsphere platforms has been critically hindered by the lack of stable and efficient carriers capable of robustly incorporating the radionuclide while maintaining biodegradability. To bridge this gap, we herein develop biodegradable chitosan microspheres engineered with an aminated surface via sequential grafting of poly(glycidyl methacrylate) (PGMA) and polyethylenimine (PEI) to establish a robust Lu(III)

Intratumoral dose heterogeneity promotes adaptive anti-tumor immunity and predicts clinical response to radiopharmaceutical therapy.

Radiopharmaceutical therapies (RPT) deliver non-uniform radiation dose in tumors and the impact of this on response is poorly understood. Dose heterogeneity could engender treatment resistance in low dose regions, yet we hypothesize that a broader array of dose-dependent immuno-radiobiological mechanisms in tumor microenvironments (TME) and preservation of immune function in low-dose regions could promote adaptive anti-tumor immunity and response. In murine models, non-uniform lutetium-177 delivering <2.5 Gy to >20 Gy in a TME induced broader immunomodulatory effects and T cell-dependent survi

Newly diagnosed high-risk localized prostate cancer after multiple [177Lu]Lu-PSMA radioligand therapy in the context of metastatic parotid carcinoma: a case report.

BACKGROUND: Prostate-specific membrane antigen-direct radioligand therapy (PSMA RLT) is primarily used to treat castration-resistant metastatic prostate cancer (PCa) after prior treatment options have been exhausted. Occasionally, other tumor entities, such as parotid carcinoma in this case, also express prostate-specific membrane antigen (PSMA) on their cell surface. CASE DESCRIPTION: We present the case of a white 67-year-old patient newly diagnosed with locally advanced PCa despite multiple cycles of Lutetium-177-labeled prostate-specific membrane antigen radioligand therapy ([177Lu]Lu-PSM

A 177Lu-Labeled Circular Bivalent Aptamer for PTK7-Targeting Radionuclide Therapy in Ovarian Cancer and Orthotopic Hepatoblastoma.

Targeted radionuclide therapy (TRT) is a promising strategy for precision oncology but is limited by the lack of high-performance targeting ligands. Here, we report a lutetium-177 (177Lu)-labeled PTK7-targeted cyclized bivalent aptamer (CBSgc8) for treating an orthotopic hepatoblastoma. The cyclized multivalent design enhances biostability and binding affinity, while 177Lu labeling preserves its functional properties. The resulting 177Lu-CBSgc8 exhibited high serum stability, strong specificity for PTK7-positive cells, efficient internalization, and dose-dependent cytotoxicity in vitro. In viv

The Synthesis and Structure of a Scandium Nitrate Hydroxy-Bridged Dimeric Complex Supported by Bipyridyl Ligands.

The current discussion on whether scandium, yttrium and lanthanum should represent Group 3 in the Periodic Table or whether lutetium should replace lanthanum in the group has prompted us to further explore the structural chemistry of the Group 3 elements and compare the coordination numbers and coordination geometries adopted. The steric and electronic properties of the coordinated ligands have a major influence on the structures adopted. We report the synthesis and crystal structure determination of an unusual dinuclear scandium complex [(bipy)(NO3)2Sc(µ-OH)2Sc(NO3)2(bipy)] obtained by the re

Thermodynamic evaluation of the stability of the bone-seeking radiopharmaceutical [177Lu]Lu(III)-DOTP under simulated blood plasma conditions.

The stability and in vivo robustness of [(177)Lu]Lu-DOTP as a potential bone-targeting radiopharmaceutical was determined with the aid of thermodynamic blood plasma modeling simulations. Glass electrode potentiometry was employed to measure the stability constants of the complexes of Lu(3+) with DOTP. Similarly, the complexes of DOTP with a selection of the important physiological metal ions: Ca(2+), Mg(2+), and Cu(2+) were determined, representing the typical interactions that the ligand would encounter upon administration. This made possible the construction of a blood plasma model of DOTP,

Metal-metal bonds in f-element chemistry.

The molecular chemistry of the f-elements is traditionally dominated by the use of carbon-, nitrogen-, oxygen-, or halide-ligands. However, the use of metal-based fragments as ligands is underdeveloped, which contrasts to the fields of d- and p-block metal-metal complexes that have developed extensively over the last fifty years. This perspective outlines the development of compounds, which possess polarised covalent or donor-acceptor f-element-metal bonds. For this review, the f-element is defined as (i) a group 3 or lanthanide metal: scandium, yttrium, lanthanum to lutetium, or (ii) an actin

Upfront 177 Lu-PSMA Radioligand Therapy With Concurrent Androgen-deprivation Therapy Producing Dramatic Clinical and Biochemical Response : A Case Report.

A 72-year-old man presented with severe weight loss, anemia, and a PSA level of 73 ng/mL. Bone scintigraphy and 99m Tc-PSMA imaging revealed extensive skeletal metastases. As part of a clinical trial, he received upfront 177 Lu-PSMA radioligand therapy combined with androgen-deprivation therapy (ADT). The second cycle of 177 Lu-PSMA resulted in complete resolution of all PSMA-avid lesions and a 99% decline in PSA to <0.4 ng/mL. Radioligand therapy was discontinued after achieving complete molecular and biochemical response. At 8 months of follow-up, the patient remains disease-controlled, supp
