Case study: Malakoplakia of the bladder.

OBJECTIVES: Malakoplakia is a rare chronic inflammatory disease thought to be the result of defective bacterial phagocytosis and lysosome function, and there is difficulty in accurate diagnosis as a result of non-specific symptoms that mimic other diseases and cancers. This study presents a case of bladder malakoplakia associated with renal failure presenting as a tumor. METHODS: A 55-year-old woman with history of kidney disease who presented with general malaise and worsening renal failure was found to have a bladder mass and underwent transurethral resection of bladder tumor (TURBT), and s

Malakoplakia of urinary tract: report of two cases with emphasizing the histologic spectrum and the morphogenesis of Michaelis-Gutmann bodies.

Two cases of urinary tract malakoplakia, one in right kidney and the other one in urinary bladder, were reported. Both cases affected middle-aged female patients with long-term urinary tract infection. Their clinical presentation simulated a neoplasm. The diagnosis entirely depended on pathologic examination. The former case also extensively involved the retroperitoneum, liver and duodenum. The latter case was unusually associated with Pseudomonas infection. Electron microscopy showed that the morphogenesis of Michaelis-Gutmann bodies occurred within the phagolysosomes. Initially, small myelin

Two cases of malakoplakia of the prostate.

2 further cases of malakoplakia of the prostate are reported, so that a total of 23 cases have now been described. The histological picture, as well as the methods of staining for the demonstration of the pathognomonic Michealis-Gutmann bodies are reviewed. The presence of Escherichia coli as a possible factor in the etiology is discussed. A description is given of the symptoms and physical findings in cases of prostatic malakoplakia, and the differential diagnosis, with particular reference to cancer of the prostate, is reviewed. Histology alone can provide the correct diagnosis. Finally it i

A Case of Urinary Bladder Malakoplakia in a Young French Bulldog: Diagnostic and Therapeutic Issues.

A 3-month-old female French Bulldog presented with hematuria, severe pollakiuria, and urinary incontinence lasting for 1.5 months. Broad-spectrum empirical antibiotic therapy and nonsteroidal anti-inflammatory drugs were initiated by the referring veterinarian. Due to a lack of improvement, the dog was referred. At referral examination, urinary clinical signs persisted (hematuria, severe pollakiuria) and a firm bladder was noted. Abdominal ultrasonography revealed severe, diffuse bladder wall thickening with a significant reduction in the bladder lumen. Urinary tract endoscopy showed whitish e

Prostatic malakoplakia: clinicopathological assessment of a multi-institutional series of 49 patients.

Prostatic malakoplakia (MP) is rare, with only case reports and small series (< five patients) available in the literature. In this study we analysed an international multi-institutional series of 49 patients with prostatic MP to more clearly define its clinicopathological features. The median age was 67 years and the median serum prostate-specific antigen (PSA) was 7.5 ng/ml. MP was clinically manifest in most cases (28 of 45 patients with data available, 62%). Of 43 patients with detailed clinical history available, 21 (49%) had concurrent or metachronous malignancies (including prostate can

An unusual case of extensive epididymotesticular malakoplakia in a diabetic patient.

A 67-year-old Saudi man with poorly controlled diabetes-mellitus underwent orchiectomy for a painful left testicular swelling that did not respond to antibiotics. Clinical diagnosis was testicular malignancy. Histologic workup demonstrated extensive involvement of the testis and epididymis by diffuse infiltrate of large histiocytes with eosinophilic granular cytoplasm and numerous Michaelis-Gutmann bodies, which were diagnostic of malakoplakia. Very rarely, malakoplakia has been described in association with diabetes mellitus. Such an extensive malakoplakia occurring at an unusual site in asso

Malakoplakia of the vagina diagnosed by fine-needle aspiration cytology.

Malakoplakia is an uncommon chronic granulomatous process that most commonly affects the urinary tract, but it may rarely recur in the female genital tract. It appears to result from a defect in macrophage function resulting in an inability to destroy ingested bacteria. We describe the fine needle aspiration cytology of malakoplakia in a 84-yr-old woman presenting with a large pelvic mass involving the apex of the vagina. The patient's history was significant for cervical squamous cell carcinoma in the remote past. CT-guided needle aspiration yielded cellular smears with large numbers of isola

Sonography of renal inflammatory disease.

The sonographic findings and differential diagnoses of a wide spectrum of acute and chronic renal inflammatory processes are reviewed. These include focal and diffuse forms of bacterial nephritis, pyonephrosis, intra- and extrarenal abscess, renal tuberculosis and fungal infection, xanthogranulomatous pyelonephritis, and renal parenchymal malakoplakia. The role of ultrasound in the diagnostic and therapeutic management of patients with these diseases is discussed.

Imaging Manifestations of Acute and Chronic Renal Infection That Mimics Malignancy: How to Make the Diagnosis Using Computed Tomography and Magnetic Resonance Imaging.

PURPOSE: To review the computed tomography and magnetic resonance imaging manifestations of acute and chronic renal infections that may mimic malignancy and to provide useful tips to establish an imaging diagnosis. CONCLUSION: Acute and chronic bacterial pyelonephritis are usually readily diagnosed clinically and on imaging when the diagnosis is suspected based upon clinical presentation. When unsuspected, focal, extensive or mass-like, acute and chronic bacterial pyelonephritis may mimic infiltrative tumours such as urothelial cell carcinoma (UCC), lymphoma, and metastatic disease. Infection

Benign Conditions That Mimic Prostate Carcinoma: MR Imaging Features with Histopathologic Correlation.

Multiparametric magnetic resonance (MR) imaging combines anatomic and functional imaging techniques for evaluating the prostate and is increasingly being used in diagnosis and management of prostate cancer. A wide spectrum of anatomic and pathologic processes in the prostate may masquerade as prostate cancer, complicating the imaging interpretation. The histopathologic and imaging findings of these potential mimics are reviewed. These entities include the anterior fibromuscular stroma, surgical capsule, central zone, periprostatic vein, periprostatic lymph nodes, benign prostatic hyperplasia (

Granulomatous prostatitis: distinction among allergic, nonspecific, and post-transurethral resection lesions.

A variety of granulomatous lesions of the prostate, with and without extensive infiltration by eosinophils, have been described. Differing concepts of their pathogenesis, and especially of their relation to allergic states, have produced confusion and controversy. In a review of 62 patients in whom granulomatous lesions in the prostate were diagnosed from 1950 to 1982 at The Johns Hopkins Hospital, the authors identified four major categories. In nine cases specific granulomatous prostatitis was caused by tuberculosis and had typical caseous foci. Nonspecific granulomatous prostatitis was pres

Malakoplakia In Lung Transplant Recipients: Case Series.

BACKGROUND: Malakoplakia is a rare granulomatous inflammatory disorder caused by impaired macrophage bactericidal activity, most often occurring in immunocompromised patients and commonly associated with chronic bacterial infection. Reports in lung transplant recipients remain limited. METHODS: We performed a retrospective review of lung transplant recipients diagnosed with malakoplakia at our center. Clinical presentation, microbiology, management strategies, and outcomes were analyzed descriptively. RESULTS: Five lung transplant recipients developed 6 episodes of malakoplakia between 5 and

Malakoplakia of the Endometrium: A Rare Unexpected Disease Raising Clinical Concern for Malignancy.

Malakoplakia is a rare disease that manifests as a histiocytic inflammatory process and most often occurs in the urinary bladder. It is caused by an impaired capacity of histiocytes to kill and digest bacteria. The typical histopathologic findings are sheets of histiocytes with granular eosinophilic cytoplasm and characteristic Michaelis-Gutmann bodies, spherical bodies with a targetoid appearance. Malakoplakia is even rarer in the gynecologic tract, and our literature search found only 21 published patients of malakoplakia involving the endometrium. Here we report a 60-year-old female patient

Malakoplakia associated with prostatic adenocarcinoma: Report of 4 cases and literature review.

Malakoplakia is an inflammatory process that has been rarely reported in the prostate. Malakoplakia in association with prostatic carcinoma is exceedingly rare with only 4 previously reported cases. We describe the clinical features and the associated pathology in 4 patients who demonstrated malakoplakia of the prostate in association with prostatic adenocarcinoma. Prostatic malakoplakia presenting in association with prostatic adenocarcinoma was identified in 4 patients through a search from the records of 3 institutional databases with large in-house and consult uropathology practices. In 2

Urinary tract malakoplakia with extension into the retroperitoneum with secondary gastrointestinal involvement.

A rare case of malakoplakia of the urinary tract with diffuse retroperitoneal extension is presented. Sonographically guided cytologic puncture revealed the pathologic diagnosis. The splenic flexure of the colon and the stomach appeared to be secondarily involved in the inflammatory retroperitoneal tumor. The relative role of the various imaging modalities in defining retroperitoneal extension of the disease is illustrated, with a special reference to computed tomography and endosonography. After antimicrobial treatment and left nephrectomy, partial regression of the retroperitoneal mass was d
