OBJECTIVE: Attention deficit hyperactivity disorder (ADHD) is a frequent comorbidity in patients with substance use disorder. The aim of this cross-sectional study was to evaluate the consequences of ADHD comorbidity in cocaine users seeking treatment in Martinique.
METHODS: During 15 months, all patients seeking treatment for cocaine dependence in a specialized center were assessed using the ADHD DSM-IV-TR (Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition-Text Revision) criteria and the French version of the Wender Utah Rating Scale.

A case of pulmonary Schistosoma mansoni bilharziasis observed in a female patient from Martinique is reported. In view of the unusual clinical and radiological features of the disease and of its rapidly worrying course, an open chest lung biopsy was performed which provided the diagnosis. A search for eggs in the stools was negative, and serological tests were weakly positive, treatment with Praziquantel resulted in rapid and complete cure.

A preliminary inquiry, conducted on Martinique Island, sought to determine professional skippers' sun-protection knowledge and behavior. Fifty-two skippers (mean age: 41 years) completed a questionnaire; 39 (75 %) had a simple sunburn over the last 6 months and 3 (6%) severe sunburn; 54 (64%) declared achieving sun protection by wearing clothes during >90% of the day. Only 17% had used sun protection >90% of the time.

BACKGROUND: To report a case of lymphomatous infiltration and bilateral retinal vasculitis observed among 83 cases of adult T-cell leukemia (ATL) treated in the University Hospital Center in Fort-de-France (Martinique, French West Indies) between 1984 and 2003.

Tropical spastic paraparesis (TSP) is a common myeloneuropathy with primary and predominant involvement of the pyramidal tract and minimal sensory loss. A preliminary inquiry, conducted on Martinique Island, sought to determine kite-surfers' sun-protection knowledge and behaviours. Results revealed good levels of knowledge and behaviours, but very frequent sunburns (74% had at least one sunburn during the last 6 months), with particularly severe sunburns exclusively among vacationers. These results argue for the double need to focus sunburn-prevention programs on vacationers and technically adapt sun-protection means to a tropical maritime environment.

The HTLV-1 virus causes a disturbance of the immune system, the evaluation of which is often difficult. We report a case of sarcoidosis in a 49 year old woman of Martinique as evidenced by bilateral hilar adenopathy, hypercalcaemia, uveitis and granulomatous lesions on histological examination. Serological was positive for HTLV-1 antibodies. Three years later she developed an adult T-cell leukemia/lymphoma. The relationships between the HTLV-1 retroviral infection and different pathologies observed are discussed.

Bothrops lanceolatus inflicts severe envenomings in the Lesser Caribbean island of Martinique. Bothrofav®, a monospecific antivenom against B. lanceolatus venom, has proven highly effective at the preclinical and clinical levels. Here, we report a detailed third-generation antivenomics quantitative analysis of Bothrofav®. With the exception of poorly-immunogenic peptides, Bothrofav® immunocaptured all the major protein components. These results, along with previous preclinical and clinical observations, underscore the high neutralizing efficacy of the antivenom against B. lanceolatus venom.

The production of twenty-seven strains of Fonsecaea pedrosoi was studied. The denticulate type (asexual reproduction) showed three morphological variations: medium-size, long and sessile forms. Four cases of severe strongyloidiasis in Martinique patients are reported. Clinical features were typical of the affection, and the outcome was fatal in two cases. Fibroscopy findings were negative but larvae were demonstrated in biopsy specimens in 3 out of the 4 cases. A barium meal follow-through examination showed the different radiological images of the infestation: non-specific parasitic duodenitis and pseudo-stenosis of D3 with duodenal dilatation, barium reflux into the main bile duct, and absence of jejunal folds, these images being very suggestive of strongyloidiasis.
