Restoration of maxillary residual ridge atrophy using Le Fort I osteotomy with simultaneous endosseous implant placement: technical report.

When residual ridge atrophy of the edentulous mandible and maxilla is accompanied by a skeletal Class III jaw relationship, surgical correction is necessary for optimal functional, phonetic, and esthetic results whenever implant-supported prostheses are to be placed. A method is described in which a single procedure combines the endosseous placement of nonsubmerged titanium implants in appropriate prosthetically predetermined locations with defined advancement of the maxilla. Preoperative diagnostic examinations, cast simulation, and transfer of the latter to the site of surgery are of the utm

Malpositioned maxillary anterior implants correction using segmental osteotomy with three-dimensional guide and micro-saw: a technical note.

The maxillary anterior region, often referred to as the "esthetic zone," presents unique challenges in implant dentistry, especially when complications such as implant malposition occur. Malpositioned implants can lead to esthetic concerns, including gingival recession or implant fixture exposure, often resulting from facial placement following immediate implantation after tooth extraction. This case report outlines the successful correction of a facially malpositioned maxillary anterior implant using segmental osteotomy facilitated by three-dimensional (3D) technology and a micro-saw, combine

Identifying Predictors of Acquired Velopharyngeal Insufficiency in Cleft Lip and Palate Following Maxillary Osteotomy Using Multiple Regression Analyses.

BACKGROUND: Maxillary osteotomy is typically undertaken to correct abnormal facial growth in cleft lip and palate. The surgery can cause velopharyngeal insufficiency resulting in hypernasality. This study aims to identify valid predictors of acquired velopharyngeal insufficiency following maxillary osteotomy by using a range of perceptual and instrumental speech investigations and multiple regression. METHODS: A prospective study was undertaken consisting of a consecutive series of patients with cleft lip and palate (N = 20) undergoing maxillary osteotomy by a single surgeon. Participants wer

Concurrent bilateral total temporomandibular joint replacement surgery and conventional maxillary osteotomy utilizing virtual surgical planning web-based technology.

INTRODUCTION: We describe the case of a 31-year-old lady who presented in 1999 with significant temporomandibular joint (TMJ) dysfunction following a previous vertical subsigmoid osteotomy. The previous surgery had displaced both condyles from their glenoid fossae, and a persistent malocclusion resulted. Despite initial conservative management, both condyles remained out of their fossae and she re-presented 10 years later. On presentation, she had significant progressive right TMJ dysfunction with severe pain. A class III malocclusion was apparent with a 5-mm anterior open bite and jaw deviati

Palatal versus vestibular piezoelectric window osteotomy for maxillary sinus elevation: a comparative clinical study of two surgical techniques.

OBJECTIVES: The goal of this study was to compare the surgical advantages and disadvantages of a new palatal access osteotomy for sinus elevation with a conventional lateral approach. STUDY DESIGN: In 32 patients, either a palatal (n = 16) or a lateral (n = 16) osteotomy to the maxillary sinus was performed under local anesthesia. The palatal access included a circular paramarginal incision and elevation of a palatal mucosal flap based on a median pedicle. The lateral access was performed by vestibular standard incision and development of a mucoperiosteal flap with a vestibular and superior b

The ISG viewing wand: an application to atlanto-axial cervical surgery using the Le Fort I maxillary osteotomy.

Surgical access to the clivus and upper cervical spine may be facilitated by a number of approaches. The Le Fort I maxillary osteotomy has been described to give improved access to the skull base for removal of tumours and treating vertebrobasilar aneurysms. We describe a case in which the combination of a particularly high translocation of the body of C2 and poor mouth opening had precluded a standard transoral approach by restricting access to the operative field. Anterior decompression of the body of C2 was therefore performed via a maxillary down-fracture with the aid of an interactive ima

Multifunctional Templates for Minimized Osteotomy, Implantation, and Palatal Distraction with a Mini-Screw-Assisted Expander in Schizodontism and Maxillary Deficit.

PURPOSE: Schizodontism is complete separation of a dental germ. It results in a twin tooth and supernumerary teeth. The treatment of transverse constriction in combination with supernumerary dental germs and impacted central incisors can pose a challenge, especially in young patients, when the number of permanent teeth is not adequate to ensure secure anchorage. The use of navigation templates based on three-dimensional X-ray images allows for precise insertion of temporary mini-implants for the acquisition of palatal distractors. In addition, templates allow for minimally invasive biopsies an

Comparison of two different absorbable membranes for the coverage of lateral osteotomy sites in maxillary sinus augmentation: a preliminary study.

INTRODUCTION: Barrier membranes, both absorbable and non-absorbable, have been used in sinus augmentation for many years. Some years ago, a new autologous blood substrate called Platelet-Rich-Fibrin (PRF) was introduced, and to date, the supporting effect on bone regeneration has been controversial. This study aimed to evaluate the effect of PRF on bone regeneration when used as a barrier membrane at the lateral osteotomy site in sinus augmentation. MATERIAL AND METHODS: Twelve sinuses from six patients requiring bilateral sinus floor augmentation were treated with a two-stage surgical techni

Aesthetic reconstruction of onco-surgical maxillary defects using free scapular flap with and without CAD/CAM customized osteotomy guide.

BACKGROUND: Reconstruction of the maxillary defects following tumor ablation remains to be a challenge for surgeons. Virtual surgical planning (VSP), intraoperative cutting guides and stereolithographic models provides the head and neck reconstructive surgeon with powerful tools for complex reconstruction planning. Despite its use in fibular osteocutaneous reconstruction, application to the scapular free flap has not been as widely reported. The aim of this study was to compare aesthetic results and operation time of free scapular flap, with and without computer-aided design and computer-aided

Autogenous Mandibular Bone Graft for Maxillary Le Fort I Osteotomy Interpositional Gap in Orthognathic Surgery: A Technique Case Series.

Correction of maxillofacial skeletal dental deformities often includes surgical interventions in the maxilla and mandible. Le Fort I maxillary osteotomies are performed to correct maxillary horizontal, vertical, and transverse discrepancies. Repositioning of the maxilla creates an interpositional gap in bone that can lead to pseudoarthrosis, instability, mobility, infection, and eventual relapse. Grafting the interpositional gap with bone creates mechanical stops to prevent relapse, provides a matrix for secondary ossification, accelerates bony union, and inhibits soft tissue herniation. This

Maxillary sinus recovery and nasal ventilation after Le Fort I osteotomy: a prospective clinical, endoscopic, functional and radiographic evaluation.

The condition of the maxillary sinus is not routinely assessed before a Le Fort I osteotomy. Performing this procedure in an infected sinus might account for a considerable proportion of the complications, such as excessive bleeding and sinusitis. The aim of this study was to evaluate the maxillary sinus and nasal ventilation after Le Fort I osteotomy. Twenty patients were evaluated before and 2 months after surgery using validated questionnaires for sinonasal complaints (RSOM-31 and VAS score), nasal endoscopy, peak nasal inspiratory flow (PNIF), and a computed tomography (CT) scan.
