Puerperal sepsis: its contribution to maternal mortality.

Puerperal sepsis is the second most common cause of maternal mortality in the developing world. In this paper the extent of the problem is described and factors affecting puerperal sepsis are identified. Methods of reducing the incidence of puerperal sepsis are suggested. This paper is based on one originally given at the ICM/WHO/UNICEF pre-congress workshop in Kobe, Japan, October, 1990.

Maternal mortality surveillance, United States, 1980-1985.

To better define the incidence, causes, and risk factors associated with maternal deaths, the Division of Reproductive Health, Center for Chronic Disease Prevention and Health Promotion, CDC, coordinated a study by the Maternal Mortality Collaborative, a Special Interest Group of the American College of Obstetricians and Gynecologists (ACOG). In 1983, this group established voluntary surveillance of maternal deaths for the years 1980-1985. The Maternal Mortality Collaborative reported 601 maternal deaths from 19 reporting areas for 1980-1985, representing a maternal mortality ratio of 14.1 per

Maternal mortality in Cape Town, 1978-1983.

During 1978-1983, 57 maternal deaths (23 in blacks, 32 in coloureds and 2 in whites) occurred among 131,288 deliveries (36,564 in blacks, 89,335 in coloureds and 5389 in whites) in the Peninsula Maternal and Neonatal Service, Cape Town. Data for whites were not analysed further. Maternal mortality rates (MMRs) were higher in blacks than in coloureds. Age- and parity-specific MMRs showed that black teenagers and primiparas and coloureds aged 20-34 years and of parity 2-4 had the lowest rates. Advanced age and grand multiparity had a much greater adverse effect in coloureds than in blacks. Eight

Maternal smoking, pregnancy complications, and perinatal mortality.

Analysis of data from the Ontario Perinatal Mortality Study has shown that perinatal mortality increases directly with the level of maternal smoking during pregnancy. Increases in smoking level are associated with increases in the frequency of early fetal deaths and of neonatal deaths due to premature delivery. These deaths in turn are associated with smoking-related increases in the incidence of bleeding during pregnancy, abruptio placentae, placenta previa, and premature and prolonged rupture of the membranes.

Maternal mortality at University Teaching Hospital, Lusaka.

Eighty maternal deaths occurring at the University Teaching Hospital (UTH) Lusaka, from 1974-1976 have been reviewed. The maternal mortality rate was 1.5 per 1000 births. The commonest causes of death were pre-eclampsia and eclampsia (23), septicaemia (14), haemorrhage (13) and ruptured uterus (11). Avoidable hospital factors were present in 52 percent of cases, while avoidable outside factors were noted in 27 percent. Thirty two per cent had no antenatal care. The mortality rate may be reduced by increased awareness of high risk patients by medical staff in the central hospital. Improvements

Reducing Maternal Mortality and Severe Maternal Morbidity: The Role of Critical Care.

Throughout most of the 20th century, the risk of maternal mortality in high resource countries decreased dramatically; however, this trend recently has stalled in the United States and appears to have reversed. Equally alarming is that for every reported maternal death, there are numerous severe maternal morbidities or near misses. Shifting maternal demographics (eg, obesity, advanced maternal age, multifetal pregnancies), with attendant significant medical comorbidities (eg, hypertension, diabetes, cardiac disease) and the increase in cesarean deliveries significantly contribute to increased

Reducing Maternal Mortality and Severe Maternal Morbidity Through State-based Quality Improvement Initiatives.

State Perinatal Quality Collaboratives (PQCs) represent a major advance for scaling up quality improvement efforts for reducing maternal mortality and severe maternal morbidity. The critical roles of partners, rapid-cycle low-burden data systems, and linkage to maternal mortality review committees are reviewed. The choice of measures is also explored. California's experience with its PQC, data center, quality improvement efforts, and promising results for reduction of maternal mortality and morbidity from hemorrhage are presented. Early data from other states is also shared.

Mortality-weighted severe maternal morbidity: a novel approach to assessing maternal health outcomes.

OBJECTIVE: The aim of this study is to develop a robust mortality-weighted severe maternal morbidity (SMM) composite measure that provides increased clinical granularity by measuring the individual and combined risk of inpatient mortality across multiple SMM indicators. The enhanced metric better reflects the complexity gradient of individual patients through a weighted SMM (w-SMM) composite score expressed on a continuous scale. METHODS: This retrospective cohort study analyzed 7,174,412 inpatient deliveries from 2016 to 2023 extracted from the Premier Healthcare Database, an all-payer, nati

Near miss maternal morbidity and maternal mortality at Kathmandu Medical College Teaching Hospital.

BACKGROUND: Maternal mortality traditionally has been the indicator of maternal health all over the world. More recently review of the cases with near miss obstetric events has been found to be useful to investigate maternal mortality. Cases of near- miss are those in which women present with potentially fatal complication during pregnancy, delivery or the puerperium, and survives merely by chance or by good hospital care. OBJECTIVES: The objective of this study is to determine the prevalence and nature of near miss obstetric cases and maternal deaths at Kathmandu Medical College Teaching Hos

Maternal cohort, time of stillbirth, and maternal age effects in Italian stillbirth mortality.

STUDY OBJECTIVE: The aim of the study was to investigate factors in stillbirth mortality in Italy. DESIGN: Italian stillbirth data from 1955 to 1979 were analysed using a log-linear Poisson model with arbitrary constraints on the parameters to identify the effects of maternal birth cohort, calendar period of stillbirth and maternal age. SETTING: Stillbirth data for the whole of Italy were obtained from the Italian Central Institute of Statistics for the years under examination. During this period the criteria for defining stillbirth did not change and the quality of birth registration was si

Maternal mortality in southern Africa, 1980-1982. Part II. Causes of maternal deaths.

Of 737 maternal deaths studied in southern Africa between January 1980 and December 1982, 660 were classified as direct obstetric deaths. Hypertensive disorders of pregnancy were the most frequent cause of death (30%). Obstetric haemorrhage and infection were associated with 20% and 19% of maternal deaths respectively. Advancing age and increased parity were strongly associated with death from obstetric haemorrhage.

[MATERNAL MORTALITY IN ISRAEL - ACHIEVEMENTS AND ONGOING CHALLENGES].

Israel has one of the lowest maternal mortality rates globally, at 2.6 per 100,000 births. Most deaths occur between the late second trimester and the postpartum period, primarily due to obstetric complications such as postpartum hemorrhage, sepsis, and amniotic fluid embolism. Unlike the U.S.A., where leading causes include violence and substance abuse, maternal deaths in Israel are predominantly obstetric. A major challenge is distinguishing between preventable and non-preventable deaths. Research suggests that more than 60% of maternal deaths could be avoided with better diagnosis, timely i

[Maternal mortality by stroke in France 2016-2018].

Between 2016 and 2018, twenty maternal deaths were associated with a stroke. The 20 deaths whose main cause was stroke represent 7.4% of all maternal deaths, i.e. a maternal mortality ratio (MMR) of 0.9 per 100,000 live births (95%CI 0.6-1.3). Among the 20 stroke deaths, it was hemorrhagic in 17 cases (85%), ischemic in 2 cases, and due to thrombophlebitis in 1 case. Stroke occurred during pregnancy in 8 women (40%) - one case before 12 weeks, 3 cases between 28 and 32 weeks, and 4 cases between 34 and 40 weeks; in 3 cases the stroke occurred intrapartum, and for the other 9 cases (45%) the st
