Sudden rupture of the internal maxillary artery causing pseudoaneurysm (mandibular part) secondary to subcondylar mandible fracture.

The mandible is one of the most frequently fractured bones of the face. Condylar fractures are the most common mandibular fractures in adults. The potential complications of mandibular fractures are infection, paresthesia, malunion, nonunion, asymmetry, and long-term functional and growth disturbances. In this article, we report an uncommon case of life-threatening pseudoaneurysm of the internal maxillary artery secondary to subcondylar fracture. We believe that the subcondylar fracture on the vertical vector and oblique direction can damage vital tissues such as the internal maxillary artery,

Superselective intra-arterial chemotherapy for advanced maxillary sinus cancer: an evaluation of arterial perfusion with computed tomographic arteriography and of tumor response.

OBJECTIVE: The purpose of our study was to elucidate the relationship between arterial perfusion in advanced maxillary sinus cancer which was opacified by superselective intra-arterial computed tomographic arteriography (IA-CTA) and the tumor response to intra-arterial chemotherapy. METHODS: Superselective IA-CTA was performed to identify the feeding arteries and their perfusion in advanced maxillary sinus cancer in 10 patients. Cisplatin was selectively infused into these feeding arteries, except for the internal carotid artery. RESULTS: The results were assessed in 9 of the 10 patients, an

Ruptured Pseudoaneurysm of the Maxillary Artery and Its Branches Following Le Fort I Osteotomy: Evidence-Based Guidelines.

BACKGROUND: Although rare, pseudoaneurysms (PA) can develop following Le Fort I osteotomy and lead to life-threatening hemorrhage. However, the typical presentation of a PA following a Le Fort I osteotomy is not well characterized. Evidence-based guidelines are not currently available for evaluation of PA following Le Fort I osteotomy. METHODS: A case report is presented of a 27-year-old man who underwent Le Fort I advancement and subsequently developed a bleeding PA. A comprehensive search of journal articles was performed using the MEDLINE/PubMed database between 1964 and April 2016. Keywor

[Maxillary artery embolisation as a method of treatment of hemorrhages in Rendu-Osler-Weber Syndrome--our experience].

The authors have described case of patient suffering from Rendu-Osler-Weber Syndrome for many years, after many unsuccessful attempts to treat massive hemorrhages from the nose. This was what made them use maxillary artery embolisation method. It was done using Seldinger technique. Radiologic visualisation of blood vessels was obtained by use of C-arm. The use of this method in that case has been successful so far. According to authors opinion embolisation of maxillary arteries is an effective alternative in treating recurring hemorrhages in patients with Rendu-Osler-Weber Syndrome.

Saphenous vein graft for bypass of the maxillary to supraclinoid internal carotid artery: an anatomical short study.

The use of a saphenous vein graft for bypass of the maxillary artery (MA) to the supraclinoid internal carotid artery (ICA) in internal carotid occlusions is investigated. Five adult cadaver sides were used. Dissection required zygomatic arch osteotomy and a pterional craniotomy with extensive removal of the floor of the middle cranial fossa. The MA was found easily medial to infratemporal crest. The clinoidal segment of the ICA was exposed with the removal of the anterior clinoid process intradurally. The bypass graft was 4 to 5 cm long and was sutured end-to-end to the MA and end-to-side to

Balloon compression of the intramaxillary sinus for intractable post-traumatic bleeding from the maxillary artery. Case report.

We present a case of severe intractable epistaxis after midfacial trauma in which the bleeding was identified as coming from the descending palatine artery, a branch of the maxillary artery. It could not be controlled by simple packing, and was stopped by inserting a balloon into the maxillary sinus, tamponading the injured vessel in the sphenopalatine fossa (pterigopalatine fossa). We describe an easy and practical emergency manoeuvre to control bleeding from inaccessible branches of the maxillary artery and to prevent rebleeding after embolisation.

Internal Maxillary Artery Preoperative Embolization Using n-Butyl Cyanoacrylate and Pushable Coils for Temporomandibular Joint Ankylosis Surgery.

BACKGROUND: Temporomandibular joint (TMJ) ankylosis causes disability through impaired digestion, mastication, speech, and appearance. Surgical treatment increases range of motion with resultant functional improvement. However, substantial perioperative blood loss can occur (up to 3 L) if the internal maxillary artery (IMAX) is injured as it traverses the ankylotic mass. Achieving hemostasis is difficult because of limited proximal IMAX access and poor visualization. Our aim is to investigate the technical feasibility and preliminary safety of preoperative IMAX embolization in patients undergo

Endovascular embolization to control life-threatening epistaxis due to a post radiotherapy pseudoaneurysm of the maxillary artery.

We report an unusual case (never described) of a false aneurysm of the maxillary artery following irradiation and biopsy that ruptured into the nasopharynx in an 34-year-old man who presented with severe epistaxis 15 months after radiation therapy of the nasopharynx (80 Gy). Angiography revealed a fusiform aneurysm of the second portion of the maxillary artery, which was successfully embolized but the patient died from a pseudomonas septicemia three months later. False aneurysms of the maxillary artery following irradiation are exceptional complications. Despite a successful endovascular treat

Three cases of organized hematoma of the maxillary sinus in patients who underwent preoperative arterial embolization.

Organized hematoma (OH) is benign tumor in the maxillary sinus. The standard treatment for OH is complete surgical resection, however massive bleeding can occur during the procedure, albeit rarely. Some reports have suggested preoperative embolization is useful for reducing the volume of intraoperative bleeding. We report 3 cases of OH in the maxillary performed preoperative embolization. We identified the feeding arteries by angiography or IVR-CT, and we embolized them using Gelatin sponge particles. The embolized artery was the maxillary artery or both the maxillary and the facial artery. Th

Traumatic Pseudoaneurysm of the Internal Maxillary Artery: A Rare Life-Threatening Hemorrhage as a Complication of Maxillofacial Fractures.

Pseudoaneurysm of the internal maxillary artery due to a traumatic event is a rare condition. Pseudoaneurysms are usually directly produced by arteries break with extravasation of blood. The compressed perivascular tissue forms the wall of aneurysmal sac. Then, this sac gradually expands and can be damaged. It is rare to see pseudoaneurysms of IMA. They are usually associated with fracture of the neck of the mandible. To the best of our knowledge the pseudoaneurysm of the internal maxillary artery related to maxillofacial trauma is an event extremely rare in the literature and if not quickly m

Vascular supply with angio-CT for superselective intra-arterial chemotherapy in advanced maxillary sinus cancer.

Recently popularised, the combined angiography and CT (angio-CT) system is useful for correctly identifying the feeding arteries and their perfusion in various organs. We applied this system for advanced maxillary cancer to expose its feeding arteries and their supplying territories. In addition to the maxillary artery, extramaxillary feeding arteries were usually observed, including the ophthalmic, accessory meningeal, facial, transverse facial and ascending palatine arteries. These extramaxillary feeding arteries exhibited uniform tendencies, depending on the site of extramaxillary tumour ex

A new aspect of tri-modal therapy with superselective intra-arterial chemotherapy in maxillary sinus carcinoma.

CONCLUSION: This superselective intra-arterial intervention technique enabled us to administer a higher dose of cisplatinum to the local lesion with a lower incidence of adverse issues by use of neutralizer, compared with the conventional methods. OBJECTIVE: A wide variety of modalities, including surgery, radiotherapy, and chemotherapy, alone or in combination, have been reported for the treatment of maxillary sinus cancer. However, there is still much controversy with regard to the optimum treatment to obtain better local control and acceptable preservation of shape and function. Since 2001

Neoadjuvant intra-arterial chemotherapy combined with radiotherapy and surgery in patients with advanced maxillary sinus cancer.

PURPOSE: The optimal treatment of advanced maxillary sinus cancer has been challenging for several decades. Intra-arterial chemotherapy (IAC) for head and neck cancer has been controversial. We have analyzed the long-term outcome of neoadjuvant IAC followed by radiation therapy (RT) and surgery. MATERIALS AND METHODS: Twenty-seven patients with advanced maxillary sinus cancer were treated between 1989 and 2002. Five-fluorouracil (5-FU, 500 mg/m(2)) was infused intra-arterially, and followed by RT (total 50.4 Gy/28 fractions). A planned surgery was performed 3 to 4 weeks after completion of IA

Arterial supply of maxillary sinus and potential for bleeding complication during lateral approach sinus elevation.

There are 3 arteries that supply the maxillary sinus (i.e., the posterior superior alveolar, infraorbital, and posterior lateral nasal arteries), any of which may be encountered during lateral approach sinus elevation surgery. These arteries are ultimate branches of the maxillary artery. Although it has not been reported, there is a theoretical potential for severing an intraosseously located artery during a vigorous curettage for sinus lining elevation in the posterior medial wall of the sinus. Techniques for cessation of bleeding are discussed, such as electrocautery and endoscopic ligation.
