Pharmacist Provision of Hormonal Contraception in the Oregon Medicaid Population.

OBJECTIVE: To describe early utilization of pharmacist prescription of contraception in Oregon's Medicaid program. METHODS: Using Oregon Medicaid claims data, we conducted a retrospective analysis and quantified overall and monthly trends in pharmacist-prescribed contraceptives from January 1, 2016, to December 31, 2017. Our population was restricted to patients obtaining a new prescription for oral and transdermal methods and who had continuous Medicaid coverage during the study period. We summarized demographic and utilization characteristics, including whether patients were continuing or s

Biological and Immunomodulator Use in Crohn's Disease in a Medicaid Population.

BACKGROUND: Immunomodulator and biological use in African Americans (AA) with Crohn's disease (CD) has been reported to be lower than in whites (W); less data exist for Hispanics (H). METHODS: Medicaid databases from 3 states were examined for patients with CD from August 1998 to July 2009. CD-related treatments, comorbidities, location, surgery, and health care utilization were assessed from diagnosis until the first biological claim or end of claims. A Cox proportional hazard regression model was used to assess the effect of race on biological initiation. RESULTS: A total of 5575 patients

Efforts to Improve Perinatal Outcomes for Women Enrolled in Medicaid.

Improving women's health and perinatal health outcomes is a high priority for Medicaid, the jointly financed federal-state health coverage program. The authorities provided by the Affordable Care Act give Medicaid new resources and opportunities to improve coverage and perinatal care. Given that the Medicaid program currently covers almost half of all births in the United States, the Centers for Medicare and Medicaid Services, working in partnership with states and other stakeholders, is using new and existing authorities to improve birth outcomes. Quality measurement, quality-improvement proj

Medicaid coverage of medications to treat alcohol and opioid dependence.

Substance use disorders affect 12% of Medicaid beneficiaries. The prescription drug epidemic and growing need for treatment of alcohol and opioid dependence have refocused states' attention on their provision of substance use disorder treatment services, including medications. This study characterized how Medicaid programs cover these treatment medications. Data were from 2013 Medicaid pharmacy documents, 2011 and 2012 Medicaid state drug utilization records, and a 2013 American Society of Addiction Medicine survey. Results showed that only 13 state Medicaid programs included all medications a

The impact of tobacco dependence treatment coverage and copayments in Medicaid.

BACKGROUND: The Affordable Care Act (ACA) expands Medicaid's tobacco dependence treatment (TDT) coverage; however, these expansions differ in comprehensiveness based on Medicaid eligibility category. PURPOSE: To examine whether more generous Medicaid TDT coverage (in terms of cost-sharing requirements and treatments covered) is associated with greater likelihood of quit attempts and successful quit rates. METHODS: This study used repeated cross-sections from the Current Population Survey (2001-2011), linked to state-level survey data on Medicaid TDT coverage. The sample included 3,071 adult

Implementation of an opioid management initiative by a state Medicaid program.

BACKGROUND: The utilization of prescription opioids has increased over the last 2 decades. Associated with this is the misuse of prescription opioids for nonmedical purposes. Medicaid programs have struggled with developing strategies that balance best practice models, appropriate utilization, and reduction in costs associated with the opioid medication class. OBJECTIVE: To examine the impact of a 2-year stepwise initiative to reduce utilization and therapy costs of long-acting opioid analgesics (LAOA) by addressing issues of high dose, daily dose, and preferred therapeutic alternatives. MET

Differences in the cost of antidepressants across state Medicaid programs.

BACKGROUND: Depression is the most prevalent major mental health disorder, affecting between eight and ten percent of the population in the United States. The U.S. Medicaid programs spent in total over USD 2.3 billion on antidepressant drugs in 2003, across three categories of antidepressants including selective serotonin reuptake inhibitors, tricyclic antidepressants, and others. Each state has its own set of cost-containment policies with respect to antidepressants, as well as other drugs, including preferred drug lists, prior authorization policies, copay systems, drug utilization reviews,

Is there a difference in hypertensive claim rates among Medicaid recipients?

The purpose of this study was to determine whether there are racial differences in the rates of prevalence and new claims to Medicaid for hypertension treatment in a population of uniformly low economic status--i.e., Georgia Medicaid recipients. Age-specific and age-adjusted prevalence rates of hypertension in 1991 and the first 1991 claim rates by race and gender were calculated. Gender-specific black-to-white risk ratios, using the Mantel-Haenszel pooled point estimate (RMH) and the corresponding test-based 95 percent confidence interval (CI) were also calculated. African-American females we

Experiences of Lactation Support Among Medicaid-Insured Mothers in Louisiana.

INTRODUCTION: Louisiana's breastfeeding rates are lower than most states, and even lower among its Medicaid members. Louisiana's Medicaid program does not cover outpatient lactation services, although expanded access to lactation providers could increase breastfeeding exclusivity and duration for women of diverse backgrounds. The purpose of this study was to explore the views of stakeholders in lactation support for Louisiana's Medicaid-insured women and increase our understanding of current barriers and opportunities to improving outpatient lactation support and decrease racial disparities in

Medicaid Expansion Timing and Pancreatic Cancer Resection Rates and Survival.

IMPORTANCE: Pancreatic cancer is among the deadliest malignancies, with 5-year survival estimated at 13%. Medicaid expansion offers the opportunity to assess whether broader insurance eligibility improves outcomes. OBJECTIVE: To evaluate whether state-level Medicaid expansion was associated with reduced mortality and increased surgical resection among adults with pancreatic cancer and whether effects varied across demographic and socioeconomic subgroups. DESIGN, SETTING, AND PARTICIPANTS: This was an observational cohort study using generalized difference-in-differences Cox and logistic regr

Medicaid Expansion and Overall Mortality Among Individuals With Lung Cancer.

INTRODUCTION: Lung cancer is the leading cause of cancer-related mortality in the United States, with outcomes disproportionately worse among uninsured and socioeconomically disadvantaged populations. The Affordable Care Act (ACA), through Medicaid expansion, sought to improve access to care, yet its long-term impact on lung cancer survival remains incompletely defined. The objective of the study was to evaluate the association between Medicaid expansion and overall mortality among adults with lung cancer and to examine whether these effects differed by race/ethnicity and lung cancer histology

Health access and prevention under Arkansas' market-based Medicaid expansion.

INTRODUCTION: Arkansas expanded Medicaid in 2014 using a Section 1115 waiver that included premium assistance for beneficiaries to purchase private plans through the state's marketplace. The waiver was modified in 2017 to require partial premium payments and implemented work requirements in mid-2018, which were later rescinded by federal court order. We compared healthcare access and preventive service utilization in Arkansas vs states with traditional Medicaid expansions. METHODS: Using Behavioral Risk Factor Surveillance System data from 2011 to 2021, we applied difference-in-differences an

Changes in Medication Use During Medicaid Continuous Enrollment and Unwinding.

IMPORTANCE: During the COVID-19 pandemic, Medicaid enrollment increased because states suspended routine eligibility determinations. After this continuous enrollment provision ended in April 2023, millions of US individuals lost Medicaid coverage. OBJECTIVE: To measure how the unwinding of Medicaid enrollment was associated with changes in patients' use of health services, such as prescription medications. DESIGN, SETTING, AND PARTICIPANTS: A cross-sectional study was carried out using interrupted time series analysis to compare changes in quarterly Medicaid enrollment and prescription medic

Amplifying community perspectives in Medicaid policymaking: findings from North Carolina.

INTRODUCTION: In the midst of sweeping federal and state policy changes that will significantly impact Medicaid, effectively integrating community feedback into Medicaid policymaking will require identifying what Medicaid beneficiaries want and need to be healthy. We aimed to identify North Carolina community leaders' and community members' wants, needs, and experiences with Medicaid. METHODS: This qualitative study collected data using semi-structured interviews with 4 leaders of community-based organizations (CBOs), and 6 focus groups with 37 community members held across North Carolina bet

Integrating Cancer Registry and Medicaid Data for Survivorship-Focused Research.

The Ohio Cancer Incidence Surveillance System (OCISS) is not part of the Surveillance, Epidemiology, and End Results (SEER) program; therefore, we are unable to leverage the linked data products using the SEER as the backbone to study survivorship in the state of Ohio. Accordingly, we developed a survivorship-focused data infrastructure integrating 1996-2020 data from the OCISS and 2016-2020 Medicaid data, allowing us to examine health care patterns and outcomes in cancer survivors with short-, medium-, and long-term follow-up, defined as those diagnosed with cancer in the years 2016-2020, 200

Rationing Rights: Administrative Burden in Medicaid Long-Term Care Programs.

CONTEXT: States use Medicaid waivers to provide supports for disabled people in communities rather than in institutions. Because waivers are not entitlements, those deemed eligible are not guaranteed these supports. How do states, in practice, use bureaucratic procedures to ration this "conditional" right? METHODS: Drawing on primary and secondary data, the authors analyze waiver programs and document state administrative procedures that indirectly and directly ration access. FINDINGS: Burdens indirectly limit disabled people's access to Medicaid home- and community-based services via a comp

The ACA Medicaid expansions and abortion rates among young adults.

This paper examines whether the expansion of Medicaid under the Affordable Care Act (ACA), which increases access to contraceptives to low-income childless women and allows them more autonomy to determine the timing of their pregnancies and births, is associated with lower abortion rates during the period 2008-2017. Using state-level data from the Guttmacher Institute and employing a difference-in-differences method, we find that Medicaid expansion is associated with a meaningful reduction in the abortion rate among women ages 18-24, presumably through increased use of contraceptives among low

Did the Medicaid expansion improve immunization among U.S. pregnant women?

OBJECTIVES: Affordable Care Act (ACA) Medicaid expansion increased financial eligibility of low-income individuals. Its effects are mixed on influenza vaccination and unknown for Tetanus, Diphtheria, and Acellular Pertussis (Tdap) vaccination among pregnant women in the United States. We aim to address mixed and missing evidence in literature. METHODS: We merged 44,320 pregnant women from 2011 to 15 Pregnancy Risk Assessment Monitoring System with National Welfare Data by residency.
